Establishing Cost-Effective Allocation of Proton Therapy for Patients With Mediastinal Hodgkin Lymphoma.

PURPOSE: For curative treatment of Hodgkin lymphoma, radiation therapy benefit must be weighed against toxicity. Although more costly, proton radiation therapy reduces dose to healthy tissue, potentially improving the therapeutic ratio compared with photons. We sought to determine the cost-effectiveness of proton versus photon therapy for mediastinal Hodgkin lymphoma (MHL) based on reduced heart disease. METHODS AND MATERIALS: Our model approach was 2-fold: (1) Use patient-level dosimetric information for a cost-effectiveness analysis using a Markov cohort model. (2) Use population-based data

Optimal allocation of distributed energy storage systems to enhance voltage stability and minimize total cost.

The enhancement of energy efficiency in a distribution network can be attained through the adding of energy storage systems (ESSs). The strategic placement and appropriate sizing of these systems have the potential to significantly enhance the overall performance of the network. An appropriately dimensioned and strategically located energy storage system has the potential to effectively address peak energy demand, optimize the addition of renewable and distributed energy sources, assist in managing the power quality and reduce the expenses associated with expanding distribution networks. Cost Implications of New National Allocation Policy for Deceased Donor Kidneys in the United States.

BACKGROUND: In December 2014, a new national deceased donor kidney allocation policy was implemented, which allocates kidneys in the top 20% of the kidney donor profile index to candidates in the top 20% of expected survival. We examined the cost implications of this policy change. METHODS: A Markov model was applied to estimate differences in total lifetime cost of care and quality-adjusted life years (QALY). RESULTS: Under the old allocation policy, average lifetime outcomes per listed patient discounted to 2012 US dollars were US $342,799 and 5.42 QALY, yielding US $63,775 per QALY gained

Strategic resource allocation for malaria elimination in endemic settings: a systematic review of cost-effectiveness evidence.

INTRODUCTION: Malaria remains a significant global health burden, especially in endemic regions where efficient use of limited resources is critical. Economic evaluations provide essential evidence to guide strategic resource allocation and optimise intervention outcomes. This systematic review synthesises recent evidence (2018-2025) on the cost-effectiveness of malaria prevention and treatment interventions in endemic settings. MATERIALS AND METHODS: A systematic search of Scopus, Web of Science, and PubMed identified English-language studies published between 2018 and 2025. Eligible studies

Bridging energy and proteome allocation models allows prediction of the resource cost of traits across environments.

Many traits divert resources from reproduction and are costly due to the physiological trade-offs organisms face. These traits might require the allocation of different resources, e.g., energy expenditure or production of macromolecules, and it is unclear how their costs can be compared given their different units. Moreover, the effect of environmental conditions and genetic background on the cost of traits is not well understood. Two different frameworks have been proposed to quantify the resource costs of traits, focusing on either energy allocation or proteome allocation. These frameworks i

Liver Transplant Costs and Activity After United Network for Organ Sharing Allocation Policy Changes.

IMPORTANCE: A new liver allocation policy was implemented by United Network for Organ Sharing (UNOS) in February 2020 with the stated intent of improving access to liver transplant (LT). There are growing concerns nationally regarding the implications this new system may have on LT costs, as well as access to a chance for LT, which have not been captured at a multicenter level. OBJECTIVE: To characterize LT volume and cost changes across the US and within specific center groups and demographics after the policy implementation. DESIGN, SETTING, AND PARTICIPANTS: This cross-sectional study col

The impact of proposed changes in liver allocation policy on cold ischemia times and organ transportation costs.

Changes to the liver allocation system have been proposed to decrease regional variation in access to liver transplant. It is unclear what impact these changes will have on cold ischemia times (CITs) and donor transportation costs. Therefore, we performed a retrospective single center study (2008-2012) measuring liver procurement CIT and transportation costs. Four groups were defined: Local-within driving distance (Local-D, n = 262), Local-flight (Local-F, n = 105), Regional-flight <3 h (Regional <3 h, n = 61) and Regional-Flight >3 h (Regional >3 h, n = 53). The median travel distance increas

Modeling the benefits and costs of integrating an acceptable HLA mismatch allocation model for highly sensitized patients.

BACKGROUND: The Eurotransplant acceptable mismatch program has improved transplantation access for highly sensitized recipients. However, the benefits and costs of implementing such a program remain unknown. METHODS: Using decision analytical modeling, we compared the average waiting time for transplantation, overall survival gains (in life-years and quality-adjusted life-years gained), and costs of integrating an acceptable mismatch allocation model compared with the current deceased-donor kidney allocation model in Australia. RESULTS: Acceptable mismatches were identified in 12 of 28 (43%)

Influence of budget allocation and design rainfall intensity on cost-effective LID strategy for urban flood mitigation.

The use of combinations of low-impact development (LID) measures is more effective than the individual measures in mitigating runoff under different design storms. The runoff reduction rate (3R) of LID measures is mainly influenced by budget constraints and the intensity of design storms. These factors ultimately influence the determination of optimal combinations. In this study, a cost-effective approach was developed to determine the optimal LID combinations for maximizing the 3R under constrained construction budgets and design storms. The proposed approach was applied to 31 urbanized subca

Optimizing resource allocation: Cost-effectiveness of specified D-dimer cut-offs in cancer patients with suspected venous thromboembolism.

An accurate diagnosis of venous thromboembolism (VTE) is crucial, given the potential for high mortality in undetected cases. Strategic D-dimer testing may aid in identifying low-risk patients, preventing overdiagnosis and reducing imaging costs. We conducted a retrospective, comparative analysis to assess the potential cost savings that could be achieved by adopting different approaches to determine the most effective D-dimer cut-off value in cancer patients with suspected VTE, compared to the commonly used rule-out cut-off level of 0.5 mg/L. The study included 526 patients (median age 65, IQ

Cost analysis and resource allocation in the management of benign thyroid nodules: a comparison of surgery and thermal ablation techniques.

INTRODUCTION: Benign thyroid nodules are common in adults, with incidental detection rates reaching 50-60% in women. While most nodules are asymptomatic and require no treatment, some grow and cause compressive symptoms. OBJECTIVE: This study evaluates the economic impact of various treatment modalities for benign thyroid nodules within the Italian healthcare system, comparing total thyroidectomy, hemithyroidectomy, and minimally invasive thermal ablation. The analysis considers both direct healthcare costs and indirect social costs, such as productivity losses, from the perspective of the It

Optimizing female allocation to reproductive technologies considering merit, inbreeding and cost in nucleus breeding programmes with genomic selection.

Female reproductive technologies such as multiple ovulation and embryo transfer (MOET) and juvenile in vitro fertilization and embryo transfer (JIVET) have been shown to accelerate genetic gain by increasing selection intensity and decreasing generation interval. Genomic selection (GS) increases the accuracy of selection of young candidates which can further accelerate genetic gain. Optimal contribution selection (OCS) is an effective method of keeping the rate of inbreeding at a sustainable level while increasing genetic merit. OCS could also be used to selectively and optimally allocate repr

[Improvement of cost allocation in gastroenterology by introduction of a novel service catalogue covering the complete spectrum of endoscopic procedures].

BACKGROUND: The German hospital reimbursement system (G-DRG) is incomplete for endoscopic interventions and fails to differentiate between complex and simple procedures. This is caused by outdated methods of personnel-cost allocation. METHODS: To establish an up-to-date service catalogue 50 hospitals made their anonymized expense-budget data available to the German-Society-of-Gastroenterology (DGVS). 2.499.900 patient-datasets (2011-2013) were used to classify operation-and-procedure codes (OPS) into procedure-tiers (e.g. colonoscopy with biopsy/colonoscopy with stent-insertion). Lessons From Analyzing the Medical Costs of Civilian Terror Victims: Planning Resources Allocation for a New Era of Confrontations.

UNLABELLED: Policy Points: Across the globe, the threat from terrorist attacks is rising, which requires a careful assessment of long-term medical support. We found 3 major sources of costs: hospital expenditures, mental health services dedicated to acute stress reactions, and ambulatory follow-up. During the first year, most of the costs were related to hospitalization and support for stress relief. During the second year, ambulatory and rehabilitation costs continued to grow. Public health specialists should consider these major components of costs and their evolution over time to properly a

Cost-effectiveness in orthopedics: providing essential information to both physicians and health care policy makers for appropriate allocation of medical resources.

Cost-effective analysis has become an important tool in helping determine what procedures are both cost-effective and appropriate in today's cost control health care. The quality-adjusted life-year (QALY) is a standard measure for health-related quality-of-life in medical cost-effectiveness research. It can be used to compare different interventions to determine the cost-effectiveness of each procedure. Use of QALY to compare health care interventions has become the new gold standard. The key words arthroscopy, cost-effectiveness analysis, QALY, shoulder, hip, knee, ankle, elbow, wrist, and pu

Return-to-work intervention for cancer survivors: budget impact and allocation of costs and returns in the Netherlands and six major EU-countries.

BACKGROUND: Return-to-work (RTW)-interventions support cancer survivors in resuming work, but come at additional healthcare costs. The objective of this study was to assess the budget impact of a RTW-intervention, consisting of counselling sessions with an occupational physician and an exercise-programme. The secondary objective was to explore how the costs of RTW-interventions and its financial revenues are allocated among the involved stakeholders in several EU-countries. METHODS: The budget impact (BI) of a RTW-intervention versus usual care was analysed yearly for 2015-2020 from a Dutch s

Modelling and simulation of blood collection systems: improvement of human resources allocation for better cost-effectiveness and reduction of candidate donor abandonment.

BACKGROUND: This study addresses the modelling and simulation of blood collection for fixed blood collection sites in a medium-sized large French city, as well as mobile blood collection in urban and rural environments. STUDY DESIGN AND METHODS: Formal Petri net models were used to describe all relevant donor flows of the various blood collection systems; the Petri net models were converted onto discrete-event simulation models, allowing the evaluation of a large number of scenarios and configurations of blood collection systems. Quantitative models were proposed that encompassed all componen

Estimates of patient costs related with population morbidity: can indirect costs affect the results?

A number of health economics studies require patient cost estimates as basic information input. However, the accuracy of cost estimates remains generally unspecified. We propose to investigate how the allocation of indirect costs or overheads can affect the estimation of patient costs and lead to improvements in the analysis of patient cost estimates. Instead of focussing on the costing method, this paper will highlight observed changes in variation explained by a methodology choice. We compare four overhead allocation methods for a specific Spanish population adjusted using the Clinical Risk
