INTRODUCTION: Validity is a term that carries significant weight within health professions education (HPE), particularly with regard to learner assessment. Validity theory has changed significantly over the last century, transforming in concert with education, measurement and assessment practices. Most recently, HPE has adopted the conceptualisation of validity as an argument supporting the interpretation (and sometimes uses) of assessment data. The spread of competency-based education and programmatic assessment has introduced new tensions and opportunities for operationalising argument-based

There is growing recognition within the medical education community that medical education in this country needs to be changed to better prepare doctors for the challenges they will face in providing their patients high-quality medical care. A competency-based medical education (CBME) approach was endorsed by the Accreditation Council for Graduate Medical Education and the American Board of Medical Specialties approximately 15 years ago, and a self-designated group-the International Competency-Based Medical Education (ICBME) Collaborators-is now calling on members of the medical education comm

BACKGROUND: Although AACN standards recognize communication as vital for establishing nurse competency, formal educational materials and direction for teaching and assessing communication are lacking.
PURPOSE: This article explores current implementation efforts and the development and preliminary testing of a competency-based framework for communication.
METHODS: A three-step process was conducted: (a) a national survey of nursing programs on communication implementation; (b) development of competency-based communication learning objectives; and (c) a scaffolded learning approach to teachin

This Foreword introduces the Advancing Competency-Based Health Professions Education (CBHPE) collection of papers. These articles seek to synthesizing practical guidance for CBHPE implementation at a time when CBHPE is rapidly expanding across the globe. Together, the papers in this series highlight that meaningful CBHPE implementation requires attention to adaptive change, fidelity to core components, relational work among learners and supervisors, responsiveness to local context, and systematic study of implementation processes and outcomes.

OBJECTIVE: To assess ophthalmology program directors' (PDs) knowledge, attitude, and perceptions regarding the adoption of Competency-Based Medical Education (CBME) and Entrustable Professional Activities (EPAs) in residency training.
DESIGN: Cross-sectional study.
SETTING: An anonymous online survey was distributed across the Association of University Professors of Ophthalmology (AUPO) PDs directory over a two-month period. The survey assessed familiarity with CBME and EPAs, perceived barriers to implementation, and opinions on essential competencies for ophthalmology training.

The educational alliance describes a framework for the trusting relationship between trainee and supervisor as they engage in meaningful, effective feedback. This framework emphasizes interpersonal relationships, shared goals, and agreement on actionable steps to achieve these goals. In this theory-informed conceptual analysis, the authors propose that the educational alliance logically extends to support and enhance competency-based medical education and health professions education, collectively known as competency-based health professions education (CBHPE). Authors describe how key features

Holistic care models such as primary palliative care offer individuals a coordinated, interprofessional and compassionate approach in any healthcare setting regardless of condition. Currently, palliative and end-of-life nursing care content is either threaded throughout nursing curricula or not included at all. As a result, new nurses report being inadequately prepared to provide quality palliative or end-of-life care in practice (Parekh de Campos et al., 2022; Lippe, 2019). A team of expert nurse faculty at a large public university utilized the RE-AIM framework to describe the development an

Pharmacy education stands on unstable ground; overloaded curricula, eroding professional identity, and widening gaps between training and practice signal a system under pressure. Though other health professions are moving toward coordinated, competency-based transformation, pharmacy education risks falling behind. Drawing on recent stakeholder-informed studies and international precedents in medical and veterinary education, this commentary proposes Competency-Based Pharmacy Education (CBPE) not as reform but as essential infrastructure, a levee that must be constructed before pharmacy educati

PURPOSE: Within competency-based medical education, self-regulated learning (SRL) requires residents to leverage self-assessment and faculty feedback. We sought to investigate the potential for competency-based assessments to foster SRL by quantifying the relationship between faculty feedback and entrustment ratings as well as the congruence between faculty assessment and resident self-assessment.
MATERIALS AND METHODS: We collected comments in (a) an emergency medicine objective structured clinical examination group (objective structured clinical examinations [OSCE] and emergency medicine OS

This Scholarly Perspective frames workplace-based training in medical education as a succession concept and explores how the benefits of the traditional apprenticeship model can integrate with modern competency-based medical education. In 2024, the author broadened the construct of medical competence into 3 semi-hierarchical layers: canonical, contextual, and personalized. Canonical competence (the canon that all professionals should know) relates to knowledge and skills that meet generalized, context-independent standards, assessed with methods of high psychometric quality. Personalized compe

Health professions education is experiencing a paradigm shift, from a focus on the structures and processes of education to a focus on the outcomes health professionals must demonstrate to meet the healthcare needs of the public served. The Competency-based Health Professions Education (CBHPE) movement now has two decades of implementation experience in some contexts. Educators across health professions working on the design, planning, implementation, or maintenance of competency-based systems can benefit from the lessons learned through implementation efforts to date. In this manuscript, the

Competency-based health professions education (CBHPE) has transformed learner education through explicit competencies, sequenced progression, direct observation, coaching, and programmatic assessment. However, faculty development-the mechanism that should prepare educators to enact CBHPE-has not undergone a parallel evolution. This misalignment limits the effectiveness and sustainability of CBHPE. In this paper, the authors propose a model of Competency-Based Faculty Development (CBFD) that applies CBHPE design principles to the growth of educators and introduces the Master Adaptive Teacher (M

It is generally assumed that competency-based health professions education (CBHPE) is a positive and ongoing educational reform. However, the assumption that CBHPE naturally leads to more equitable outcomes has not been thoroughly examined. This 'Eye Opener' identifies global challenges in implementing equitable CBHPE, with particular attention to majority-world (low- and middle-income) contexts. These tensions include: 1) variable approaches to adoption that limit transferability, 2) uneven infrastructure for implementation and innovation, 3) export of predeveloped conceptual frameworks that

Competency based medical education (CBME) is an outcome-driven training model that assesses individual learners based on competencies and observable abilities. Although implementation of competencies and milestones by the Accreditation Council of Graduate Medical Education has provided some ground work, full adoption of CBME is in its infancy in emergency medicine. Successful implementation of CBME will require dedicated attention to faculty and learner development; however, there is limited data or best practices available for guidance. To address this need, the Society of Academic Emergency

INTRODUCTION: Nursing faculty and academic leaders are embracing a transformative shift to competency-based education (CBE) pedagogies, guided by the 2021 AACN Essentials. This remodeling in nursing education is designed to better prepare practice-ready nurses who can effectively meet the complexities of today's healthcare landscape.
METHODS: A faculty-led project team utilized a Logic Model framework and partner-centered approaches to plan, implement, and evaluate innovative CBE teaching and learning strategies for undergraduate accelerated Bachelor of Science in Nursing (BSN) students. Pilo

BACKGROUND: The pace of implementation of competency-based medical education (CBME) in postgraduate surgical training has varied substantially across settings, due in part to inconsistent definitions of what constitutes CBME in practice. Employing the lens of Van Melle et al.'s 2019 codification of the core components of CBME, this scoping review aims to characterize how CBME has been implemented in surgical residency programs worldwide.
METHODS: Embase, PubMed, MedEdPortal, and ERIC were searched for English-language articles describing active CBME implementation in surgical residency progra

Dr. David Price and his colleagues, in their article in this issue of Academic Medicine, summarize the findings from recent studies regarding the association of Maintenance of Certification and physicians' learning and improvements in care. Their evaluation demonstrates that physicians often changed their practice because of these educational interventions. In this Invited Commentary, the author argues that, although this finding is reassuring, it may be the right answer to the wrong question. The critical questions the profession faces are whether physicians have the humility to routinely sub

Microbiology is fundamental to veterinary medicine, providing the scientific basis for infectious disease diagnosis, antimicrobial stewardship, vaccination, bio-security, zoonotic disease management, and public health. Despite its importance, little information exists regarding how pre-clinical veterinary microbiology is taught or how curricular content aligns with competency-based veterinary education (CBVE). A survey conducted in collaboration with the American Association of Veterinary Medical Colleges (AAVMC) examined curricular structure, faculty composition, instructional methods, and co

BACKGROUND: Pediatric musculoskeletal radiographs (pMSK) are the primary imaging modality for evaluating injured children, yet there is no widely adopted, standardized approach to teach and assess this specific skill among orthopaedic surgery postgraduate trainees (OS-PGT). We evaluated an online educational and assessment intervention using pMSK radiographs to improve fracture/dislocation detection and risk stratification among OS-PGT.
METHODS: We conducted a multicenter prospective cohort study using a web-based educational intervention, recruiting a convenience sample of OS-PGT. The interv

Competency-based medical education (CBME) is driving a paradigm shift in anesthesiology graduate medical education, moving from fixed-duration training to assessment-centered models. This narrative review aims to synthesize the sources of validity evidence for existing assessment tools (milestones, entrustable professional activities, and workplace-based assessments), equity threats during evaluation and their intervention strategies, and the impact of faculty readiness on assessment quality.
