Tidsskrift for den Norske laegeforening : tidsskrift for praktisk medicin, ny raekke2000nor
BACKGROUND: Premature closure of the metopic suture leads to inhibited growth of the frontal bones, producing a keel-shaped forehead (trigonocephaly). Simple metopic synostosis is usually sporadic. Trigonocephalic patients account for 8-16% of the referrals to craniofacial centers, with a marked male predominance. Intracranial pressure (ICP) may be increased, whereas shunt-dependent hydrocephalus is infrequent. Infrequently, patients have intra- or extracerebral anomalies; one third have varying degrees of neuropsychological problems. The treatment is primarily surgical.

The forehead is the cranial part of the forehead, and represents an important component of facial esthetics. The deformations linked to craniosynostoses are characterized by modifications in height, width, and angulation. Their surgical correction during childhood is based on well-established techniques of remodeling using resorbable osteosynthesis. Today, distraction and springs allow less invasive procedures with good outcomes. Sequellae mainly correspond to hypotrophies of the temporal region, corrected with fat grafting. The presence of a mutation increases the risk of reoperation in the f

Although it is currently thought that surgery is indicated mainly for cosmetic reasons in isolated craniosynostoses, the functional aspects of the treatment must not be underestimated. Prospective studies on intracranial pressure and mental evolution of these children have shown that there were functional consequences in a significant proportion of cases even of single suture fusion. The frequency of increased intracranial hypertension and the risk of mental impairment depend on the age of the child and the type of craniosynostosis. In nonsyndromic cases, the higher risks are observed in multi

We describe three families to highlight the variability of expression and penetrance that can occur in the craniosynostoses. In two of the families, gene carriers were only identified in retrospect by looking at photographs of other family members. In the third family, identical twins were initially thought to be discordant for sagittal craniosynostosis until early skull x rays were examined and both were found to be affected. The dilemmas faced when counselling these families are discussed.

Syndromic craniosynostose exhibit variable clinical and genetic heterogeneity. Many of this disorders are caused by mutations in the fibroblast growth factor receptor genes: FGFR2, FGFR3 (encoding fibroblast growth factor receptors), TWIST1 (functions as an upstream regulator of FGFRs) and EFNB1 (gene encoding fibrillin1). However recent advances in molecular genetics have led to a discover of other genes implicated in different craniosynostosis syndromes.

Craniosynostosis (CS) is defined as the premature fusion of cranial sutures, leading to an abnormal skull shape. The overall incidence is between 1: 2,000 and 1: 3,000 live births. Genetic causes are found in 20% of cases. CS can be isolated (non-syndromic CS/NSCS) or they can be part of multiple congenital abnormalities syndromes (syndromic CS/SCS). A few SCS, such as Crouzon, Pfeiffer, Apert and Saethre-Chotzen syndromes, are very well known and their molecular bases have been clarified in the 90s and early 2000s, thus showing the major role of the FGF receptors and TWIST signaling pathways

Craniosynostoses are the most frequent craniofacial malformations. However, with a prevalence of 3-6 cases per 10,000 live births they are amongst the rarely seen diseases and their definite diagnosis thus poses a challenge to the physician. When an abnormal calvarial configuration is detected, a radiological evaluation is necessary to characterize the deformity and to guide the corrective surgical procedure. The demand for clear diagnostic criteria is justified by the severity of the disease and the possible consequences of delayed diagnosis. In addition to the clinical signs (deformation of

OBJECTIVE: To compare our data with recent studies that have suggested a change in the distribution of the forms of nonsyndromal synostosis in the clinical population, we conducted a retrospective analysis of the diagnoses of children with isolated synostosis examined at a large craniofacial center between 1987 and 2009. This also included the range of nonsyndromal multisuture synostoses.
DESIGN: A retrospective chart review of all preoperative patients with nonsyndromal synostosis seen between 1987 and 2009 was performed. Only patients with a radiologically confirmed craniosynostosis were in

Craniosynostoses are defined as the premature closure of one or more sutures of the cranial bones. The resulting craniofacial deformities may be associated with neurologic complications owing to the disproportion between the growing brain and the limited content of the intracranial cavity. Plain skull radiographs are often contributive to the diagnosis. However, they may not be sufficient to detect early cases and to properly evaluate the more complex cases. Computed tomography has been proved to be more efficient in the pretherapeutic evaluation. The authors present here their results in the

The aim of this article was to present a treatment strategy for complex craniosynostoses, such as Apert syndrome and Crouzon's disease, based on the author's experience of over 16 years. The most favourable results of primary decompression have been achieved by the radical osteoclastic procedure described by Powiertowski. Subsequent frontal advancement and Le Fort III osteotomy are necessary in these patients when they reach 6-10 years of age. Orthopaedic treatment with a Delaire mask should be started immediately after the operation to achieve optimal growth of the skull base with anterior ro

Premature fusion of cranial sutures underlies the clinical condition of 'craniosynostosis', a common human disorder that occurs in both nonsyndromic and syndromic forms. The subgroup of syndromic craniosynostoses usually associates limb abnormalities and facial dysmorphism to skull distortion. Over the past decade, some of the genes causing these phenotypes have been identified. Among these, the gene encoding FGFR2, one of four members of the fibroblast growth factor receptor(FGFR) family, has been shown to account for several severe conditions including Apert, Pfeiffer, Crouzon, Beare-Stevens

Procedures for relief of coronal synostosis and metopic synostosis have resulted in some undesirable sequelae. The authors present combined neurological and plastic surgical modifications to prevent additional synostoses, forehead ridging, and lateral orbital wall step-off. They recommend bifrontal craniotomy with lateral wall osteotomy into the body of the zygoma and self-retaining grafts.

BACKGROUND: Although most craniosynostoses can be determined by pattern recognition, some patients defy phenotypic diagnosis and require computed tomography to determine sutural fusions.
METHODS: The authors' craniofacial database was reviewed for patients treated for craniosynostosis between 1989 and 2007. Children with single-suture synostosis, isolated bilateral coronal synostosis, and pansynostosis were excluded. Recorded variables included the pattern of sutural fusion, age at presentation, cranial shape, presence of elevated intracranial pressure, genetic testing, and types of operative

The craniosynostoses are both etiologically and pathogenetically heterogeneous and many syndromes have been delineated. The present paper highlights well-known craniosynostoses that can present asymmetrically. These include plagiocephaly resulting from premature unilateral synostosis of the coronal or lambdoid sutures. Apert syndrome, and Saethre-Chotzen syndrome.

Fibroblast growth factors are structurally related proteins associated with cell growth, differentiation, migration, wound healing, angiogenesis, and oncogenesis. At the cellular level, their function is mediated by transmembrane tyrosinekinase receptors, fibroblast growth factor receptors. Four genes encoding fibroblast growth factor receptors have been identified, and mutations in three of these, FGFR1, FGFR2, and FGFR3, can cause different congenital, autosomal dominant disorders affecting the craniofacial and skeletal development: craniosynostosis and chondrodysplasias. The craniosynostosi

Le Fort III midfacial distraction using internal and external devices is a generally accepted procedure for midfacial retrusion in patients with craniosynostoses. Many novel techniques for correction of cranial deformities in craniosynostoses are being introduced today. The authors described 14 consecutive cases of Le Fort III midfacial distraction using internal and external distraction devices. The paper discusses advantages and limitations of these methods.

Craniosynostoses belong to a heterogenous group of diseases characterized by a preterm closing of cranial sutures. The symptoms may be nonsyndromal, incomplete syndromal and syndromal. Most popular are now hereditary and genetic theories of its development. The attention is focused on the factors of fibroblast growth and receptors to these factors located on osteoblasts. Gene mutation results in alteration of protein receptor structure, increased osteoblast activity and production of great amount of osteoinducing factors, collagen, acceleration of mineralization, enhancing osteogenesis in the
