Neurodevelopmental care across pediatric critical care settings: toward brain-focused critical care.

UNLABELLED: Advances in neonatal and pediatric critical care have expanded the focus from short‑term survival to include greater attention to long‑term neurodevelopmental health and family well‑being. This narrative review synthesizes the evolution of neurodevelopmental and neuroprotective care across neonatal (NICU), pediatric (PICU), and cardiovascular (CVICU/PCICU) intensive care settings. Developmental care emerged in the NICU through individualized, cue‑based approaches such as the Newborn Individualized Developmental Care and Assessment Program (NIDCAP), emphasizing stress reduction, pro

Critical care ultrasonography.

Resuscitative ultrasonography provides rapid, repeatable, and multisystem assessment to guide diagnosis and management of critically ill patients in the emergency department (ED). Cardiac ultrasonography offers new anatomic and hemodynamic information, previously unavailable in an ED setting, whereas other applications match or exceed the speed and utility of existing tests such as chest radiograph (thoracic ultrasonography) or central venous pressure determination (inferior vena cava ultrasonography). Evolving areas of resuscitative ultrasonography include neurologic applications and transeso

[Critical care echocardiography].

Critical care echocardiography (CCE) using the transthoracic approach is a noninvasive imaging tool at the bedside and is of great value in the intensive care setting because of its portability, widespread availability, and rapid diagnostic capability. Time-dependent scenarios occur during shock and during cardiopulmonary resuscitation. Potentially treatable causes of shock or cardiac arrest including hypovolemia, cardiac tamponade, left ventricular failure, or massive pulmonary embolism should be detected or excluded as soon as possible. Basic CCE is assessed by means of "focused" or "goal-di

Critical care monitoring.

Small mammals and birds present unique challenges for the clinician in treatment of life-threatening conditions. Numerous books have been written on shock, critical care, and cardiopulmonary resuscitation in small animals. The basic protocols can be adapted for use in small mammals and birds. A general review of the pathophysiology of shock is important to the understanding of fluid therapy plans discussed in this article. Using the general principles of cardiopulmonary-cerebral resuscitation in small animals, protocols are discussed for use in birds and small mammals.

Critical Care Organizations: Business of Critical Care and Value/Performance Building.

OBJECTIVE: New, value-based regulations and reimbursement structures are creating historic care management challenges, thinning the margins and threatening the viability of hospitals and health systems. The Society of Critical Care Medicine convened a taskforce of Academic Leaders in Critical Care Medicine on February 22, 2016, during the 45th Critical Care Congress to develop a toolkit drawing on the experience of successful leaders of critical care organizations in North America for advancing critical care organizations (Appendix 1). The goal of this article was to provide a roadmap and call

[Anticoagulation in critical care].

Anticoagulation is used in intensive care medicine to prevent and treat thromboembolism. The increased risk of bleeding in this patient population requires a personalized approach to management including individualized anticoagulant selection and dose adjustment. While low-molecular-weight heparins are equivalent or superior to unfractionated heparin for thromboprophylaxis, unfractionated heparin is the preferred therapy for the treatment of thromboembolism. Its advantages include a relatively short half-life, the ability to monitor therapy using the activated partial thromboplastin time and t

Ophthalmology in Critical Care.

Ophthalmic disease encountered in the intensive care unit (ICU) has a wide spectrum of prevalence and severity. Prolonged exposure of the cornea is common and preventable. Trauma, glaucoma, infection, vascular disease, and burns are among the potential causes of vision loss. Patients are predisposed to ocular complications by the ICU environment and critical illness itself. Critically ill patients require prioritization of life-sustaining interventions, and less emphasis is placed on ophthalmic disease, leading to missed opportunities for vision-saving intervention. It is therefore imperative

Otolaryngology in Critical Care.

Diseases affecting the ear, nose, and throat are prevalent in intensive care settings and often require combined medical and surgical management. Upper airway occlusion can occur as a result of malignant tumor growth, allergic reactions, and bleeding events and may require close monitoring and interventions by intensivists, sometimes necessitating surgical management. With the increased prevalence of immunocompromised patients, aggressive infections of the head and neck likewise require prompt recognition and treatment. In addition, procedure-specific complications of major otolaryngologic pro

Bioterrorism and critical care.

A bioterrorist attack of any kind has the potential to overwhelm a community and, indeed, in the case of smallpox, an entire nation. During such an attack the number of patients requiring hospitalization and specifically critical care is likely to be enormous. Intensivists will be at the forefront of this war and will play an important role in dealing with mass casualties in an attempt to heal the community. A high degree of suspicion and prompt recognition of an event will be required to contain it. Specific knowledge of the possible agents that can be used will be key in managing patients an

Critical care in obstetrics.

Pregnancy is a normal physiologic process with the potential for pathologic states. Pregnancy has several unique characteristics including an utero-placental interface, a physiologic stress that can cause pathologic states to develop, and a maternal-foetal interface that can affect two lives simultaneously or in isolation. Critical illness in pregnant women may result from deteriorating preexisting conditions, diseases that are co-incidental to pregnancy, or pregnancy-specific conditions. Successful maternal and neonatal outcomes for parturients admitted to a maternal critical care facility ar

Critical care in pregnancy.

Childbirth is a major event in the lives of mothers and their families. Critical illness in pregnancy is uncommon but may arise from conditions unique to pregnancy, conditions exacerbated by pregnancy and coincidental conditions. According to the latest Confidential Enquiry into Maternal Deaths in the UK, haemorrhage remains a leading direct cause of mortality; however, there has been an increase in mortality due to indirect causes. The obstetric population has changed over the past decade and we are caring for much older mothers with pre-existing disorders and advanced chronic medical conditi

Critical care of neurotrauma.

Traumatic brain injury often affects people in their most productive years, inflicting a significant burden on families and society. The advances in modern critical care have improved survival of patients; thus more patients live after traumatic brain injury, which raises an important issue about their neurologic outcome. At the present time, there are limited data regarding methods to optimize neurologic recovery. In this review, we try to bring information from different sources to show new approaches to achieve that goal. Some of the techniques employed are investigational and some are wait

Critical care infectious disease.

Septic shock, toxic shock syndrome, acute respiratory distress syndrome, and catheter-related infections are conditions in which intensive care management of the patient may be necessary. Toxic shock syndrome is a toxin-mediated illness that is not limited to young menstruating women and should be considered in women and men who present with fever, hypotension, rash, and multiorgan dysfunction. Sepsis is the leading cause of death in critically ill patients in the United States and is the most common predisposing factor for acute lung injury or acute respiratory distress syndrome. Year in review 2011: Critical Care--Neurocritical care.

Contributions from the neurosciences to Critical Care in 2011 covered an array of topics. We learned about potential biomarkers for, and the effect of cerebral oxygen metabolism on, delirium, in addition to treatment of the latter. A group of investigators studied surface cooling in healthy awake volunteers, and incidence of infection associated with therapeutic hypothermia. The effects of statin and erythropoietin on stroke were revisited, and the role of adhesion molecule in the inflammatory reaction accompanying intracerebral hemorrhage was scrutinized. Biomarkers in subarachnoid hemorrhage
