Is deinstitutionalization working in our community?

This exploratory study examined the impact of deinstitutionalization on consumers admitted to a regional community care unit (CCU) between 1996 and 2007, and looked at lengths of stay and re-admissions to acute psychiatric care units and the impact this might have on quality of life. The results showed that the original and current residents of CCU have improved quality of life through friendships, a home-like environment, and reduced re-admissions to acute psychiatric care units; however, further improvements can be made with more emphasis on employment/vocational services and social inclusio

Deinstitutionalization and criminalization: tinkering in the interstices.

The mental health service system has undergone continuous change for more than three decades. Undoubtedly, some of its problems are related to economic, social and political factors. This contribution discusses problems of deinstitutionalization, homelessness and criminalization of the mentally ill persons. To conclude, it proposes a number of ways by which better services could be rendered to the mentally disordered persons, particularly in areas that interface with the criminal justice system.

"Community Care": Historical Perspective on Deinstitutionalization.

This article examines the discrepancies between the rhetoric surrounding deinstitutionalization and community care and the reality of the abandonment of the seriously mentally ill to their fate. It discusses how the earlier commitment to the asylum came to be abandoned, analyzes various attempts to explain why the mentally ill were decanted from institutions into a largely unreceptive community, and dissects the shortcomings of these actions. The realities of care in the community and the connections of deinstitutionalization to the collapse of public psychiatry, the epidemic of homelessness,

Elderly schizophrenic inpatients in the wake of deinstitutionalization.

The authors investigated the factors relating to the observation that in a large state hospital, there is an age-related increase in the ratio of patients with a DSM-II diagnosis of paranoid schizophrenia to those with a diagnosis of nonparanoid schizophrenia. Elderly inpatients (N = 1,518) treated over a 5-year period were studied and the hypotheses of proportionately greater numbers with late onset of paranoid conditions, longer lengths of stay for recently admitted paranoid patients, and changing diagnosis over time were tested and rejected. The data suggest that the buildup among the elder

[Limits of deinstitutionalization?--perspective of the specialty clinic].

Deinstitutionalization is a complex process that in the past was often misunderstood solely as a run down or even closure of psychiatric hospitals. Although chronically mentally ill patients were prevented from long term hospitalisation some fundamental mistakes were repeated often: patients were simply discharged without any preparations and outpatient care was badly organised. In some cases this led to therapeutic neglect, social disintegration, homelessness, incarceration or other forms of dramatic loss of quality of life. These findings are internationally confirmed although there are rema

Expand the community care system: deinstitutionalization can work.

The author argues that most of the mentally ill should be treated in the community, where they can develop the skills necessary to function in society. The state hospitals should be used only to treat the most unmanageable patients. The author blames the presently inadequate community care system on insufficient funding and on the tendency of states to divide newly available funds between community services and state hospitals so that neither system is adequate. But whereas community care's problems can be resolved through greater commitment of funds and other measures, the author contends, th

The Pendulum Swings from Deinstitutionalization to Expanded Civil Commitment.

Over time, U.S. civil commitment laws have evolved from broad, paternalistic statutes to more narrowly defined legal standards that emphasize individual liberty, least-restrictive treatment, and due process. This shift contributed to deinstitutionalization as state hospital beds decreased and the number of those meeting the new involuntary commitment standards diminished. Subsequent analyses found that, as states narrowed their criteria for involuntary commitment and as community mental health systems strained to absorb the gap, homelessness and criminal justice involvement among people with m

Deinstitutionalization and attitudes toward mental illness in Jamaica: a qualitative study.

OBJECTIVE: To consider whether or not deinstitutionalization and the integration of community mental health care with primary health care services have reduced stigma toward mental illness in Jamaica. METHODS: A qualitative study of 20 focus groups, with a total of 159 participants grouped by shared sociodemographic traits. Results were analyzed using ATLAS.ti software. RESULTS: Participant narratives showed that stigma had transitioned from negative to positive, from avoidance and fear of violent behavior during the period of deinstitutionalization to feelings of compassion and kindness as

Deinstitutionalization and mortality: findings of a controlled research design in New Jersey.

In previous studies of the mortality of deinstitutionalized persons with disabilities in California and Pennsylvania, investigators failed to employ a prospective controlled research design. We identified a sample of 150 "movers" scheduled for placement from an institution and a matched sample of 150 institutional "stayers." The matching and other risk variables were measured in 1994. Visits to all residences occurred between 1997 and mid-2000 at specified intervals after movers left the institution. Logistic regression analyses identified the 1994 preclosure risk variables of age, low self-ca

Deinstitutionalization and schizophrenia in Finland II: discharged patients and their psychosocial functioning.

Three representative cohorts of schizophrenia patients deinstitutionalized from psychiatric hospitals in 1982, 1986, and 1990 were followed up for 3 years in Finland. Patients of the last cohort were older, more disturbed, and had been ill for a longer time than those discharged at the beginning of the 1980s. Despite this, the mortality of patients deinstitutionalized in 1990 did not increase, and their psychosocial functioning seemed to become even better during the 3-year follow-up period compared with those deinstitutionalized during the previous decade. Patients who had been discharged in

Deinstitutionalization for older adults with severe mental retardation: results from Australia.

A deinstitutionalization research project in which residents from the largest institution in Queensland, Australia, were relocated after a government decision to close the center was described. Outcomes of relocation into community living for adults with severe mental retardation, many of whom were older (over 40 years) and had been institutionalized for much of their lives, was addressed within the Australian context. Results from the relocation and outcomes for people aged 20 to 39 years (n = 37), 40 to 59 years (n = 39), and over 60 years (n = 24) were described. Group results focused on ad

Deinstitutionalization second time around - What can we learn from the psychiatric experience?

This historical opinion article draws parallels between mid-20th-century psychiatric deinstitutionalization and today's rapid expansion of home-based medical care. While psychiatric conditions differ fundamentally from acute medical illnesses, lessons from deinstitutionalization can inform current and future telemedicine models. Deinstitutionalization resulted from social, legal, and pharmaceutical advances but also produced unforeseen consequences, including increased incarceration, homelessness, medication misuse, and significant family burden. Similarly, shifting acute care to the home may

Process of Deinstitutionalization of Aging Individuals With Severe and Disabling Mental Disorders: A Review.

BACKGROUND: For more than 60 years, psychiatric services has gradually gone from an asylum model to a community model. This change has led to the emergence of a deinstitutionalization movement. This movement seems to have left behind long-term hospitalized aging individuals with severe and disabling mental disorders. The objective of this article is to conduct a review on the challenges and issues associated with the process of deinstitutionalization among hospitalized aging individuals with severe and disabling mental disorders. METHODS: Using PRISMA statement, the research methodology was c

Longitudinal study on the adaptive and challenging behaviors of deinstitutionalized adults with mental retardation.

Adaptive and challenging behavior changes associated with movement from Minnesota's state institutions to community homes were examined. Most of the 148 participants who moved had severe or profound mental retardation, significant challenging behavior, and several decades of institutionalization. Adaptive and challenging behavior were assessed in the institution and thereafter annually in the community. Relative to institutional levels, adaptive behavior declined among residents who moved to community ICFs/MR but was unchanged for the HCBS Waiver group. Challenging behavior tended to worsen in

Survey of nursing and medical profile prior to deinstitutionalization of a population with profound mental retardation.

During the planning stages of deinstitutionalization, the importance of nursing services must be recognized and given priority consideration in the community placement of persons with serious developmental disabilities. The objective of this study was to survey the medical and nursing profile of a group of nonambulatory, institutionalized individuals with profound mental retardation in anticipation of their nursing and medical needs in the community. Data were collected from the Individual Habilitation Plans of 55 individuals who had resided in a residential facility for individuals with menta

Understanding deinstitutionalization policies for older adults through sarcopenia and the place of aging in Spain.

To support deinstitutionalization policies in Spain, it's useful to understand sarcopenia determinants. This study analyzed 267 community-dwelling and institutionalized older adults, measured sarcopenia using the European Working Group of Sarcopenia in Older People 2 (EWGSOP2) algorithm and looked for the role of place of aging among a set of social, economic, psychological and environmental variables. Findings reveal that the place of aging might have a significant relationship with sarcopenia in Spanish older adults and that a large proportion of older adults with sarcopenia could end up liv

Assessing the Impact of Psychiatric Deinstitutionalization and Substance Use on Patient Outcomes: A Multi-Faceted Analysis.

Background and Objectives: The worldwide shift toward psychiatric deinstitutionalization has aimed to enhance patient autonomy, social integration, and overall quality of life. However, limited studies have examined how concurrent substance use-particularly alcohol, marijuana, and inhalable drugs-affects clinical outcomes in these populations. This study aimed to evaluate psychiatric patients with varying degrees of institutionalization and investigate whether substance use complicates or exacerbates treatment outcomes. We hypothesized that individuals using substances would demonstrate worse
