Five-Year Trends of Critical Care Practice and Outcomes.

BACKGROUND: Longitudinal analyses of large, detailed adult critical care datasets provide insights into practice trends and generate useful outcome and process benchmarks. METHODS: Data representing 991,571 consecutive critical care visits to 160 US adult ICUs from 2009 to 2013 from the eICU Research Institute clinical practice database were used to quantitate patient characteristics, APACHE IV-based acuity predictions, treatments, and outcomes. Analyses for changes over time were performed for patient characteristics, entry and discharge locations, primary admission diagnosis, treatments, ad

Telemedicine critical care availability and outcomes among mechanically ventilated patients.

PURPOSE: Telemedicine Critical Care (TCC) improves adherence to evidence based protocols associated with improved mortality among patients receiving invasive mechanical ventilation (IMV). We sought to evaluate the relationship between hospital availability of TCC and outcomes among patients receiving IMV. MATERIALS AND METHODS: We performed a cross-sectional study of 66,522 adults who received IMV for non-postoperative acute respiratory failure at 318 non-federal hospitals in New York, Massachusetts, Maryland, and Florida in 2018. Hospital-level TCC availability was ascertained from the 2018

Development of a core outcome set for pediatric critical care outcomes research.

BACKGROUND: Pediatric Intensive Care Unit (PICU) teams provide care for critically ill children with diverse and often complex medical and surgical conditions. Researchers often lack guidance on an approach to select the best outcomes when evaluating this critically ill population. Studies would be enhanced by incorporating multi-stakeholder preferences to better evaluate clinical care. This manuscript outlines the methodology currently being used to develop a PICU Core Outcome Set (COS). This PICU COS utilizes mixed methods, an inclusive stakeholder approach, and a modified Delphi consensus p

Progress on core outcome sets for critical care research.

PURPOSE OF REVIEW: Appropriate selection and definition of outcome measures are essential for clinical trials to be maximally informative. Core outcome sets (an agreed, standardized collection of outcomes measured and reported in all trials for a specific clinical area) were developed due to established inconsistencies in trial outcome selection. This review discusses the rationale for, and methods of, core outcome set development, as well as current initiatives in critical care. RECENT FINDINGS: Recent systematic reviews of reported outcomes and measurement instruments relevant to the critic

Admission pattern and outcome in critical care obstetric patients.

We undertook a six-year retrospective study to determine the pattern of admission and outcome for obstetric patients admitted to the intensive care unit of the University of Nigeria Teaching Hospital, Enugu. The hospital records (case notes and intensive care unit records) were used to extract the necessary data. A total of 816 patients were admitted to the intensive care unit during the period under review. Eighteen (2.2%) were obstetric patients. Nine (50%) were preeclamptic and eclamptic patients. Four patients (22.2%) had obstetric haemorrhage. Five others presented with the following: ast

Critical Care Nurse Practitioner Outcomes: A Scoping Review of the Literature.

AIM: To support the development of nursing-sensitive indicators specific to this role. BACKGROUND: The role of the advanced critical care nurse practitioner is a relatively new role in the Irish healthcare system. The role is attributed to the provision of high-quality care to patients. Preparing to evaluate the role is proving challenging due to the varying inconsistencies and responsibilities across hospitals. Nursing-sensitive indicators are measurements where nursing care has had a direct influence on patient outcomes. DESIGN: A scoping review. METHODS: The review was conducted using th

Outcomes after critical care admission in people with a learning disability.

INTRODUCTION: People with learning disabilities experience worse healthcare outcomes than the general population. There is evidence that they are more likely to experience avoidable mortality and less likely to receive critical care interventions during an acute illness. Decisions regarding critical care admission or intervention must be based on evidence of whether a patient will receive lasting benefit from a critical care admission. We therefore investigated outcomes from critical care admissions in people with learning disabilities and compared them to general critical care patients. METH

Hearing screening outcomes in pediatric critical care survivors: a 1-year report.

BACKGROUND: Hearing loss is a potentially serious complication that can occur after surviving a critical illness. Study on screening for hearing problems in pediatric critical care survivors beyond the neonatal period is lacking. This study aimed to identify the prevalence of abnormal hearing screening outcomes using transitory evoked otoacoustic emission (TEOAE) screening in children who survived critical illness and to find possible associating factors for abnormal hearing screening results. METHODS: This study was a single-center, prospective, observational study. All children underwent ot

Critical care management of hydrofluoric acid burns with a negative outcome.

Hydrofluoric acid is a highly corrosive acid widely used in various industries. When in contact with skin it causes local and systemic reactions due to the generation of fluoride ions. Severe burns are associated with high mortality rates, approaching 100%. We present a 21-year-old man with 15% full thickness burns, severe metabolic acidosis, hypoxia and electrolyte disturbances. The burns were treated with topical and subcutaneous injections of calcium gluconate, and the patient was given intravenous fluid, calcium chloride, magnesium and insulin-glucose infusions. Continuous renal replacemen

Vasopressor Use, Critical Care Management, and Outcomes in Dihydropyridine Calcium Channel Blocker Toxicity.

INTRODUCTION: Although dihydropyridine calcium channel blockers (DHP CCBs) are considered to have less direct myocardial toxicity than non-dihydropyridines, DHPs remain a common cause of morbidity and mortality. We sought to examine various indices of critical illness and describe the clinical course of a population of DHP CCB-poisoned patients with special attention to vasopressor dosing and ischemic complications. METHODS: This is a retrospective chart review of DHP CCB exposures admitted to a single center. The study site was a single tertiary referral center with an in-house medical toxic

Characterizing Bronchoscopy in Pediatric Critical Care: A Cohort Analysis of Practice and Outcomes.

BackgroundFlexible bronchoscopy (FB) has become an essential diagnostic and therapeutic tool in pediatric intensive care units (PICUs). Despite increasing adoption, comprehensive data on FB's current use, clinical impact and safety in critically ill children, particularly those supported by extracorporeal membrane oxygenation (ECMO), remain limited. We aimed to describe the utilization, findings, microbiological yield and complications of FB in critically ill pediatric patients, including its diagnostic and therapeutic applications and associated physiologic effects.MethodsThis retrospective c
