Surgical correction for hearing impairment from collapse of the external auditory canal.

A patient is presented who was symptomatic from collapse of his left external auditory meatus. The attenuation produced by meatal collapse was eliminated by a special meatoplasty. Although surgical intervention may not be indicated in every case, it is certainly a viable alternative and was effective for this patient.

5xFAD mutations induce hearing impairment in the Ahl-corrected 5xFAD mice.

Age-related hearing impairment (ARHI) has been strongly associated with cognitive impairment in case-controlled and longitudinal population-based studies. However, a mechanistic link between ARHL and Alzheimer's disease (AD)-related dementia remains unclear. In this study, we developed Ahl-corrected C57BL/6-background 5xFAD mice (referred to as FAD2), an AD mouse model suitable for precise assessment of auditory function, along with their wildtype littermates (WT2). WT2 mice of both sexes exhibited stable auditory brainstem response (ABR) thresholds from 3-4 months to 9-10 months of age at fre

The extent of hearing impairment amongst Australian Indigenous prisoners in Victoria, and implications for the correctional system.

The hearing status of 109 Indigenous prisoners was investigated at five prison locations in Victoria, using audiological methods and face-to-face interview. The study found predominantly mild, sensorineural hearing loss. The rate of conductive hearing impairment was consistent with an age-matched general adult population (UK). All eardrums were intact, and 89% of middle-ears were normally air filled. Results showed 12% of prisoners had a hearing loss (average. 0.5, 1, 2, & 4 kHz >or=25 dB) in at least one ear, compared with 5% in an age-matched Australian adult population. More than a third (3

Genetic correction of TRMU allele restored the mitochondrial dysfunction-induced deficiencies in iPSCs-derived hair cells of hearing-impaired patients.

Sensorineural hearing loss often results from damaged or deficient inner ear hair cells. Mitochondrial 12S rRNA 1555A>G mutation has been associated with hearing loss in many families. The m.1555A>G mutation is a primary factor underlying the development of hearing loss and TRMU allele (c.28G>T, p.Ala10Sser) encoding tRNA thiouridylase interact with m.1555A>G mutation to cause hearing loss. However, the tissue specificity of mitochondrial dysfunction remains elusive and there is no highly effective therapy for mitochondrial deafness. We report here the generation of induced pluripotent stem ce

Incorporating Patient Narratives to Enhance Audiological Care and Clinical Research Outcomes.

Purpose The engagement of patients as key stakeholders in their experience of care processes is a critical component of quality improvement efforts for both clinical care and translational research. Increasingly, health care systems are soliciting input from patients on care processes and experiences through surveys, patient interviews, and patient video narratives. The purpose of this viewpoint article is twofold: (a) to describe the increasing role of patient narratives about their experiences with adverse health conditions to inform patient-centered research and quality improvement efforts

A scale for hearing aid benefit measures and auditory disability.

Quoting disability scores or measures of hearing aid benefit based on percent correct word identification or on dB signal-to-noise ratio at criterion performance poses a communication problem as scientists and clinicians may have no intuitive understanding of the scales involved. In particular, percent correct does not allow direct comparison of differences obtained at different performance levels, for example by different subject groups. In circumstances where the better ear hearing levels are known to be a major determinant of disability, it is both possible and legitimate to transform perfo

Hearing impairment and the elderly patient.

Hearing impairment is one of the most common causes of disability in the elderly. In addition to its obvious adverse effects on communication, hearing impairment has been associated with impaired mobility, cognitive impairment, and depression. Treatment of hearing impairment can reduce symptoms of depression and improve quality of life in hearing impaired patients. Yet in the Framingham cohort, although 41% of those 65 years of age and older admitted to hearing difficulty, only 10% had ever used a hearing aid. This article discusses the epidemiology of hearing problems in the elderly, types of

[Cochlear implantation].

Since the Israeli cochlear implant program was started in June 1989, 74 candidates have been evaluated for implantation. The main causes for hearing loss were meningitis (16 patients), aminoglycoside toxicity (9), congenital (6), viral (e.g. mumps) (6), and middle ear infection. Candidates were rejected mainly due to psychosocial factors (40%) and audiological parameters (30%). 16 postlingual deaf were implanted. In 7 of them (43.7%) the results were excellent, in 4 (25.0%) good, in 3 (18.7%) fair; 1 case is currently in a training program and 1 is a failure. There were 2 major complications:

Audiological approaches to address the psychosocial needs of adults with hearing loss: perceived benefit and likelihood of use.

OBJECTIVE: To explore the perceived benefit and likely implementation of approaches used by audiologists to address their adult clients' psychosocial needs related to hearing loss. DESIGN: Adults with hearing loss and audiologists completed separate, but related, surveys to rate their perceived benefit and also their likely use of 66 clinical approaches (divided over seven themes) that aim to address psychosocial needs related to hearing loss. STUDY SAMPLE: A sample of 52 Australian adults with hearing loss, and an international sample of 19 audiologists. RESULTS: Overall, participants rate

Speech Perception Training in Children: The Retention of Benefits and Booster Training.

OBJECTIVES: Speech perception training can be a highly effective intervention to improve perception and language abilities in children who are deaf or hard of hearing. Most studies of speech perception training, however, only measure gains immediately following training. Only a minority of cases include a follow-up assessment after a period without training. A critical unanswered question was whether training-related benefits are retained for a period of time after training has stopped. A primary goal of this investigation was to determine whether children retained training-related benefits 4

Individual Hearing Aid Benefit in Real Life Evaluated Using Ecological Momentary Assessment.

Ecological momentary assessment (EMA) was used in 24 adults with mild-to-moderate hearing loss who were seeking first hearing-aid (HA) fitting or HA renewal. At two stages in the aural rehabilitation process, just before HA fitting and after an average 3-month HA adjustment period, the participants used a smartphone-based EMA system for 3 to 4 days. A questionnaire app allowed for the description of the environmental context as well as assessments of various hearing-related dimensions and of well-being. In total, 2,042 surveys were collected. The main objectives of the analysis were threefold:

Sound therapy and aural rehabilitation for tinnitus: a person centred therapy framework based on an ecological model of tinnitus.

Purpose: Tinnitus is a common oto-neurological complaint often accompanying hearing loss. In this perspective on rehabilitation we describe a framework for sound therapy and aural rehabilitation of tinnitus based on the ecological model of tinnitus. Method: A thematic network analysis-based approach was used to relate aural rehabilitation methods to the ecological model of tinnitus and the client-oriented scale of improvement in tinnitus. Results: Aural rehabilitation methods were mapped to concepts of: (1) Context, (2) presence of sound and (3) reaction to sound. A global theme was: adaptatio

Objective measure of low-frequency amplification reduction in canal hearing aids with adaptive circuitry.

The total amount of "ampclusion" (low-frequency amplification + occlusion) and its reduction were measured in two identically fitting canal hearing aids with three circuitry capabilities: (1) K-Amp, (2) linear, and (3) noise reduction (ANP II). The insertion gains of all three circuits were adjusted to be closely matched and the amounts of occlusion for both aids were almost identical. Real-ear measurements were obtained as the author vocalized the phoneme /i/ at 65 dB SPL at 2 ft in three conditions: (1) unoccluded, (2) occluded, aid off, and (3) occluded, aid on. The results indicated a redu

Hearing problems and the elderly.

Hearing problems have a profound influence on the lives of the elderly. The ability to communicate is frequently a deciding factor in determining a person's autonomy, independence, and overall well-being and happiness. In the works of Ralph Naunton, in the introduction to the report, "Society is ill-informed about the importance of speech communication and about the devastating effects of communication difficulties, and it is ill-informed about the cost effectiveness of providing communication aids to the elderly and about the cost to society and to the individual of not providing them." This

Amplification and aural rehabilitation in resource-constrained environments.

This article attempts to highlight the challenges and possibilities for hearing healthcare through technology and aural rehabilitation in a resource-constrained setting, using South Africa as an example. Results and conclusionThe authors argue that it is possible to enhance service delivery by using free resources and maximising the limited existing resources. In order to provide a sustainable hearing healthcare service in developing countries, it is pertinent to understand the context where the services are needed, and not just adopt an approach developed for a different context.
