Two critical confounding factors in periodontal epidemiology.

The prevalence and severity of periodontitis is considerably lower than was previously estimated. The available epidemiological methods are based on the premise that loss of periodontal attachment is a unique sign of periodontitis. However, additional factors resulting in loss of periodontal attachment confound data obtained from modern studies. The physiological process of continuous tooth eruption results in loss of periodontal attachment over a lifetime, at an approximate rate of 0.1 mm/year. Anthropological and clinical evidence for continuous tooth eruption is reviewed. The position of th

Oral contraceptive use and venous thromboembolism: a consideration of the impact of bias and confounding factors on epidemiological studies.

Recent epidemiological studies have reported an approximately two-fold, significant, increased odds ratio for venous thromboembolism in users of third-generation oral contraceptives (OCs) compared to users of second-generation OCs. However, in each study, this association is of borderline statistical significance, and the studies do not indicate an increase in the absolute risk of venous thromboembolism for users of third-generation OCs compared with the data from prior studies primarily involving second-generation products. The data derived from these recent studies show a lower reported inci

Epidemiology studies on effects of lithium salts in pregnancy are confounded by the inability to control for other potentially teratogenic factors.

INTRODUCTION: In bipolar women who took lithium during pregnancy, several epidemiology studies have reported small increases in a rare fetal cardiac defect termed Ebstein's anomaly. METHODS: Behavioral, environmental, and lifestyle-associated risk factors associated with bipolar disorder and health insurance status were determined from an Internet search. The search was conducted from October 1, 2023, through October 14, 2023. The search terms employed included the following: bipolar, bipolar disorder, mood disorders, pregnancy, congenital heart defects, Ebstein's anomaly, diabetes, hypertens

Hypothesis: gonadal hormones act as confounders in epidemiological studies of the associations between some behavioural risk factors and some pathological conditions.

There are grounds for suspecting that, to varying degrees, smoking, alcohol consumption, oral contraceptive use, vasectomy and induced abortion are markers for high steroid hormone levels. So in epidemiological studies, false inferences may be drawn that these markers (treated as risk factors) have causal or exacerbating effects on diseases which are truly partially caused by high levels of hormones (e.g. probably prostatic cancer and breast cancer). Analogously, such studies of conditions which are truly partially caused by low levels of hormones (e.g. bone fractures, poor sperm quality, and

[Methods for controlling time-varying confounding in pharmaco-epidemiological studies: a systematic reveiw].

Objective: To systematically review the application of methods for controlling time-varying confounding in pharmaco-epidemiological studies. Methods: PubMed, Embase, CNKI, and Wanfang were searched for pharmaco-epidemiological studies involving time-varying confounding on June 15th, 2020. The basic characteristics, drug exposure and outcome, time-varying confounders and the application of methods to control these confounders were analyzed. Results: A total of 298 articles were included. An increasing trend was observed in numbers of studies dealing with time-varying confounding in pharmaco-epi

Mediational E-values: Approximate Sensitivity Analysis for Unmeasured Mediator-Outcome Confounding.

BACKGROUND: Mediation analysis is a powerful tool for understanding mechanisms, but conclusions about direct and indirect effects will be invalid if there is unmeasured confounding of the mediator-outcome relationship. Sensitivity analysis methods allow researchers to assess the extent of this bias but are not always used. One particularly straightforward technique that requires minimal assumptions is nonetheless difficult to interpret, and so would benefit from a more intuitive parameterization. METHODS: We conducted an exhaustive numerical search over simulated mediation effects, calculatin

Assessment of structured socioeconomic effects on health.

Social epidemiologists study effects of variables such as education or income on health outcomes. Because other factors may influence both the exposure and the outcome, adjustments are commonly made in an effort to estimate the "independent" effect of exposure. The validity of common adjustment strategies when estimating the outcome distribution under hypothetical interventions of the exposure is potentially compromised by structured relations between covariates, observed and unobserved. These considerations of covariate structure may be particularly important for the study of "distal" socioec

Negative controls and how to use them: a guidance paper.

Negative controls have gained traction as a useful tool for causal research to address confounding and bias. However, general guidance on how to select and apply negative controls and how to report and interpret their findings is lacking. This paper lays out the key steps of conducting a negative-control analysis and provides all relevant background information, important feasibility and validity considerations, and practical suggestions for the implementation and interpretation of each step. This includes a convenient overview of all methods, complete with their rationale, key considerations,

Confounder selection in firearm policy research: a scoping review.

Legislative firearm policies are often proposed as a way of preventing firearm-related harm. Confounding is a substantial threat to accurately estimating the causal effects of firearm policies. This scoping review characterizes the selection of potential confounders in US firearm policy evaluations in the health sciences literature. We identified empirical research articles indexed in PubMed from January 1, 2000, to January 9, 2021, that examined any of 18 prespecified firearm policies and extracted key study elements, including the exposure (firearm policy), outcomes, potential confounders ad

Negative controls: a tool for detecting confounding and bias in observational studies.

Noncausal associations between exposures and outcomes are a threat to validity of causal inference in observational studies. Many techniques have been developed for study design and analysis to identify and eliminate such errors. Such problems are not expected to compromise experimental studies, where careful standardization of conditions (for laboratory work) and randomization (for population studies) should, if applied properly, eliminate most such noncausal associations. We argue, however, that a routine precaution taken in the design of biologic laboratory experiments--the use of "negative

Simulation as a Tool for Teaching and Learning Epidemiologic Methods.

In aspiring to be discerning epidemiologists, we must learn to think critically about the fundamental concepts in our field and be able to understand and apply many of the novel methods being developed today. We must also find effective ways to teach both basic and advanced topics in epidemiology to graduate students, in a manner that goes beyond simple provision of knowledge. Here, we argue that simulation is one critical tool that can be used to help meet these goals, by providing examples of how simulation can be used to address 2 common misconceptions in epidemiology. First, we show how si

Sensitivity analysis of treatment effect to unmeasured confounding in observational studies with survival and competing risks outcomes.

No unmeasured confounding is often assumed in estimating treatment effects in observational data, whether using classical regression models or approaches such as propensity scores and inverse probability weighting. However, in many such studies collection of confounders cannot possibly be exhaustive in practice, and it is crucial to examine the extent to which the resulting estimate is sensitive to the unmeasured confounders. We consider this problem for survival and competing risks data. Due to the complexity of models for such data, we adapt the simulated potential confounder approach of Car

Confounding in statistical mediation analysis: What it is and how to address it.

Psychology researchers are often interested in mechanisms underlying how randomized interventions affect outcomes such as substance use and mental health. Mediation analysis is a common statistical method for investigating psychological mechanisms that has benefited from exciting new methodological improvements over the last 2 decades. One of the most important new developments is methodology for estimating causal mediated effects using the potential outcomes framework for causal inference. Potential outcomes-based methods developed in epidemiology and statistics have important implications fo

[Risk of pregnancy complications associated with residency].

OBJECTIVE: To determine, through a review of published articles, whether a higher prevalence of pregnancy complications is associated with residency in medicine. DATA SOURCES: Articles published between January 1980 and December 1992 found through a MEDLINE search using the MeSH terms "pregnancy complications" and "internship and residency" and through a review of the bibliographies of articles found. STUDY SELECTION: Of the 17 articles found, 6 contained data on the prevalence of pregnancy complications in residents. DATA EXTRACTION: The methodologic quality of the studies was evaluated syste
