[Degloving injuries of the lower limb].

Although seldom dangerous to life, these degloving injuries are all potentially infected and, unless treated as acute surgical emergencies, inevitably lead to serious complications. Diagnostic is done according to a standardized protocol, which eventually must be integrated in the standard polytrauma management. Multidisciplinary (orthopedic surgery, plastic surgery, dermatology, physiotherapy) defect management is of utmost importance and requires an "integrated therapy concept". The success or failure of primary treatment of degloving injuries is determined by an adequate primary care includ

Degloving injuries of the abdominal wall.

OBJECTIVE: To stress the local, regional and global impact of degloving injuries of abdominal wall based on which the surgeon can design the management protocol. METHOD: A retrospective series of seven cases who met with high velocity run-over accidents between the year 2002 to 2007. All patients were clinically examined and the findings confirmed radiologically, which guided the management. RESULTS: Out of the seven patients treated, six survived. All patients had open degloving injury of abdominal wall and in different areas of the lower limbs, while three had an additional closed deglovin

Heel Pad Degloving Injuries: A Case Series.

INTRODUCTION: Heel pad degloving injuries are rare but serious injuries, with minimal data available regarding optimal management. In this study, a series of patients treated for heel pad degloving injuries in association with ipsilateral lower leg and foot fractures is described. METHODS: Records were retrospectively reviewed for patients aged 18 years and older treated for heel pad degloving injuries associated with ipsilateral foot or ankle fractures at one American College of Surgeons Level 1 center between January 1, 2005, and January 1, 2024, screened using International Classification

Degloving injuries of the hand and fingers.

Wound coverage after a complete degloving injury of the hand and fingers is 1 of the most difficult problems in hand surgery. Important structures such as tendons, nerves, and bones are exposed and will necrose if not covered adequately. The goal of treatment should be coverage with a pliable, sensitive, and cosmetically similar tissue that will allow early mobilization. The authors' experience has led them to adopt the following general guidelines: replantation of the avulsed structures whenever possible; early transfer of a free composite flap from the foot for a complete and irreparable deg

[Degloving injury of the thorax and abdomen].

A 22-year-old man was caught between a truck and a wagon. His chest was torn apart from the sternum downward; both lungs and the heart were visible. The skin and mucosa were covered with oil from the truck. He was immediately treated with intubation, massive fluid infusion and--after stabilisation--surgery. Afterwards, two plastic surgeries were performed. On the 27th day he was dismissed from hospital. Except for a cicatrice on the upper chest, he is in his usual condition. He survived this massive injury because of immediate intubation at the scene of the accident. Fortunately, none of the i

Degloving injury of small intestine.

Self-inflicted stab wounds are seldom very grave. We report a 25-year-old man who inflicted stab wounds with a glass piece under the influence of alcohol. This led to evisceration and later degloving injury to the small bowel. He recovered following surgery.

A case of degloving injury of the colon.

The left side of a truck driven by a 71-year-old man was hit by another car at an intersection, and his abdomen was compressed by the steering wheel. On arrival, he complained of severe lower abdominal pain; and physical examination demonstrated involuntary rigidity and rebound tenderness. Enhanced truncal computed tomography exhibited that the descending colon was shifted ventral and medial because of a low-density mass with contrast extravasation. An emergency laparotomy disclosed injuries of the descending colon, which could be mobile because of degloving of the colon, left mesenterium, spl

Massive degloving injury of the trunk.

A case of massive degloving injury of the perineum, thigh, and buttocks is presented. Hemostasis was achieved with a pneumatic anti-shock garment (PASG), followed by direct suturing of bleeding areas. A colostomy was performed. Initial conservative debridement was followed in ten days by multiple skin grafts. The patient was treated with sodium bicarbonate and mannitol to preclude myoglobinuric renal failure. Intravenous hyperalimentation was also utilized.

Microsurgical arterialization of degloving injuries of the upper limb.

The authors describe a technique in which a microsurgical anastomosis between an artery and a vein in an afferent direction (through a vein graft, when necessary) of a degloved flap is used to revascularize or arterialize the flap. In 3 difficult cases, there was substantial salvage of flap tissue.

Good practice in the management of serious degloving injuries.

Many lacerations involve degloving, a form of avulsion in which skin is torn off the underlying tissue. Patients with lacerations involving degloving must be managed appropriately so that they do not experience too much pain and to ensure that wound healing is not delayed. This article presents a case study involving a patient with a degloving injury of the leg. It examines the important features of her journey of care, and discusses factors that influence wound healing and outcomes.

Barbed Suture as a Treatment Approach in Complex Degloving Injuries.

In the late 19th century, French physician Morel-Lavallée was challenged with a group of patients who sustained similar patterns of degloving injuries, which today carry his eponym. In 1853, he reported a series of cases as well as proposed strategies for the management of these complex degloving injuries. Treatment strategies have not varied significantly over the years, and these lesions continue to plague surgeons today with failure rates in excess of 50%. We present 2 case series using barbed suture in the management of these complex injuries.

Reconstruction of total degloving injuries of the foot in children.

BACKGROUND: The aim of this study is to introduce experiences of reconstruction of total degloving injuries of the foot in children. METHODS: Seven children, five male and two female, were treated for total degloving injury of the foot at our medical institution between January 2002 and December 2008. Patients were between 5 years and 9 years of age with a mean age of 7 years. All injuries involved the total foot. In cases 1-3, the foot was covered by intermediate split-thickness skin graft. In cases 4-7, the dorsal aspect of the foot was covered by full-thickness skin graft obtained from the

Severe lower extremities degloving injuries--medical problems and treatment results.

Degloving injuries consist in detachment of skin and subcutaneous tissue from underlying fascia and muscles. They mostly address total surface of extremities or trunk, resulting in high morbidity and mortality. Their treatment, due to severity of the injury, high percentage of serious concomitant injuries and massive blood loss is time-consuming and its results are often unfavourable. The authors present results of treatment of three cases of patients with degloving injuries of the lower extremities and trunk. The patients presented poor healing prognosis connected with the extension of the in

[Degloving injuries of lower extremity--proposal of a treatment protocol].

OBJECTIVE: Degloving injuries on the lower extremities are often serious injuries. It is difficult to decide on the most appropriate treatment, whether flap repositioning and suturing or converting the avulsed flap to split-thickness skin grafting. METHODS: This study assessed patients with degloving injuries in lower extremities, reviewing the epidemiological profile and treatment performed. It is proposed a treatment protocol for management of those lower extremity avulsion injuries. RESULTS: Twenty-one patients were evaluated. The cause of trauma was running over in 11 patients (52.4%) an

Missed closed degloving injuries: late presentation as a contour deformity.

Closed degloving injuries are uncommon, and the diagnosis may be missed at the time of acute trauma. This study reports on seven patients seen during a 5-year period who presented with a contour deformity a number of months after the initial closed degloving injury. The median time to presentation was 1.4 years. In five of the seven patients, the contour deformity was present on the lateral thigh, and in two patients it occurred on the calf. Liposuction was employed to treat five patients, and all these patients reported an improvement in the contour of their limb. In two patients, because of

Degloving injuries. A retrospective study at the University Hospital Rotterdam.

Degloving injuries are severe and frequently underestimated lesions. In a retrospective study, 65 patients treated between 1985 and 1991 were reviewed. Therapy consisted of surgical exploration as soon after injury as possible, with defatting of the avulsed skin and its replacement on the most functionally important sites. The remaining raw areas were covered with split skin grafts (SSG). This technique reduced morbidity, hospital stay and work incapacity, as compared with patients treated by other methods.

Replantation or revascularization for the treatment of hand degloving injuries.

PURPOSE: The purpose of this study was to explore effective strategies of replantation or revascularization for the treatment of hand degloving injuries. METHODS: Ten patients who had received replantation or revascularization surgery for hand degloving injury between 2002 and 2014 were included for this study. The average age at the time of surgery was 34.5 years (range 18-49 years). There were nine left and one right hand replantations. The cause of injury was an industrial machine press in all of them. The skin was avulsed from the palm to the distal phalangeal level in five patients, to t
