Cordocentesis.

Access to the fetal circulation via cordocentesis has resulted in many advances in both prenatal diagnosis and fetal therapy over the past decade. In this review, current research in the assessment and treatment of erythrocyte isoimmunization and fetal infection by fetal blood sampling, as well as the role of cordocentesis in the evaluation of fetal platelet abnormalities, intrauterine growth retardation, fetal thyroid function, and genetic investigation, are discussed. Finally, a brief review of complications and loss rates relating to this procedure is given.

Fetal bradycardia following cordocentesis.

Several clinical investigations on the course and outcome of pregnancies following cordocentesis have mentioned the occurrence of fetal bradycardia at the time of umbilical cord puncture. The prognostic impact of this common complication has remained controversial. Our purpose was to investigate the prevalence and the short-term and long-term consequences of fetal bradycardia associated with cordocentesis. This study included all 339 cordocenteses performed in 290 fetuses at the Division of Prenatal Diagnosis and Therapy, University of Vienna, between 1991 and 1994. Clinically significant brad

Cordocentesis for rapid karyotyping.

Pure fetal blood was obtained by cordocentesis in 101 fetuses of 96 patients at 15 to 38 weeks' gestation. Rapid karyotype was obtained within 2 to 4 days by fetal lymphocyte culture. Chromosomal abnormality was detected in 12 (11.9%) fetuses. Abnormal karyotype was found in 5 of 44 fetuses with structural malformations, 3 of 13 fetuses with intrauterine growth retardation or oligohydramnios, 1 of 3 fetuses with nonimmune hydrops fetalis, 2 (one monozygotic set) of 10 discordant twins, 1 of 12 isoimmunized gestations, none of 8 cases with advanced gestational-maternal age, and none of 6 immune

Cordocentesis: an appraisal of risks.

Fetal blood sampling via cordocentesis is being used with increasing frequency. Between March 1986 and February 1988, we performed 58 intravascular transfusions (64 attempts) and 27 fetal blood samplings (28 attempts) guided by ultrasonography. The complication rates for intravascular transfusion and fetal blood sampling were 9.4% (6 of 64) and 7.1% (2 of 28), respectively. The procedure-related mortality rates were 4.7% (3 of 64) and 0%. We caution that the potential complications be seriously considered when appropriate patients are selected for cordocentesis. Additionally, we recommend that

[Fetomaternal alloimmunization: role of cordocentesis].

Twenty one pregnancies complicated by alloimmunization were managed by the use of intravascular method on an outpatient basis. One group was made of 9 women having had at least one pregnancy with a severely affected fetus. The other group was composed of 12 women without a previously affected infant; in 5 cases a situation at risk, either a transfusion (4 cases) or a severe obstetrical hemorrhage (1 case), was evidenced. Knowledge of fetal blood type (2 cases) and hematocrit determination obtained by fetal blood sampling allowed treatment individualized to the specific needs of each patient. I

[Detection of chromosomal abnormalities using cordocentesis].

Four cases of cytogenetic prenatal diagnosis of fetuses with chromosomal aberrations are presented: (1) the Patau syndrome; (2) and (4) the Down syndrome; (3) the Klinefelter syndrome. Cordocentesis has been shown to be expedient for rapid and accurate determination of fetus karyotype. Indicative for cytogenetic examination were ultrasonic data, maternal age, the values of AFP, HGG and nonconjugated estreol in maternal serum. Comparison of ultrasonic examination of fetuses with the data on abortus autotopsia was undertaken. The results demonstrate importance of ultrasonic, cytogenetic, biochem

Role of cordocentesis in perinatal medicine.

Cordocentesis is a feasible, reliable, and safe method for fetal evaluation of a wide variety of disorders. It has allowed for in-utero treatment of fetal red cell isoimmunization and opened future routes for other fetal therapies. In this review, results of a multicenter intrauterine fetal blood sampling registry will be presented, outlining current indications, techniques, and complications of the procedure. In addition, current research in the assessment and treatment of fetal erythrocyte isoimmunization and infection, as well as the role of cordocentesis in rapid genetic karyotyping, work-

Cordocentesis in the diagnosis of intrauterine starvation.

Glycine and valine were measured in umbilical venous blood obtained by cordocentesis from 28 small-for-gestational-age fetuses. The values were compared with those from 62 appropriate-for-gestational-age fetuses. The ratio of glycine to valine was higher in the small-for-gestational-age fetuses and correlated significantly with the degree of fetal hypoxemia. These findings indicate that placental insufficiency is associated with a biochemical disturbance in the fetus that is similar to that observed in children with protein deprivation and kwashiorkor syndrome.

Prenatal diagnosis using sonographic guided cordocentesis.

Cordocentesis has been practiced as a diagnostical tool for prenatal diagnosis of intrauterine infections, hematological disorders, metabolic status of the fetus and rapid cytogenetic analysis. The performance of 198 cordocentesis is presented over 3 years of experience. A 21 gauge spinal needle is inserted via the optimal point on the maternal abdomen under real-time ultrasonic guidance into the insertion of the umbilical cord in the placenta. Successful cordocentesis were achieved in 98.5% of the cases. Termination of pregnancy was directly related to the procedure in only 1%. Hematoma surro

Safer cordocentesis by new 25-gauge needles.

OBJECTIVE: The objective of this study was to establish safer cordocentesis by using newly produced 25-gauge needles rather than the needles currently used. METHODS: Human umbilical cords were preserved in experiment 1. Pinching both ends, cordocenteses were performed by filling the cord with saline inside the sphere of the pressure-equilibrated Ringer's solution bag in an aquarium. Pressure alterations were recorded before and after the cordocenteses (n = 5 in each needle) using 22-, 23-, and 25-gauge needles. In experiment 2, human umbilical blood after delivery was preserved. Maternal thrombocytopenia. Predicting neonatal thrombocytopenia with cordocentesis.

OBJECTIVE: To evaluate the efficacy of cordocentesis for predicting fetal thrombocytopenia in the presence of maternal thrombocytopenia. STUDY DESIGN: We studied platelet counts obtained by cordocentesis from 42 consecutive immune thrombocytopenia purpura patients. Platelet counts were obtained on 36 neonates, and the statistical analysis included only these infants. Presence of maternal antiplatelet antibodies, interval from fetal sampling to delivery, neonatal platelet counts and outcome were evaluated. Thrombocytopenia was defined as a platelet count < or = 150,000/microL, with < or = 50,0

Cordocentesis for rapid karyotype: 421 consecutive cases.

Between October 1985 and December 1993, 421 patients underwent fetal blood sampling for rapid karyotyping (426 samplings, 5 twin pregnancies). The aim of the study was to evaluate cordocentesis in terms of results, complications and additional information in this specific indication especially in case of abnormal pregnancy sonogram. The fetal loss rate possibly related to fetal blood sampling was 1.9%. Abnormal sonograms represented 91% of rapid karyotype indications. Chromosomal abnormalities were found in 9.5% of abnormal sonograms and in 16.9% of fetal structural anomalies (37 cases). The c

[Cordocentesis for prenatal diagnosis of fetal diseases].

For the purpose of prenatal diagnosis, 30 fetal blood samplings were performed in 30 pregnant women from 19 to 36 gestational weeks. The samples were taken with a 22-gauge needle guided by ultrasound. Twenty-five cordocenteses were succeeded at the first attempt. A second attempt was needed in other 5 cases. The duration of procedure was less than 15 minutes in 83.3% cases. 1-6 ml fetal blood was obtained for each case. No lethal complication occured in our series. It is believed that cordocentesis could be a useful tool for prenatal diagnosis, and the intrahepatic-vein sampling is a preferabl

Diagnostic use of cordocentesis in twin pregnancy.

We performed percutaneous umbilical blood sampling, under ultrasound guidance, in both twins of 11 twin pregnancies in whom twin-to-twin transfusion syndrome was suspected. Chorionicity and the presence of placental vascular anastomoses were assessed postnatally. The hemoglobin concentration, hematocrit and other biochemical variables in fetal blood were evaluated in both twins. The mean hemoglobin difference between the large and small twins was 4.8 g/dl (range 1.8-8.1 g/dl) in 5 monochorionic discordant twin pairs, and 1.2 g/dl in a monochorionic twin pair without discordancy. [Hematological values in fetal blood by cordocentesis].

Reference ranges for four hematological parameters--hemoglobin, hematocrit, white blood cell and platelet count--in each gestation were established from values for fetal blood samples obtained by cordocentesis from 72 pregnancies at 16-39 weeks gestation. These 72 fetuses turned out as normal or single malformations which should not affect fetal hematological values. Significant correlations were observed between hemoglobin, hematocrit and platelet counts and the number of weeks of gestation. No correlation was found between the white blood cell count and gestational age. Fetal anemia (hematoc
