Healthy Sleep Practices for Consumers of Home Total Parenteral Nutrition: A Mixed-Methods Community-Based Participatory Study.

BACKGROUND: Consumers of overnight home parenteral nutrition (HPN) often experience sleep disruption; however, existing healthy sleep recommendations are widely inapplicable to consumers. OBJECTIVES: The aim of this mixed-methods, community-based participatory research study was to develop tailored recommendations on healthy sleep practices for HPN consumers. METHODS: The multipart study involved the following: 1) an initial draft of sleep recommendations based on the evaluation of existing general sleep hygiene guidelines by an expert panel of clinicians and consumers with lived experience;

[Total communication in aphasia therapy].

Total Communication Therapy is based on the possibilities which aphasic patients have left to communicate. All aspects of language are trained. The environment stimulates the patient and uses all possible means of communication in order to achieve an optimal level of communication. An evaluation test is being prepared at the moment.

Language outcomes for children with cochlear implants enrolled in different communication programs.

OBJECTIVES: The aims of this study were to (a) compare language outcomes in pediatric cochlear implant users enrolled in three different communication programs: sign and spoken language, auditory-oral, and auditory-verbal therapy, and (b) examine factors influencing language outcomes. METHODS: Post-implant standard scores on language assessments of receptive vocabulary, auditory comprehension, and expressive communication were collected from files of 42 children with prelingual hearing loss who were implanted by 3;6 years of age. Early intervention history, device details, and demographic inf

Use of the alternating treatments design as a strategy for empirically determining language training approaches with mentally retarded children.

The present study was conducted as a practical demonstration of the potential utility of the alternating treatments design (ATD) in determining the most efficacious language training approach with mentally retarded children. Two subjects were chosen who used single words to communicate but who did not combine words to form sentences. Two sentence triads consisting of four words each and rated as equivalent in difficulty were trained. Each triad had one sentence trained using oral speech methods, a second trained using total communication methods, and a third sentence trained using a "modified"

An alternating treatments comparison of oral and total communication training with minimally verbal retarded children.

This study was a comparison of the effects of oral speech with total communication (speech plus sign language) training on the ability of mentally retarded children to repeat 4-word sentences. Three children were chosen who used single words to communicate but who did not combine words into complete sentences. Three sentence pairs were trained, with each pair having one sentence trained using oral methods and an equivalent one trained using the total communication approach. Both training procedures involved chaining sentence parts, reinforcement, and prompting. Oral methods involved presenting

A validated case study of facilitated communication.

The case of a 13-year-old boy with autism, severe mental retardation, and a seizure disorder who was able to demonstrate valid facilitated communication was described. In three independent trials, short stories were presented to him, followed by validation test procedures with an uninformed facilitator providing physical support to the subject's arm. In Trials 1 and 3, several specific answers were provided that clearly indicated that the young man, not the uninformed facilitator, was the source of the information. Moreover, some responses seemed to imply that the subject was employing simple

[New approaches in speech therapy].

Following a brief introduction of the neurological basis and the localization of aphasias, we discuss approaches to language therapy which are based on linguistic theories. We first sketch principles of language-based therapy. Then we introduce communicative aphasia treatment and explain aspects under which the latter represents an extension of the language-based approach. In order to provide neurological arguments in favour of communicative therapy, we discuss the language theory of Hughlings Jackson. Finally, we give examples for communicative therapy paradigms, for some of which implementat

Establishing the manding function of communicative gestures with individuals with severe/profound mental retardation.

Procedures for teaching communicative gestures to individuals with mental retardation are focused mostly on establishing requesting or manding. It is often taken for granted that when a student has been taught to make a request, that he or she is indeed manding. Five students with severe/profound mental retardation accepted referents, regardless of whether the referents matched the gestures they made. Apparently, their responding could not be defined as manding. A correction procedure, including interruption, physical guidance, verbal instruction, and repetition, was then applied and increased

Evaluating facilitated communications of people with developmental disabilities.

A quasi-experimental message-passing procedure was used to assess the validity of the facilitated communication (FC) by people with autism and mental retardation or with mental retardation. The 23 participants were classified as having intellectual skills within the range of severe to profound mental retardation. Message-passing consisted of showing and verbally labeling a picture of a familiar object with the facilitator absent, and subsequent facilitation to generate a label or description of the object. Three-trial blocks were conducted with each participant on two different days. Blocks we

Literacy learning in users of AAC: A neurocognitive perspective.

The understanding of written or printed text or discourse - depicted either in orthographical, graphic-visual or tactile symbols - calls upon both bottom-up word recognition processes and top-down comprehension processes. Different architectures have been proposed to account for literacy processes. Research has shown that the first steps in perceiving, processing and deriving conceptual meaning from words, graphic symbols, manual signs, and co-speech gestures or tactile manual signing and tangible symbols can be seen as identical and collectively (sub)activated. Results from recent brain resea

Predicting progress in Picture Exchange Communication System (PECS) use by children with autism.

BACKGROUND: The Picture Exchange Communication System (PECS) is a widely used communication intervention for non-verbal children with autism spectrum disorder. Findings for the benefits of PECS have almost universally been positive, although there is very limited information about the characteristics of PECS users that determine the amount of progress that they are likely to make. AIMS: To explore the utility of using children's developmental age to predict the subsequent degree of progress using PECS. METHODS & PROCEDURES: In a retrospective study, 23 non-verbal 5- and 6-year-old children w

[Combined verbal and sign language for promoting communication development in non-verbal or or pre-verbal children and adolescents with mental retardation].

Conveying alternative and augmentative approaches to communication is becoming necessary for increasing numbers of persons with mental retardation with no or poor oral skills. The preverbal and nonverbal forms of communicating, hence, have special relevance in a developmental context. A natural extension of gestic messages, it in particular is the use of signing that is apt to enhance speech and language acquisition and bring about distinct improvement in understanding and making oneself understood. Exchanging messages by sign language can reveal to the child the significance of his/her own be

[Employment and communication of hearing disabled patients].

Communicative disorders can remain an uncomfortable, costly, and debilitating factor throughout the lives of millions of children and adults. Deafness is a common problem and no age group is spared. Causes of hearing loss are numerous, and may be multifactorial in an individual. Important causes include trauma, including noise and air pressure changes; genetic hearing loss; infection, including rubella; drug damage, metabolic and neoplasia. Prompt detection and management of hearing loss in children is essential to ensure adequate development of language and associated skills. Untreated hearin

Augmentative and alternative communication for preschool children: intervention goals and use of technology.

This study sought to describe speech-language interventions for preschool-aged children who required augmentative and alternative communication (AAC) as provided by AAC experts and by general speech-language pathologists who were not AAC experts. The study also examined the types of technology used in AAC intervention by AAC experts. A retrospective chart review was conducted in which clinic records of 38 preschool-aged children who received expert AAC services were examined. Results showed that interventions provided to the children by general speech-language pathologists (who were not AAC ex

On what basis hope? Modern progress and postmodern possibilities.

Modern and postmodern versions of hope as they apply to services for persons labeled as having mental retardation were examined. Proponents of modernism construct hope as relying on an ever-improving science to accurately comprehend mental retardation and other disabilities and the effectiveness of professional interventions. This myth of scientific progress is traced in various forms through American intellectual history to the development of special education as interventionist social science. Advocates of postmodernism cast doubt upon the grand narrative of modernism and critique modern soc

Effects of varying lengths of synthetic speech output on augmented requesting and natural speech production in an adolescent with Klinefelter syndrome.

Students with developmental disabilities and limited or no functional speech often use speech-generating devices. While the speech-output function of such devices is considered to have potential advantages, it is unclear whether the length of synthetic speech output influences augmented communication and natural speech production. To this end, we describe a two-phase study involving an adolescent with Klinefelter syndrome. In Phase 1, the frequency of augmented requests and natural speech were compared under three speech-output conditions (no-output, short-output, and long-output). In Phase 2,

Reported use of communication strategies by SHHH members: client, talker, and situational variables.

Two hundred and twelve members of the Self Help for Hard of Hearing People (SHHH) organization completed a survey that evaluated their use of communication strategies. Reported use of strategies was correlated with attitudinal variables and social-interaction indices. On average, subjects agreed most strongly with questionnaire items stating they would ask a talker to repeat a misperceived utterance. They agreed less strongly with items stating they would ask the talker to restructure or elaborate an utterance. Subjects agreed strongly that they would use the communication strategies with fami
