Decision making with clinical diagnostic tests in terms of statistical decision theory and information theory.

The value of a diagnostic test for an individual patient is expressed in terms of the test's ability to determine whether the patient does or does not have a disease. For diagnostic tests with a continuous diagnostic variable, various decision thresholds can be selected to adjust the sensitivity and specificity of the diagnostic test to conform with the clinician's concerns (e.g., health costs, financial costs, and diagnostic information of the test). Three methods of statistical decision theory and information theory are presented for critically evaluating the value of a clinical diagnostic t

Academic decision making and prospect theory.

Two studies are reported that investigate the applicability of prospect theory to college students' academic decision making. Exp. 1 failed to provide support for the risk-seeking portion of the fourfold pattern predicted by prospect theory but did find the greater weighting of losses over gains. Using a more sensitive dependent measure, in Exp. 2 the results of the first experiment were replicated in terms of the gain-loss effect and also found some support for the fourfold pattern in the interaction between probabilities and gain versus loss. The greatest risk-seeking was found in the high p

Decision analysis with cumulative prospect theory.

BACKGROUND: Individuals sometimes express preferences that do not follow expected utility theory. Cumulative prospect theory adjusts for some phenomena by using decision weights rather than probabilities when analyzing a decision tree. METHODS: The authors examined how probability transformations from cumulative prospect theory might alter a decision analysis of a prophylactic therapy in AIDS, eliciting utilities from patients with HIV infection (n = 75) and calculating expected outcomes using an established Markov model. They next focused on transformations of three sets of probabilities: 1)

Hierarchical Decision-Making Theory: A Cognitive-Affective Framework for Explaining Decision Quality.

Drawing on the trade-off patterns of "smaller-sooner-loss for larger-later-gain" (SSL-LLG) and "smaller-sooner-gain for larger-later-loss" (SSG-LLL), this study proposes a hierarchical decision-making theory from a dual-system (cognitive-affective) perspective. The theory posits that decision quality arises from the synergy between cognitive maturity-the capacity to weigh long-term interests-and emotional maturity-the ability to maintain affective independence. This yields a four-type, three-level taxonomy of decision makers. We tested our hypotheses across behavioral experiments, eye-tracking

Optimal metacognitive decision strategies in signal detection theory.

Signal detection theory (SDT) has long provided the field of psychology with a simple but powerful model of how observers make decisions under uncertainty. SDT can distinguish sensitivity from response bias and characterize optimal decision strategies. Whereas classical SDT pertains to "type 1" judgments about the world, recent work has extended SDT to quantify sensitivity for metacognitive or "type 2" judgments about one's own type 1 processing, e.g. confidence ratings. Here we further advance the application of SDT to the study of metacognition by providing a formal account of normative meta

Opportunities for theory-informed decision science in cancer control.

Cancer prevention and control involves navigation of complex clinical decisions, often laden with uncertainty and/or intricate interpersonal dynamics, which have implications for both physical health and quality of life. Cancer decision-making research in recent decades has primarily focused on working to improve the quality of decisions by providing patients with detailed information about their choices and through an increased emphasis in medicine on the importance of shared decision making. This emphasis is reflective of a model of decision making that emphasizes knowledge, options, and del

The potential use of game theory in decision-making in CHD.

Background: Congenital cardiac care involves multiple stakeholders including patients and their families, surgeons, cardiologists, anaesthetists, the wider multidisciplinary team, healthcare providers, and manufacturers, all of whom are involved in the decision-making process to some degree. Game theory utilises human behaviour to address the dynamics involved in a decision and what the best payoff is depending on the decision of other players. Aim: By presenting these interactions as a strategic game, this paper aims to provide a descriptive analysis on the utility and effectiveness of game t

A statistical theory of optimal decision-making in sports betting.

The recent legalization of sports wagering in many regions of North America has renewed attention on the practice of sports betting. Although considerable effort has been previously devoted to the analysis of sportsbook odds setting and public betting trends, the principles governing optimal wagering have received less focus. Here the key decisions facing the sports bettor are cast in terms of the probability distribution of the outcome variable and the sportsbook's proposition. Knowledge of the median outcome is shown to be a sufficient condition for optimal prediction in a given match, but a

Dynamic prospect theory: Two core decision theories coexist in the gambling behavior of monkeys and humans.

Research in the multidisciplinary field of neuroeconomics has mainly been driven by two influential theories regarding human economic choice: prospect theory, which describes decision-making under risk, and reinforcement learning theory, which describes learning for decision-making. We hypothesized that these two distinct theories guide decision-making in a comprehensive manner. Here, we propose and test a decision-making theory under uncertainty that combines these highly influential theories. Collecting many gambling decisions from laboratory monkeys allowed for reliable testing of our model

Regret theory-based clinical efficacy evaluation method with three-way decision.

Scientific assessment of clinical efficacy regarding treatment regimens is crucial for ensuring healthcare quality and improving patient prognosis. For most diseases, efficacy evaluation process of treatment regimens is a temporal process. Under the intervention of each treatment regimen, a patient's efficacy information often alternates among improvement, deterioration, or stability of the condition. This dynamic variation demonstrates that the clinical efficacy evaluation problem is inherently a three-way classification problem involving temporal data. While numerous clinical efficacy evalua

I want to believe: delusion, motivated reasoning, and Bayesian decision theory.

Introduction: Several arguments suggest that motivated reasoning (occurring when beliefs are not solely shaped by accuracy, but also by other motives such as promoting self-esteem or self-protection) is important in delusions. However, classical theories of delusion disregard the role of motivated reasoning. Thus, this role remains poorly understood. Methods: To explore the role of motivated reasoning in delusions, here we propose a computational model of delusion based on a Bayesian decision framework. This proposes that beliefs are not only evaluated based on their accuracy (as in classical t

Certainty-Based Marking on Multiple-Choice Items: Psychometrics Meets Decision Theory.

When a response to a multiple-choice item consists of selecting a single-best answer, it is not possible for examiners to differentiate between a response that is a product of knowledge and one that is largely a product of uncertainty. Certainty-based marking (CBM) is one testing format that requires examinees to express their degree of certainty on the response option they have selected, leading to an item score that depends both on the correctness of an answer and the certainty expressed. The expected score is maximized if examinees truthfully report their level of certainty. However, prospe

Using physiological models and decision theory for selecting appropriate ventilator settings.

OBJECTIVE: To present a decision support system for optimising mechanical ventilation in patients residing in the intensive care unit. METHODS: Mathematical models of oxygen transport, carbon dioxide transport and lung mechanics are combined with penalty functions describing clinical preference toward the goals and side-effects of mechanical ventilation in a decision theoretic approach. Penalties are quantified for risk of lung barotrauma, acidosis or alkalosis, oxygen toxicity or absorption atelectasis, and hypoxaemia. RESULTS: The system is presented with an example of its use in a post-su

A grounded theory of youth athlete concussion under-reporting decisions.

BACKGROUND: Sport-related concussions leave youth vulnerable to significant health, physical, social, and educational disruptions. Many youth athletes under-report concussions, but the scope of the issue for this population is not well understood, nor are the underlying motivations for (under)reporting. It is necessary to examine the relationship between sport-related concussion knowledge and reporting and invite athletes to share their decision-making processes. The purpose of the research was to understand how athletes' attitudes, experiences and beliefs regarding concussions influence their

Application of information theory to decision analysis in potentially prostaglandin-responsive neonates.

To define settings in which use of prostaglandin E1 before transfer from a community hospital to a tertiary care center benefits neonates with possible heart disease, information theory was used to predict the probability of a favorable response to prostaglandin therapy from the limited information of clinical variables. Records of 250 patients, newborn to 7 days old, with suspected heart disease were reviewed to assess six clinical variables (cyanosis, respiratory distress, heart murmur, pulse contour, hepatomegaly and prematurity). According to the anatomic and hemodynamic cardiovascular con

Primate anterior insular cortex represents economic decision variables proposed by prospect theory.

In humans, risk attitude is highly context-dependent, varying with wealth levels or for different potential outcomes, such as gains or losses. These behavioral effects have been modelled using prospect theory, with the key assumption that humans represent the value of each available option asymmetrically as a gain or loss relative to a reference point. It remains unknown how these computations are implemented at the neuronal level. Here we show that macaques, like humans, change their risk attitude across wealth levels and gain/loss contexts using a token gambling task. Neurons in the anterior

When to give aspirin to prevent preeclampsia: application of Bayesian decision theory.

BACKGROUND: There is good evidence that first-trimester assessment of the risk for preterm preeclampsia and treatment of the high-risk group with aspirin reduces the incidence of preterm preeclampsia. Furthermore, there is evidence that aspirin is associated with an increased risk of maternal and neonatal hemorrhagic complications. Against this background, there are ongoing debates whether aspirin should be recommended for all women or to a subpopulation of women predicted to be at increased risk of developing preeclampsia. Moreover, if a strategy of the prediction and prevention of preterm pr

Inverse decision theory with applications to screening and diagnosis of cervical intraepithelial neoplasia.

OBJECTIVE: Medical decision makers would like to use decision theory to determine optimal treatment strategies for patients, but this requires priors, likelihoods, and losses. It can be very difficult to specify a loss or utility function in a medical setting, especially when considering both patient health outcomes and economic costs. These issues led to the development of Inverse Decision Theory (IDT), which involves determining the set of losses under which a given decision rule is optimal. METHODS: We apply IDT to the current standard of care for the diagnosis and treatment of precancerou
