American journal of transplantation : official journal of the American Society of Transplantation and the American Society of Transplant Surgeons2019eng
Although minimized by expert evaluation, operative technique, and postoperative care, the extremely low risk of perioperative mortality following living kidney or liver donation will never be eliminated. Furthermore, anticipation of poor donor outcome may simultaneously be a source of anxiety for physicians and programs and also be a circumstance for which they are unprepared. We conducted a national survey of US transplant surgeons to understand experiences with and systemic preparedness for the event of a living donor death. Respondents represented 87 unique transplant programs (71 kidney an

INTRODUCTION: Hysteroscopy is a common gynecologic procedure increasingly performed in outpatient settings, yet in Canada cases still occur in operating rooms. As care transitions to ambulatory suites, processes for managing complications remain poorly defined. This educational initiative aimed to design in-situ simulation scenarios to enhance patient safety during outpatient hysteroscopy by strengthening team preparedness and identifying system gaps.
METHODS: A needs assessment was conducted via literature review and interviews with local staff. Findings informed the creation of 4 simulation

The COVID-19 pandemic has challenged every surgical discipline. Lessons learned from Hurricane Katrina have informed our department's management of the current crisis. That experience impressed upon us a profound appreciation for shared decision making in the face of scarce resources, an evolving clinical context, and potential harm to patients and health care workers. To that end, we have formed a Resource Utilization Committee to prospectively review all nonemergent surgical cases during the current crisis. This has allowed "state-of-the-pandemic" otolaryngologic care in a real-time, collabo

According to the Situation Report 65 of the World Health Organization of March 25, 2020, the COVID-19 incidence rate indicates 413 467 confirmed cases and 18 433 deaths. Genetic diversification of the Corona virus has resulted in strains that cause severe respiratory tract infections in humans via drip and animal mediation. S-proteins covering its surface, which bind to the cell receptor - angiotensin converting enzyme 2 (ACE-2) and transmembrane serine protease (TMPRSS2) are important in shaping virus activity. The course of infection varies from mild to severe. The ability to control infecti

The Covid-19 pandemic has further illuminated the already existing need for methods of building resilience in perinatal caregivers. Using a scoping review approach, literature was examined to identify evidence-based models of resilience building in a cohort of perinatal clinicians. Research published between January 2015 and 2020 was evaluated using PubMed, CINAHL, EMBASE, and PsycINFO databases. Of the initial 3399 records reviewed, 2 qualitative studies met the inclusion criteria. Given the deleterious effects of Covid-19 on perinatal care providers, and in light of the paucity of available

Background Treating children with cancer requires multiple different skills. For the healthcare personnel (HCP) in Germany the practice of ongoing training to improve professional skills is almost non-existent. Therefore, we developed a programme called 'SICKO' to support HCPs skills and attitudes by means of a multidisciplinary workshop. Methods Following a qualitative analysis, we then designed a modular (3 day) workshop. During day one (8 h) participants learn practical skills, the fundamentals of chemotherapy, and effective communication skills. Workshop day 2 (8 h) includes education rega

Preventable medical errors remain a leading cause of morbidity and mortality in the United States. While healthcare systems have achieved safety improvements by adopting protocols from commercial aviation, such as surgical checklists and Crew Resource Management, broad safety gains have arguably stalled. Commercial aviation relies on highly standardized environments, whereas healthcare is characterized by extreme biologic unpredictability and the need for rapid, decentralized decision-making. To break through this safety plateau, healthcare leaders must dismantle rigid hierarchies, intimidatin

BACKGROUND: Interprofessional team training is increasingly implemented in healthcare, especially in the acute care domain. Research shows a positive effect of Crew Resource Management (CRM) training on teamwork and non-technical skills, and there are indications that it might improve patient care. However, CRM training requires a lot of resources, time, and energy. There is a paucity of data on the costs of these programs. The objective of this study was to evaluate and categorize costs related to an in-situ CRM training program for surgical teams in the Netherlands.
METHODS: An evaluation o

BACKGROUND: The Ryder Cup is a men's golf competition and one of the most high-profile sporting events worldwide, presenting significant challenges for healthcare providers, especially when hosted in non-urban areas. Medical professionals must address sport-related illness and injuries in elite athletes, exacerbations of pre-existing conditions in spectators, and occupational health issues among staff and volunteers. Proper healthcare planning and infrastructure are crucial to reduce the burden on local hospitals and ensure timely care for everyone involved. Aim of the study is to describe the

BACKGROUND: Out-of-hospital-cardiac-arrest requires a robust emergency response system for survival. In low-resource settings like suburban and rural Sindh in Pakistan, little is known about the readiness of healthcare systems and communities.
AIM: The aim of this study was to comprehensively assess the system-readiness for out-of-hospital-cardiac-arrest management in rural and suburban areas of Sindh, Pakistan by studying out-of-hospital and in-hospital elements of chain of survival in these areas.
METHODS: A triangulation mixed-methods design was used. The study was conducted in seven hosp

A high-risk acute care area, such as the operating room (OR), requires seamless teamwork for the provision of safe care due to its fast-paced nature. Any breakdown in teamwork in this high-stakes setting could lead to adverse patient outcomes. Simulation-based team training strategies adopted from the principles of Crew Resource Management (CRM) from the aviation industry are a prominent modality used to train multispecialty teams in the OR. While sufficient evidence in the literature highlights the positive behavioral attitudes and improved non-technical skills of the OR teams, this review ai

BACKGROUND: Patient safety has been pursued largely through systems, protocols, and checklists, yet preventable harm remains common and effective interventions are often difficult to sustain or transfer between settings. A consistent observation is that the same intervention succeeds in one unit and fails in another, the difference lying less in the tool than in how the team functions. This review argues that an under-examined determinant of that difference is peer accountability: the lateral, reciprocal responsibility of team members to uphold shared standards and address departures from them

BACKGROUND: Cardiopulmonary resuscitation is a crucial skill for emergency medical services. As high-risk-low-frequency events pose an immense mental load to providers, concepts of crew resource management, non-technical skills and the science of human errors are intended to prepare healthcare providers for high-pressure situations. However, medical errors occur, and organizations and institutions face the challenge of providing a blame-free error culture to achieve continuous improvement by avoiding similar errors in the future. In this case, we report a critical medical error during an anaph

The onset of proteolysis targeting chimeras (PROTACs) has reshaped the entire context of targeted cancer therapy by offering a novel approach for the selective degradation of disease-causing proteins, overcoming the limitations of traditional occupancy-driven inhibition. This heterobifunctional technology recruits endogenous E3 ubiquitin ligases to mark proteins of interest (POI) for proteosomal degradation via the ubiquitin-proteasome system (UPS). Unlike conventional inhibitors, PROTACs function catalytically and can target previously "undruggable proteins", such as transcription factors, sc

The low-bacterial diet (LBD) is a widely used dietary regimen to reduce the risk of food-borne infections in patients with neutropenic cancer, but its role is controversial due to its unclear benefits. The purpose of this study was to provide an updated analysis of the available evidence on the efficacy of the LBD to reduce the risk of infections, mortality rates, and quality of life (QoL) in neutropenic patients with cancer. A systematic literature search was conducted in the biomedical databases Cochrane Library, PubMed, CINHAL, and EMBASE. The process of the screening, selection, inclusion

The COVID-19 pandemic has inevitably caused those involved in cancer care to change clinical practice in order to minimize the risk of infection while maintaining cancer treatment as a priority. General advice during the pandemic suggests that most patients continue with ongoing therapies or planned surgeries, while follow-up visits may instead be delayed until the resolution of the outbreak. We conducted a literature search using PubMed to identify articles published in English language that reported on care recommendations for cancer patients during the COVID-19 pandemic from its inception u
