Update on the male hormonal contraceptive agents.

There remains a need for new acceptable and effective male contraceptives to increase the choice for couples throughout the world. There have been no recent advances in available male contraceptive methods although a number of promising approaches have been identified, of which the hormonal approach is currently undergoing clinical investigation. In recent years the pace of research in this area has quickened significantly with increasing interest and now investment by the pharmaceutical industry. This is vital if the work undertaken so far by the public sector is to be transformed into a comm

The future of contraception: innovations in contraceptive agents: tomorrow's hormonal contraceptive agents and their clinical implications.

Currently available contraceptive methods have not eliminated the burden of unintended pregnancy. Future progress will require technological advancements, leading to the development of novel and highly effective birth control strategies that will be acceptable to the general population. Included are highlights of some of the ongoing research, demonstrating the development of methods with the potential to be highly effective, widely accepted, easy to use, and more cost-effective, with fewer side effects, than currently available options.

The use of hormonal contraceptive agents and mood disorders in women.

BACKGROUND: Mood disorders are a major cause of disability in developed countries, and contraceptive agents among the most widely used medications. The relationship between contraceptive agents and mood is unclear. The aim of this study was therefore to investigate the association between current contraception use and mood disorders in a random population-based sample of women. METHODS: This study examined epidemiological data obtained from 498 women aged 20-50year participating in the Geelong Osteoporosis Study (GOS). Mood disorders were diagnosed using a clinical interview (SCID-I/NP) and i

Biology of female sex hormone action in relation to contraceptive agents and neoplasia.

This article will present an overview of estrogen and progestin action at a cellular level, with emphasis on points that are relevant to neoplasia. In breast, endometrium and ovary, these two classes of hormone are clearly implicated in carcinogenesis, but their involvement with cancers of the liver, cervix and other tissues is more problematic. In the latter situations, I will highlight the major points to be considered if the hormones are involved without wishing to judge whether there actually is a causal involvement.

[Results of cytological and histological studies on the uterine cervix after long-term intake of hormonal contraceptive agents].

Cytologic examinations with 880 women having constantly used oral contraceptives from one to eight years show no agglomeration of epithelial atypias if the primary status was clearly in the negative. Among 669 histologically sure atypias of cervix uteri the part of women using oral contraceptives for at least one year is significantly lower in carcinoma in situ as well as in early invasive carcinoma. The differences in dysplasia are not significant. The results are also discussed with regard to the efficiency of permanent control examinations before and during the use of oral contraceptions. T

Triptonide is a reversible non-hormonal male contraceptive agent in mice and non-human primates.

There are no non-hormonal male contraceptives currently on the market despite decades of efforts toward the development of "male pills". Here, we report that triptonide, a natural compound purified from the Chinese herb Tripterygium Wilfordii Hook F displays reversible male contraceptive effects in both mice and monkeys. Single daily oral doses of triptonide induces deformed sperm with minimal or no forward motility (close to 100% penetrance) and consequently male infertility in 3-4 and 5-6 weeks in mice and cynomolgus monkeys, respectively. Male fertility is regained in ~4-6 weeks after cessa

Concomitant use of combined hormonal contraceptives and antifibrinolytic agents for the management of heavy menstrual bleeding: A practice pattern survey.

INTRODUCTION: Concomitant antifibrinolytic agents and combined hormonal contraceptives (CHC) have been anecdotally used to manage refractory heavy menstrual bleeding (HMB). Yet, there remains uncertainty among clinicians regarding the safety of this therapeutic option as concomitant CHC is listed as a contraindication to tranexamic acid use in the United States. AIM: To describe current treatment practices and physician-reported safety and effectiveness of concomitant antifibrinolytics and CHCs. METHODS: We surveyed clinician members of the Hemostasis and Thrombosis Research Society and the

An intravaginal ring that releases three antiviral agents and a contraceptive blocks SHIV-RT infection, reduces HSV-2 shedding, and suppresses hormonal cycling in rhesus macaques.

Women globally need access to multipurpose prevention technologies (MPTs) that prevent human immunodeficiency virus (HIV), sexually transmitted infections that increase HIV acquisition/transmission risk, and unintended pregnancy. Seeking an MPT with activity against HIV, herpes simplex virus-2 (HSV-2), and human papillomavirus (HPV), we developed a prototype intravaginal ring (IVR), the MZCL IVR, which released the antiviral agents MIV-150, zinc acetate, and carrageenan (MZC for short) and the contraceptive levonorgestrel (LNG). Previously, we showed that an MZC gel has potent activity against

Evaluation of pharmacokinetic interactions between long-acting cabotegravir or emtricitabine/tenofovir disoproxil fumarate and hormonal contraceptive agents: a tertiary analysis of South African participants in HPTN 084.

INTRODUCTION: HPTN 084 found that long-acting cabotegravir (CAB-LA) was well-tolerated and significantly reduced the risk of HIV acquisition in women compared to tenofovir disoproxil fumarate/emtricitabine (F/TDF). During the blinded phase of the trial, participants were required to use an effective method of contraception, including an injectable or implantable hormonal contraceptive (HC) agent. A contraceptive sub-study assessed the pharmacokinetic interactions between pre-exposure prophylaxis agents (CAB-LA or F/TDF) and etonogestrel (ENG), medroxyprogesterone acetate (MPA) or norethindrone

Case of ischemic colitis in a young adolescent associated with triphasic hormonal contraceptive therapy: a case report and review of the literature.

There has been speculation that third generation hormonal contraceptives may be less prone to inducing clotting than the earlier generation products. We present a case of colonic ischemia in a young adolescent receiving pharmacotherapy with a third-generation hormonal contraceptive. Ischemic colitis is an uncommon adverse effect in young adolescents associated with hormonal contraception, especially the third generation agents. We believe this case to be the second-youngest patient reported with ischemic colitis due to this therapy. Clinical vigilance is recommended for women presenting with a

Fertility regulatory methods and development of complications after evacuation of complete hydatidiform mole.

In a prospective, simple randomised study, we evaluated the relative efficacy of hormonal (oral contraceptive pill) and non-hormonal (intrauterine contraceptive device) methods of contraception as fertility regulatory agents in patients with complete hydatidiform moles and assessed the development of complications and sequelae if any, following their use. Fifty-six patients each on oral contraceptive pills (OCP) and intrauterine contraceptive device (IUCD) were recruited into the study. Ten patients, six on OCP and four on IUCD discontinued for varying reasons. Five patients (two on OCP, and t

Patterns of hormonal contraception use in women with sickle cell disease and employer-sponsored insurance.

OBJECTIVE: While an improved understanding of the epidemiology and outcomes of hormonal contraceptive use in persons with sickle cell disease is needed to better inform evidence-based guidelines, patterns of contraception use remain understudied. Our objective was to examine patterns of contraception use among patients with sickle cell disease and employer-sponsored insurance. STUDY DESIGN: Our analyses utilized the Merative™ MarketScan® Commercial Database (2012-2019). The study included females 12-44 years meeting the study definition of sickle cell disease (presence of three ICD-9 or ICD-1

Male contraception.

Currently approved male-directed contraceptive methods include condoms and vas occlusion. Vas occlusion is very effective but is intended to be non-reversible. Condoms have a relatively high failure rate, at least partially due to compliance problems and are not accepted by many couples. The only other male-oriented methods in clinical trials utilize the administration of testosterone alone or its combination with another gonadotropin-suppressing agent such as a progestin or a gonadotropin-releasing hormone antagonist. Studies published in the 1990s demonstrated that a testosterone-containing

Despite decades of assiduous inquiry and burgeoning societal demand, contraceptive options for men remain circumscribed. Women have benefitted from highly efficacious hormonal modalities, but the burden of contraceptive responsibility continues to rest almost exclusively upon them. In light of the potential sequelae of female hormonal agents and the inviolable right to bodily autonomy, the quest for safe, reversible male methods is assuming urgency.Hormonal contraception for men relies on suppression of spermatogenesis through exogenous testosterone combined with progestins.
