Obstetrical factors related to nuchal cord.

BACKGROUND: To investigate the intrapartum factors related to umbilical cord nuchal loops (nuchal cord) with particular reference to shoulder dystocia. METHODS: We studied all singleton pregnancies with a vertex presentation and a birth weight of at least 2500 g from 1 January 1978 to 31 March 1997 and the 13,717 pregnancies with nuchal cord were compared to the 44,136 without nuchal cord. RESULTS: When compared to pregnant women without nuchal cord, a greater proportion of pregnant women with nuchal cord underwent induction of labor (adjusted OR 1.09, 95% CI 1.04-1.15) and augmentation with

Neonatal factors impacting umbilical cord blood unit characteristics.

A promising alternative to bone marrow in hematopoietic stem cell transplantation is umbilical cord blood (UCB). Major barrier to its use in transplantation is stem cell quantity and quality. It is crucial to determine the variables impacting the quality of these cells for bankability. The study aimed to investigate the impact of neonatal factors on UCB units. A total of 150 UCB units that were collected during the caesarean section were included in the study. The sex, birth order, gestational age, birth weight, chest circumference, head circumference, and Apgar score of the newborns were reco

Predisposing and risk factors of umbilical cord prolapse.

Altogether 117 deliveries (0.14%) out of 82 308 were complicated by umbilical cord prolapse (UCP) in the obstetrical department of Tampere University Central Hospital in the period 1962-81. The gross perinatal mortality rate in the present study was 16.2%, but during the last 5-year period the incidence of UCP (0.09%) as well as the perinatal mortality rate (5.9%) declined significantly. UCP seemed to be more prevalent during the first half of the year. The most prominent precipitating factors for the occurrence of UCP seemed to be abnormal presentation, multiple pregnancy, low birth weight, a

Acute spinal cord injury: analysis of epidemiologic factors.

The authors studied the epidemiologic aspects of acute spinal cord injury in 358 patients admitted to the Toronto General and Sunnybrook hospitals between 1948 and 1973. The ages of the patients ranged from 14 to 89 years (median 32 years) and the male:female ratio was 4.5:1. The most frequent causes of injury were traffic accidents (34.4%), accidents at work (29.3%), sports-recreational injuries (15.4%) and falls at home (9.8%). Several epidemiologic factors were identified which are important for developing programs to reduce the frequency of cord injury. In particular young persons should b

Spinal Cord Infarcts: Risk Factors, Management, and Prognosis.

There are no standard guidelines for treatment of spinal cord ischemia due to how rare it is and the diverse etiology and presentations involved. In addition, to date, there have been no large clinical trials examining ideal pharmacologic treatment options for spinal cord infarct. In our practice, we rely on hemodynamic augmentation initiated as soon as possible. Otherwise, treatment is usually geared towards the etiology of spinal cord ischemia. For instance, spinal cord ischemia occurring after aortic aneurysmal repair may improve with CSF drainage through a lumbar catheter in the periproced

Prognostic factors and outcome of spermatic cord sarcoma.

PURPOSE: To evaluate management and outcome in a large series of patients with spermatic cord sarcomas (SCS), a rare malignancy. METHODS: Eighty-two patients with localized SCS treated at two dedicated sarcoma units between 1992 and 2013 were included. Disease-specific survival (DSS) and crude cumulative incidence of local recurrence and distant metastases (DM) were estimated by Kaplan-Meier plots and log rank tests. RESULTS: Median follow-up was 33 months (interquartile range 13-72 months). Sixty-one patients presented with primary disease. Liposarcoma was the most common histotype, but sur

Factors influencing cord blood viability assessment before cryopreservation.

BACKGROUND: Cord blood (CB) viability determines product quality and varies with time and temperature of exposure before cryopreservation. Global viability assessment may not reflect viability of white blood cell (WBC) subsets, CD34+ cell viability, or hematopoietic stem/progenitor cells function. STUDY DESIGN AND METHODS: We compared trypan blue (TB) and acridine orange/propidium iodide (AO/PI) staining with flow-cytometric (7-aminoactinomycin D [7-AAD]) viability in total WBCs (Tot-AAD), granulocytes, monocytes, lymphocytes, and CD34+ cells and total nucleated cell, CD34+, and colony-formin

Serum rheumatoid factors in spinal cord injury patients.

The rheumatoid factor (RF) levels of IgM-RF and IgG-RF types in sera from patients with spinal cord injuries (SCI) and osteochondrosis were investigated by ELISA. According to the findings in the late stages of the disease, SCI patients showed 46% seropositiveness for IgG-RF and 40% for IgM-RF. Patients with fresh central nervous system (CNS) injuries were 12% and 14% seropositive, respectively. Patients with osteochondrosis exhibited 33% seropositiveness for IgG-RF. The determination of serum RF enables us to identify the systemic body lesion in the SCI late stages as a rheumatoid disease. Th

Umbilical cord separation: histological findings and perinatal factors.

The aim of this study was to investigate if perinatal factors influenced the histology of the umbilical area of neonates who died when umbilical cord separation was occurring or had just been completed. It was found that the older the infant was at death the greater the degree of infiltration of polymorphonuclear leucocytes into the area of separation of the umbilical cord. The infiltration was less when the infant was delivered by caesarean section than when the infant was delivered vaginally. The other factors studied (sex, birth-weight, gestational age, positive blood culture, time of ruptu

[Predictive factors of vocal cord paralysis following thyroid surgery].

INTRODUCTION: One of the most serious potential complications of thyroid surgery is temporary or permanent dysfunction of the recurrent laryngeal nerve. According to the literature, the development of nerve injury is influenced by several predictive factors, including demographic, surgical and pathological factors. OBJECTIVE: The aim of our research was to determine the prevalence of temporary and permanent vocal cord paralysis following thyroid surgery in our patient population, and to identify preoperative factors that may be associated with the development of paralysis. METHOD: We retrosp

Factors associated with contractures in acute spinal cord injury.

The purpose of this study was to examine the occurrence of contractures in acute SCI and clarify possible contributing factors such as early versus late admission, level of injury, completeness of the lesion, pressure ulcers, spasticity, co-existent head injury, extremity fractures, heterotopic ossification (HO), peripheral nerve injury, and shoulder pain. The records of all patients (n = 482) admitted between 1990 and 1995 with acute SCI to a model SCI system were reviewed. Of 482 patients 44 (9%) (33 male, 11 female) developed contractures during their initial hospitalization. There were 30

The effect of perinatal factors on cord thyroxine concentration.

Cord blood thyroxine (T4) concentrations were measured in 4,068 infants from 28 wk gestation to term. Each chart was reviewed for the following factors: delivery by cesarean section, prolonged rupture of membranes, neonatal asphyxia, meconium-stained amniotic fluid, maternal diabetes mellitus and twinning. Each neonate was evaluated for the Idiopathic Respiratory Distress Syndrome, and low (SGA) or high (LGA) birthweight for gestational age. Within each gestational age group, the mean cord T4 value was similar except for a significantly lower mean cord T4 concentration for the term SGA subgrou

Abnormal umbilical cord coiling and association with pregnancy factors.

The umbilical cord, as a connecting bridge between two lives, plays an important role in fetal development. Though studies on the umbilical cord date back many years, extensive studies on certain umbilical cord characteristics, such as umbilical cord coiling, are rare. Cord coiling, measured by the umbilical coiling index, is a physiological phenomenon that offers resistance to external pressures. Umbilical cord coiling is a result of several factors, both environmental and genetic. However, umbilical cords sometimes coil abnormally, resulting in hypocoiling, hypercoiling, or non-coiling which

[Factors modifying cord blood IgE levels - a pilot study].

AIM: The complex influence of internal and external environmental factors on the individual and his/her immune system and the lack of suitable markers to assess and reduce the risk of the development of allergies during the lifetime can explain the continuous increase in the number of people affected by some form of allergy. According to the results of some studies, cord blood IgE level could be a useful early marker for assessing the risk of atopic diseases, but the studies showed controversial results. In addition, several authors discuss the origin of these antibodies (synthesis in utero, p

Risk factors for transient vocal cord palsy after thyroidectomy.

BACKGROUND: Transient recurrent laryngeal nerve palsy affects to 5-10 per cent of patients after extracapsular thyroidectomy. This prospective study assessed the impact of surgical injury and extralaryngeal branching of the inferior laryngeal nerve (ILN) on vocal cord dysfunction (VCD). METHODS: Total thyroidectomy or lobectomy was performed in 188 patients, with 302 ILNs at risk. The anatomy of the ILN and degree of injury to the nerve, based on the Laryngeal Nerve Injury Score (LNIS), were recorded. Fibreoptic laryngoscopy was performed a mean(s.d.) of 10.6(4.1) days after thyroidectomy. R
