Surgical technique for trigeminal microvascular decompression.

BACKGROUND: Microvascular decompression (MVD) is a non-ablative technique designed to resolve the neurovascular conflict responsible for typical idiopathic trigeminal neuralgia (TN). METHOD: With the patient in a supine position, a small elliptical retrosigmoid craniectomy is used to approach the cerebellopontine angle and the trigeminal nerve. After careful exploration of the trigeminal root entry zone, the offending vessel is identified and moved away. Oxidized regenerated cellulose is used to keep the vessel in its new position far from the nerve. CONCLUSION: MVD represents the gold stand

[Surgical decompression for massive cerebellar infarction].

The authors report 10 patients with progressive neurological deterioration due to massive cerebellar infarctions. Computerized tomography scans confirmed obstructive hydrocephalus and brain stem compression. All 10 patients (seven men, three women; mean age, 59 years) were treated by external ventricular drainage and decompressive suboccipital craniectomy. After discharge from the hospital, they were followed up (23-101 months) and their functional independence was evaluated by the Barthel Index. The condition of three patients with brain-stem infarction had deteriorated despite decompressive

Surgical decompression of idiopathic facial palsy.

There is no consensus on the cause of Bell's palsy, just as there is no proven medical therapy. Disregarding the numerous theoretic causes, preponderant anatomic, electrophysiologic, radiologic, clinical, and pathologic evidence supports entrapment at the medical foramen of the fallopian canal as a final common pathway resulting in facial nerve ischemia and degeneration. Middle cranial fossa decompression of the meatal foramen may benefit those patients most severely affected with Bell's palsy.

Surgical decompression in traumatic optic neuropathy.

BACKGROUND: The management of traumatic optic neuropathy remains controversial. This retrospective study presents the results of 19 cases of indirect optic neuropathy where surgical decompression represented the primary mode of treatment. METHODS: Nineteen patients (20 optic nerves) with traumatic optic neuropathy underwent primary surgical decompression. The surgery was performed using the transethmoidal technique. RESULTS: The extent of bony optic canal injury documented during surgery indicated that this had been underestimated in this series by CT findings. Vision improved in eight patie

Surgical orbital decompression for thyroid eye disease.

BACKGROUND: Orbital decompression is an established procedure for the management of exophthalmos and visual rehabilitation from optic neuropathy in cases of thyroid eye disease. Numerous procedures for removal of orbital bony wall, fat or a combination of these for a variety of indications in different stages of the disease have been well reported in the medical literature. However, the relative effectiveness and safety of these procedures in relation to the various indications remains unclear. OBJECTIVES: To review current published evidence for the effectiveness of surgical orbital decompre

Surgical decompression of impingement in the weightbearing shoulder.

Shoulder pain is a common affliction, posing particular limitations on the spinal cord injured person. Abnormalities of the rotator cuff mechanism are a common cause of shoulder injury in the general population and it has recently been observed that a large percentage of persons with paraplegia suffer from chronic shoulder pain. This report describes six cases of impingement syndrome in four spinal cord injured persons who depend on upper extremity function for the execution of activities of daily living and mobility. All had failed prolonged trials of conservative therapy and subsequently und

Surgical outcomes of decompression, decompression with limited fusion, and decompression with full curve fusion for degenerative scoliosis with radiculopathy.

STUDY DESIGN: A retrospective clinical cohort study at a single spine center of patients with degenerative scoliosis and radiculopathy severe enough to require surgery. OBJECTIVE: To evaluate the functional outcomes of 3 surgeries for degenerative scoliosis with radiculopathy; decompression alone, decompression and limited fusion, and decompression and full curve fusion. SUMMARY OF BACKGROUND DATA: Although these 3 surgical treatments have all been described for this problem, there exists little information as to what outcomes to expect. METHODS: The study cohort consisted of 85 patients wh

Outcomes of surgical paraclavicular thoracic outlet decompression.

BACKGROUND: Thoracic outlet syndrome (TOS) is a constellation of signs and symptoms caused by compression of the neurovascular structures in the thoracic outlet. These structures include the brachial plexus, the subclavian vein, and the subclavian artery, resulting in neurogenic (NTOS), venous (VTOS), and arterial (ATOS) types of TOS, respectively. The purpose of this study was to evaluate the outcomes of paraclavicular surgical decompression for TOS. METHODS: A prospective analysis of patients who underwent surgical decompression for TOS at a newly established center was performed. Diagnosis

Surgical decompression in bell's palsy - our viewpoint.

Bell's palsv or acute idiopathic lower motor neuron facial palsy is a common cause of facial palsy seen in clinical practice. The cause of this disease is not exactly known though there arc many theories regarding Ils etiology. In this article, we present the theory of "Tertiary is chacmia " in eases of pertinent Bell's palsy. Our concept of Tertiary ischaemia is further supported by histological findings of the nerve sheath biopsy taken during surgical decompression In eases of Bell' s palsy. The procedure, of performing posterior tympanotomy and improving the exposure of the middle ear struc

Comparative Analysis of Decompression Versus Decompression and Fusion for Surgical Management of Lumbar Spondylolisthesis.

OBJECTIVE: When lumbar stenosis involves spondylolisthesis, many surgeons include fixation. Two recent trials have shown no consensus to definitive treatment. We aimed to add to the discourse of fusion versus decompression in patients with lumbar spondylolisthesis by providing a large-scale generalizable study. METHODS: We used multicenter, prospectively collected data from the American College of Surgeons National Surgical Quality Improvement Program database to compare 30-day outcomes for decompression alone versus combination decompression and fusion in the treatment of lumbar spondylolist

Infinity decompression: a stepwise biportal endoscopic over-the-top decompression - surgical technique and outcomes.

PURPOSE: To describe the Infinity Decompression (ID), a standardized 13-step workflow for unilateral biportal endoscopic (UBE) over-the-top (OTT) decompression of lumbar central stenosis, and to compare its perioperative and short-term clinical outcomes with conventional “N” technique with piecemeal flavectomy (N-PF). METHODS: We retrospectively analyzed consecutive patients with single-level lumbar central canal stenosis treated by UBE OTT decompression by the same surgeon. Forty cases treated with N-PF were compared with 40 cases treated with the ID technique. Demographic data, operative tim

[Surgical Techniques for Microvascular Decompression in Hemifacial Spasm].

This study discusses the key microvascular decompression(MVD)techniques for the treatment of hemifacial spasm(HFS). The author's experience is based on their institution's cases, highlighting three critical techniques. (1)Vertebral artery(VA)repositioning: Repositioning the VA is essential in challenging cases. This article focuses on the relocation of the proximal portion of the VA and the importance of careful dissection. (2)Relocating vessels compressing the peripheral branches of the facial nerve: HFS can result from nerve compression at various locations, including the cisternal portion.

Strategies in Surgical Decompression for Thyroid Eye Disease.

Surgical management of thyroid eye disease- (TED-) associated morbidity has been plagued by the complex interplay of different operative techniques. Orbital decompression is the well-recognized procedure for disfiguring exophthalmos and dysthyroid optic neuropathy (DON). There are numerous published techniques described for the removal of the orbital bone, fat, or a combination. The diverse studies are noncomparative as they include different indications, stages of disease, and methods of evaluation. Thus, it is difficult to conclude the most efficient decompression technique. To obtain effect

Endoscopically assisted proximal radial nerve decompression: surgical technique.

STATE OF THE ART: The proximal radial nerve compression syndrome includes supinator syndrome and proximal radial nerve constrictions. This article presents a new endoscopic assisted radial nerve decompression surgical technique described for the first time by Leclère et al. in 2013. SURGICAL TECHNIQUE: Endoscopic scissor decompression of the proximal radial nerve is always performed under plexus anaesthesia. It includes 8 key steps documented in this article. We review the indications and limitations of the surgical technique. CONCLUSION: Early clinical results after endoscopic assisted deco
