Computerized adaptive testing of neurocognitive deficits in psychosis.

Computerized adaptive testing (CAT) is a technology that has been used to improve the precision and efficiency of individual difference estimates for cognitive, achievement, personality, and clinical symptom assessments. Motivated by the psychometric limitations of experimental cognitive tasks, we determined whether experimental cognitive tasks, such as those being developed for the National Institute of Mental Health's Research Domain Criteria initiative and similar projects, could also benefit from CAT. Participants were 64 patients diagnosed with psychosis spectrum disorders and 50 healthy

Computerized Adaptive Diagnosis and Testing of Mental Health Disorders.

In this review we explore recent developments in computerized adaptive diagnostic screening and computerized adaptive testing for the presence and severity of mental health disorders such as depression, anxiety, and mania. The statistical methodology is unique in that it is based on multidimensional item response theory (severity) and random forests (diagnosis) instead of traditional mental health measurement based on classical test theory (a simple total score) or unidimensional item response theory. We show that the information contained in large item banks consisting of hundreds of symptom

Maximin criterion for item selection in computerized adaptive testing.

In computerized adaptive testing (CAT), information-based item selection rules (ISRs), such as maximum Fisher information (MFI), often excessively rely on discriminating items, leading to unbalanced utilization of the item pool. To address this challenge, the present study introduced the MaxiMin Information (MMI) criterion, which is grounded in decision theory. MMI calculates each item's minimum information (Imin) within the current confidence interval (CI) of the trait level, selecting the item with the maximum Imin to be administered. For examinees with broader CIs (less precise trait estima

Computerized Adaptive Testing for Patient Reported Outcomes in Ankle Fracture Surgery.

BACKGROUND: Advantages of using computerized adaptive testing (CAT) include decreased survey-burden, diminished floor and ceiling effect, and improved ability to detect the minimal clinical significant difference (MCID) among patients. The goal of this study was to compare the legacy patient-reported outcome measures (PROMs) to the Patient-Reported Outcomes Measurement Information System (PROMIS) scores in terms of ability to detect clinically significant changes in patients who have undergone surgery for ankle fractures. METHODS: Patients who underwent osteosynthesis for an unstable ankle fr

Interactions Between Termination Criteria and Ability Estimators in Computerized Adaptive Testing.

Computerized adaptive testing (CAT) aims to optimize measurement by tailoring item administration to individual examinees. The efficiency and precision of a CAT heavily depend on the choice of ability ( θ ) estimator and the termination criterion (stopping rule). Prior research suggests these components interact, but comprehensive evaluations across varying item bank characteristics remain limited. This simulation study investigated the interactive effects of four θ estimators (maximum likelihood [MLE], weighted likelihood [WLE], maximum a posteriori [MAP], and expected a posteriori [EAP]) a

ROAR-CAT: Rapid Online Assessment of Reading ability with Computerized Adaptive Testing.

The Rapid Online Assessment of Reading (ROAR) is a web-based lexical decision task that measures single-word reading abilities in children and adults without a proctor. Here we study whether item response theory (IRT) and computerized adaptive testing (CAT) can be used to create a more efficient online measure of word recognition. To construct an item bank, we first analyzed data taken from four groups of students (N = 1960) who differed in age, socioeconomic status, and language-based learning disabilities. The majority of item parameters were highly consistent across groups (r = .78-.94), an

Development and validation of a computerized adaptive testing for borderline personality disorder.

BACKGROUND: Borderline personality disorder (BPD) is characterized by a high prevalence rate, frequent comorbidities, and complex manifestations. The challenges associated with evaluating BPD underscore the need for efficient assessment tools. METHOD: To facilitate efficient BPD screening in the general population, this study developed a computerized adaptive test for BPD (CAT-BPD) using a Chinese sample (N = 1,097). The CAT-BPD item bank was constructed from several widely used BPD scales and calibrated using item response theory (IRT). A CAT simulation was subsequently conducted using the e

Overview and current management of computerized adaptive testing in licensing/certification examinations.

Computerized adaptive testing (CAT) has been implemented in high-stakes examinations such as the National Council Licensure Examination-Registered Nurses in the United States since 1994. Subsequently, the National Registry of Emergency Medical Technicians in the United States adopted CAT for certifying emergency medical technicians in 2007. This was done with the goal of introducing the implementation of CAT for medical health licensing examinations. Most implementations of CAT are based on item response theory, which hypothesizes that both the examinee and items have their own characteristics

Computerized adaptive testing for PRWHE measurements using domains of pain and motor function.

The Patient-Rated Wrist and Hand Evaluation is an outcome measure for patients with conditions affecting the wrist or hand. We evaluated the structural validity of the Patient-Rated Wrist and Hand Evaluation using psychometric techniques, then developed computerized adaptive testing algorithms. Factor analysis found two health constructs consistent with 'Pain' and 'Motor Function'. Two computerized adaptive tests were developed, which reduced the number of items from five to a median of one ('Pain'), and from 10 to two ('Motor Function'). Both computerized adaptive tests achieved high levels o

Leveraging computerized adaptive testing for cost-effective evaluation of large language models in medical benchmarking.

Large language models (LLMs) are increasingly applied in healthcare, yet their evaluation relies predominantly on static benchmarks that are costly, contamination-prone, and lack calibrated measurement properties for fine-grained performance tracking. We developed and validated a computerized adaptive testing (CAT) framework grounded in item response theory to enable scalable, psychometrically rigorous assessment of standardized medical knowledge in LLMs. A two-phase study comprising Monte Carlo simulations and empirical evaluation of 38 LLMs was conducted between July and September 2025.
