Differences in Services Offered by Certified Community Behavioral Health Clinics and Community Mental Health Centers.

OBJECTIVE: This study examined differences between certified community behavioral health clinics (CCBHCs) and community mental health centers (CMHCs) in the services offered and populations served. METHODS: Data from the 2020 National Mental Health Services Survey were used to quantify the proportion of CCBHCs (N=336) and CMHCs (N=1,953) that offered services and served populations described in the CCBHC certification criteria. RESULTS: A higher proportion of CCBHCs than CMHCs offered crisis services, peer support, substance use disorder treatment, treatment for co-occurring disorders, antip

Mental health literacy among nurses at community health centers in rural Rwanda.

AIM: To assess mental health literacy and its association with professional variables among community health center (CHC) nurses. BACKGROUND: Mental health problems are highly prevalent in post-conflict countries, but access to treatment is acutely limited. The integration of mental health care into primary healthcare (PHC) is one of the key recommended strategies to scale up access to treatment. However, little is known about PHC nurses' mental health literacy (MHL). METHOD: Seventy-four nurses (46 women, 28 men) were recruited from 13 CHCs in the western province of Rwanda. Participants an

Integrated Pharmacies at Community Mental Health Centers: Medication Adherence and Outcomes.

BACKGROUND: Patients receiving psychiatric services at community mental health centers (CMHCs) are often prescribed medication that is critical to the treatment of behavioral health conditions, including schizophrenia, bipolar disorder, anxiety, and depression. Previous studies have shown correlation between rates of medication adherence and risk of hospitalization, but potential differences in medication adherence and other outcomes for patients of CMHCs by pharmacy type have not been widely studied. OBJECTIVE: To determine potential benefits of placing a pharmacy within a mental health serv

Service delivery patterns for adults with mild mental retardation at community mental health centers.

Results of a national survey on mental health services for adults with mild mental retardation sent to directors of psychological services at community mental health centers was discussed. In general, adults with mild mental retardation were less likely to receive the full array of available services. Individuals with mental retardation were not frequent consumers of community mental health services and received differential treatment at these Centers. Implications of these findings were discussed.

Administrative Burdens as Distinct Barriers to Accessing Mental Health Services in Community Mental Health Centers and Federally Qualified Health Centers: A Mixed-Methods Assessment.

Increasingly, adolescents are struggling with poor mental health outcomes, making it essential to improve access to high-quality mental health services. Community Mental Health Centers (CMHCs) and Federally Qualified Health Centers (FQHCs) act as key "safety-net" health centers for low-income youth seeking mental health services, as the majority accept Medicaid. This study examines how administrative burdens, i.e., challenges citizens face when interacting with a government agency, may act as barriers to accessing mental health services, especially for adolescents and their caregivers. This ex

Factors influencing implementation of smoking cessation treatment within community mental health centers.

OBJECTIVE: Consumers with serious mental illness smoke more and are at higher risk for smoking-related illness. We examined provider and consumer factors influencing the implementation of the evidence-based "5 A's" (ask, advise, assess, assist, arrange) in six community mental health centers in greater Baltimore. METHODS: Data collected as part of a larger study examining the effectiveness of delivery of the 5 A's at patient visits. First, we examined responses to a survey administered to 49 clinicians on barriers and attitudes toward delivering the 5 A's. Second, we used multilevel models to

Trauma history screening in a community mental health center.

OBJECTIVE: This study assessed the lifetime prevalence of traumatic events among consumers of a community mental health center by using a brief trauma screening instrument. This study also examined the relationship between trauma exposure and physical and mental health sequelae and determined whether the routine administration of a trauma screening measure at intake would result in increased diagnoses of posttraumatic stress disorder (PTSD) and in changes in treatment planning in a practice setting. METHODS: A 13-item self-report trauma screening instrument, a shortened version of the Trauma

Metabolic Syndrome Screening in People With Severe Mental Illness: Results From Two Spanish Community Mental Health Centers.

BACKGROUND: The excess of mortality in people with severe mental illness is due to unnatural causes such as accidents or suicides and natural causes such as metabolic syndrome. The presence of modifiable risk factors like tobacco consumption increases cardiovascular and metabolic risk. AIMS: The purpose of this study was to identify the prevalence of metabolic syndrome and other cardiovascular risk factors in people with severe mental illness. This study also aimed to identify the prevalence of patients receiving treatment for any metabolic syndrome risk factor. METHOD: A cross-sectional descr

Evaluation of psychodynamic psychotherapy in a community mental health center.

This study describes an evaluation of the effectiveness of psychodynamic psychotherapy provided in an outpatient community mental health clinic. The study used a single group pretest-posttest design involving 78 clients. Clinical outcomes included overall psychosocial functioning and quality of life, level of subjective distress, interpersonal functioning and role functioning, measured by the Outcome Questionnaire (Lambert, Hansen, Umpress, Lunnen Okilshi, & Burlingame, 2000). Clients showed statistically significant improvement from pretest (first sessions) to completion of treatment in overa

Smoking Cessation and Electronic Cigarettes in Community Mental Health Centers: Patient and Provider Perspectives.

Little is known about patients' electronic cigarette use, interest in and use of smoking cessation treatments, and providers' attitude towards such treatment. We assessed patients (N = 231) and providers (45 psychiatrists, 97 case workers) in four Community Mental Health Centers. Interestingly, 50% of smokers reported interest in using electronic cigarettes to quit smoking, and 22% reported current use. While 82% of smokers reported wanting to quit or reduce smoking, 91% of psychiatrists and 84% of case workers reported that patients were not interested in quitting as the lead barrier, limitin

PERSPECTIVE: Community Health Centers: The Missed Opportunity to Widely Integrate Mental and Primary Health Care in France.

BACKGROUND: Access to mental health care and its integration with primary care remain critical challenges worldwide. In France, these problems are compounded by fragmented provision, poor coordination, and limited reimbursement for psychotherapy, despite the country's high levels of health expenditure. AIMS: This perspective examines how community health centers (centres de santé) can contribute to integrated care, understood as the systematic coordination of physical and mental health services across providers. We analyze the case of the Centre de Santé de Belleville in Paris as an illustrat

Characteristics of children with autism spectrum disorders who received services through community mental health centers.

Despite the presence of significant psychiatric comorbidity among children with autism spectrum disorders (ASDs), little research exists on those who receive community-based mental health services. This project examined one year (2004) of data from the database maintained by 26 community mental health centers (CMHCs) in the Midwestern US state of Kansas. Children with autism were compared to children with other ASDs - Asperger's disorder, Rett's disorder, and PDD-NOS. Children with autism predictably received more special education services than children with other ASDs, while the latter were

The patient care crisis in community mental health centers: a need for more psychiatric involvement.

The authors examine the role of psychiatrists in community mental health centers and suggest that extensive psychiatric involvement is needed to ensure proper patient care. Within the context of four basic clinical models--social, psychological, behavioral, and biomedical--they describe the psychiatrist's unique clinical skills in history taking, differential diagnosis, case formulation, use of psychopharmacologic agents, and provision of leadership in patient care. Five case reports illustrate instances in which psychiatric intervention corrected serious problems in diagnosis and treatment th

Violence and mental disorders. A retrospective study of people in charge of a community mental health center.

BACKGROUND: Numerous studies conducted in inpatient settings have highlighted how mental disorders are associated with an increased risk of violence, particularly during acute phases. However, to date a more limited number of studies have been performed to assess the risk of violence in outpatients, particularly in Italy. The present study aims to evaluate the prevalence of violent events in a sample of patients in charge of a community mental health center in Italy. METHODS: Based on data obtained from standardized clinical records, a retrospective study was undertaken to investigate acts of

Beyond the emergency: Centering mental health in community disaster preparedness and recovery.

BACKGROUND: Extreme weather events are increasing in frequency and severity, disproportionately affecting marginalized and under‑resourced communities. In Houston, Texas, where insurance coverage and mental health access remain limited, successive disasters compound existing inequities, leaving residents with unmet psychological needs. Yet little is known about how chronically impacted communities conceptualize their mental health experiences, barriers to accessing care, and priorities for support following repeated disaster events. METHODS: We conducted a qualitative study using town hall-st

The relationship of personal financial status to the utilization of community mental health centers in rural Appalachia.

As a follow-up to an aggregate study of the relationship of economic factors to the utilization of mental health facilities, this study, using individual data, attempted to determine the associative strengths of income factors--welfare and unemployment--for utilization of community mental health centers (CMHCs) in rural Appalachia. On the basis of the earlier study and of previous studies of social class and utilization of CMHCs it was hypothesized that for this sample of 151 clients of eight CMHCs in Ohio and West Virginia, unemployment and welfare factors would be most strongly associated wi

A comparative study of neurotics seen in a community mental health center and in private practice.

Differences that emerge in comparisons of persons applying for psychiatric care at a mental health center with those applying to the private sector are comfounded by marked diagnostic differences beyond obvious social class differences. To circumvent that problem and to ascertain what differences, if any, persist after diagnosis is held constant, the authors examined demographic and psychosocial characteristics of neurotics in a one-year adult applicant population, the only major diagnostic group treated in large numbers and similar proportions by the center and the private sector. Findings sh

Preventive counseling for chronic disease: missed opportunities in a community mental health center.

BACKGROUND: The tremendous burden of cardiovascular risk among persons with serious mental illness underscores a critical need for prevention. Counseling by primary care clinicians increases patient smoking cessation, physical activity, and the consumption of fruits and vegetables. The extent to which community mental health clinicians counsel about cardiovascular risk factors has not been reported. METHODS: This cross-sectional study examines the rates of counseling about cardiovascular risk factors by mental health providers at an urban community mental health center (n = 154). Logistic reg

Sociodemographics, clinical characteristics, and life stressors of community-dwelling suicidal risk and nonsuicidal risk elders referred to a community aging and mental health provider were compared in this study. Information was collected through case manager surveys and agency records on 683 older adults referred to the Elder Services Program of Spokane Mental Health in 1994 and the first 6 months of 1995. This sample included 109 individuals who were clinically judged to be at suicide risk by case managers at the time of initial assessment. Comparisons between suicidal risk and nonsuicidal
