Native Coronary Artery Pseudoaneurysm after Coronary Artery Bypass Grafting.

Coronary artery pseudoaneurysms are extremely rare and most often occur after trauma or endovascular procedures [Aoki 2008; Kar 2017]. Delay in diagnosis or treatment may lead to coronary thrombosis with resultant ischemia or hemorrhage subsequent tamponade. Here, we present the case of a 66-year-old female who developed a coronary artery pseudoaneurysm of a non-grafted vessel three weeks after coronary artery bypass grafting. To avoid re-sternotomy, the pseudoaneurysm was successfully managed with a covered coronary stent and mini-left anterior thoracotomy to evacuate the hemopericardium and

Coronary artery bypass graft for anomalous right coronary artery.

A 27-year-old man with anomalous right coronary artery (RCA) presented with ventricular fibrillatory arrest. Computed tomography angiography revealed an anomalous RCA arising from the left sinus of Valsalva with an acute angulation at the coronary takeoff and compressed interarterial segment. This patient underwent a short saphenous vein coronary artery bypass graft to the proximal RCA and recovered uneventfully. A review of the literature and discussion of the surgical management is presented.

[Off-pump coronary artery bypass].

UNLABELLED: Development of beating heart coronary artery bypass surgery was possible since introduction of heart stabilizing instruments in mid-90's. THE AIM OF THIS REVIEW: To summarize available evidence of benefits of beating heart coronary artery bypass surgery in comparison with use of cardiopulmonary bypass during surgery. Methods of heart stabilization during operation without cardiopulmonary bypass allow to performing complete revascularization with lower degree of myocardial damage. The main advantage of beating heart coronary bypass surgery is excluding systemic inflammatory respons

Robotic coronary artery bypass for aberrant right coronary artery stenosis.

Anomalous coronary arteries that course between the aorta and pulmonary artery are subject to compressive forces and can manifest angina, myocardial infarction and sudden death. The current report presents a young, female patient who presented with a short duration of severe, rapidly progressive angina despite optimal medical therapy. Combined computed tomography and myocardial perfusion scanning identified an anomalous dominant right coronary artery that appeared kinked at its origin between the aorta and main pulmonary artery. A robot-assisted right internal thoracic artery to right coronary

Coronary artery spasm after off-pump coronary artery bypass grafting.

BACKGROUND: Coronary artery spasm (CAS) in the immediate postoperative period has been recognized as a possible cause for perioperative myocardial ischaemia after off-pump coronary artery bypass grafting (CABG). It varies in severity and can be associated with circulatory collapse and death. The purpose of this study was to present our experiences on CAS after off-pump CABG and detail its management. METHODS: The case reports of three patients with similar clinical presentations of ischaemic heart disease who underwent CABG using an off-pump technique are reviewed. Severe manifestations of CA

Staged carotid artery stenting and coronary artery bypass surgery versus isolated coronary artery bypass surgery in concomitant coronary and carotid disease.

BACKGROUND: We aimed to compare the outcomes of patients who underwent carotid artery stenting (CAS) followed by coronary artery bypass grafting (CABG) with the outcomes of those who underwent isolated CABG without carotid intervention. METHODS: In this prospective cohort study, conducted between March 2007 and February 2010, all patients who had significant carotid artery stenosis (>70%) and were candidates for CABG were included. The outcome measures, including 30-day post-stenting complications, cardiac surgery neurological complications, myocardial infarction (MI), and mortality rates, we

[Percutaneous transluminal coronary angioplasty after coronary artery bypass].

Anginal syndrome may recur early of several years after coronary artery bypass surgery (CABS), and may be due to narrowing of a bypass graft, progression of pre-existing coronary artery lesions, or the appearance of new lesions. Repeat CABS is associated with considerable morbidity and mortality. We therefore performed percutaneous transluminal angioplasty (PTCA) in saphenous or internal mammary bypass grafts or native coronary arteries in 23 patients after CABS. We successfully dilated 35 of 37 lesions (95%) present in 33 of 35 vessels (94%) of 21 of 23 patients (91%). The mean vessel narrowi

Coronary artery bypass following percutaneous transluminal coronary angioplasty.

During a 4-year period, 286 patients underwent coronary artery bypass grafting (CABG) following percutaneous transluminal coronary angioplasty (PTCA). Seventy-three patients had single-vessel and 213 (74.5%) had multivessel coronary artery disease. Twenty-nine patients underwent PTCA because of an evolving acute myocardial infarction (MI). Forty-two patients had previously undergone 47 CABG procedures. One hundred fifteen patients underwent CABG on an emergency basis. Indications for emergency CABG after PTCA were prolonged chest pain (79.1%), worsening of coronary artery obstruction (59.1%),

[Integrated coronary artery bypass strategy prevents urgent pump conversion during off-pump coronary artery bypass grafting].

Urgent pump conversion during off-pump coronary artery bypass (OPCAB) results in high morbidity and mortality. We retrospectively evaluated if the peri-operative integrated strategy prevents this lethal event in our 400 consecutive OPCAB operations. The patients with preoperative cardiogenic shock and/or ventricular arrhythmias underwent on-pump coronary artery bypass grafting (CABG). All other patients (99% of total CABG) were scheduled to undergo OPCAB (n=400). Prophylactic intraaortic balloon pumping (IABP) was applied to the patients with critical (>95%) left main trunk stenosis or low (<0

Coronary artery disease and coronary artery bypass grafting in Behçet's disease.

There is a high frequency of pseudoaneurysm formation in patients with Behçet's disease and their inflammed and fragile tissues are difficult to manipulate. Five patients with Behçet's disease were referred to our cardiovascular surgery department for coronary artery bypass grafting (CABG). Three of them were operated and two were treated medically. Patients that were managed medically had left anterior descending (LAD) lesions below 80% and their stable angina pectoris responded well to medication. There was no early mortality and morbidity. One patient developed pseudoaneurysm of ascending a

[Emergency coronary artery bypass grafting for left main coronary artery disease].

Emergency coronary artery bypass grafting (CABG) for acute coronary syndrome due to left main coronary artery (LMCA) disease still remains to be a therapeutic challenge. Seventy consecutive patients underwent CABG for LMCA disease were divided into two groups, that is 21 patients with emergency CABG (Group E) and 49 with scheduled CABG (Group S). There were four in-hospital deaths (mortality rate: 19.1%) in Group E and two (4.1%) in Group S. Three of 4 deaths in Group E had cardiogenic shock associated with acute myocardial infarction (AMI) preoperatively, and low output syndrome followed by m

[Coronary artery bypass grafting in two patients with single coronary artery].

We experienced two patients with single coronary artery who underwent CABG using arterial grafts successfully. In two patients coronary angiography demonstrated a single coronary artery which was originated in left coronary sinus and was bifurcated to LAD and LCx, and then RCA branched off proximal LAD, passing in front of the right ventricular out flow tract (Sharbaugh Type L-IIa). To the first patient, a 52-year-old man who had angina on exertion due to long stenosis of RCA, CABG to RCA using RITA was carried out. To the second patient, a 57-year-old man who had inferior myocardial infarctio

[Coronary artery bypass grafting for a patient with single coronary artery].

A 47-year-old man was admitted with suddenly developed chest pain. Coronary arteriography could not identify the right coronary orifice at the right coronary sinus. Left coronary arteriography revealed that the right coronary artery originated from a proximal segment of the left anterior descending artery with subtotal obstruction at the segment just distal to the bifurcation of the right and anterior descending arteries. Coronary artery bypass grafting to the middle segment of the left anterior descending artery was carried out with satisfactory result. To our knowledge, only seven cases with

[Operation of coronary artery aneurysm after previous coronary artery bypass grafting].

A 43-year-old male, who had undergone coronary artery bypass grafting 11 years ago, developed exertional chest pain. Selective coronary angiograms revealed severe stenosis and a large aneurysm in the obtuse marginal branch of the circumflex coronary artery. Previous grafts to the left anterior descending coronary artery and diagonal branch were patent. Ligation of the aneurysm and internal mammary artery grafting were performed through a left anterolateral thoracotomy. This approach made it easy to reach the aneurysm and to minimize bleeding during dissecting the adhesions. The patient had an

Aorto-left common carotid artery bypass with off-pump coronary artery bypass.

BACKGROUND: Although carotid artery disease can occur in patients who have coronary artery disease (CAD), ostial stenosis of a left common carotid artery (CCA) along with CAD is a rarely reported condition. CASE REPORT: We describe a rare case of severe stenosis of the left CCA ostium and CAD, which were successfully treated with an off-pump coronary artery bypass and aorto-left CCA bypass. This strategy minimizes the risk of perioperative strokes. Furthermore, an aorto-supra aortic trunk bypass provides a more physiological blood flow pattern than cerebral approaches. This approach would be
