[Antibiotic resistance and community-acquired infections].

The selective pressure of extended broad-spectrum antibiotic administration is leading to a progressively growing selection of common gram-positive and gram-negative organisms which show unpredictable in vitro antimicrobial susceptibility levels to current first-line therapeutic choices. This phenomenon is examined from an epidemiological, microbiological, therapeutical, and pharmacoeconomic point of view, focusing our attention on the most common community-acquired infectious diseases interesting upper and lower airways, genito-urinary and gastrointestinal tract, skin and soft tissues. Community-acquired methicillin-resistant Staphylococcus aureus infections.

Staphylococcus aureus causes a variety of minor diseases but also is responsible for staphylococcal pneumonia and sepsis, both of which can be fatal. It is thought to be responsible for many of the pneumonia deaths associated with the influenza pandemics of the 20th century. The introduction of penicillin in the 1940s greatly improved the prognosis for patients with severe staphylococcal infections. However, after a few years of clinical use, most staphylococcal strains were able to hydrolyze penicillin by producing b-lactamases, making penicillin a useless antibiotic to treat staphylococcal i

Pulmonary Infections in Adults: Community-Acquired Pneumonia.

Community-acquired pneumonia (CAP) is a common and potentially serious illness, particularly in older patients and those with significant comorbidities. Recent evidence indicates diverse communities of microbes reside within the alveoli as part of the lung microbiome and may play a role in the development of pneumonia. A CAP diagnosis is based on the demonstration of a new infiltrate on imaging in a patient with symptoms and signs of pneumonia. Although vaccination has decreased its incidence, Streptococcus pneumoniae (pneumococcus) remains the most common bacterial cause of CAP. Macrolide res

Community-acquired methicillin-resistant Staphylococcus aureus infections.

Methicillin-resistant Staphylococcus aureus (MRSA) should no longer be regarded as a strictly nosocomial pathogen. During the past decade, community-acquired MRSA (CA-MRSA) infections among young persons without healthcare-associated (HCA) risk factors have emerged in several areas worldwide. These infections are caused by strains that almost exclusively carry the staphylococcal cassette chromosome mec type IV element and the Panton-Valentine leukocidin genes and, unlike HCA-MRSA strains, are not multiresistant. Although the majority of CA-MRSA infections are mild skin and soft tissue infectio

Viral infections in children with community-acquired pneumonia.

Viral pathogens are commonly isolated from children with community-acquired pneumonia (CAP). Viruses like respiratory syncytial virus, human rhinovirus, human metapneumovirus, parainfluenza viruses, and influenza may act as sole pathogens or may predispose to bacterial pneumonia by a variety of mechanisms. New, emerging, or reemerging viral pathogens occasionally cause outbreaks of severe respiratory tract infection in children. The 2009-2010 H1N1 influenza virus pandemic resulted in increased rates of influenza-related hospitalizations and deaths in children. Rapid viral diagnostic tests base

Community-acquired infections and their association with myeloid malignancies.

INTRODUCTION: Antigenic stimulation is a proposed aetiologic mechanism for many haematological malignancies. Limited evidence suggests that community-acquired infections may increase the risk of acute myeloid leukaemia (AML) and myelodysplastic syndrome (MDS). However, associations with other myeloid malignancies including chronic myeloid leukaemia (CML) and myeloproliferative neoplasms (MPNs) are unknown. MATERIALS AND METHODS: Using the Surveillance, Epidemiology and End Result (SEER)-Medicare database, fourteen community-acquired infections were compared between myeloid malignancy patients

Clarithromycin extended-release in community-acquired respiratory tract infections.

Clarithromycin extended-release (ER) is a second-generation macrolide with bactericidal activity against a broad group of pathogens. It allows for convenient once-daily dosing (2 x 500 mg/day), and has less severe adverse effects than the immediate-release (IR) formulation of the drug, which may result in improved patient compliance. Clarithromycin ER has been approved for the treatment of community-acquired pneumonia, acute maxillary sinusitis and acute bacterial exacerbation of chronic bronchitis. In comparison to the IR formulation, clarithromycin ER demonstrates prolonged absorption from t

[Cefditoren and community-acquired lower respiratory tract infections (corrected)].

Cefditoren is a third-generation oral cephalosporin with good activity against respiratory tract pathogens, including penicillin-intermediate and -resistant strains of S. pneumoniae, and beta-lactamase producing strains of H. influenzae and M. catarrhalis. Its bacterial activity, measured by minimum inhibitory concentration (MIC), is similar or superior to that of many other commonly used antibiotics (penicillins, cephalosporins and fluoroquinolones). Considering the target attainment of T > MIC of >or= 40% a more reliable predictor of clinical and microbiologic outcomes, cefditoren covers str
