Delayed emergence from anesthesia resulting from cerebellar hemorrhage during cervical spine surgery.

Cerebellar hemorrhage is an unpredictable complication of spinal surgery. We encountered a case of cerebellar hemorrhage presenting with delayed emergence from anesthesia and hemiplegia after resection of an intradural extramedullar tumor from the cervical spine. Postoperative brain computed tomography revealed hematoma in the cerebellar vermis and right cerebellar hemisphere. The patient made a gradual recovery with conservative treatment. Although the mechanism of cerebellar hemorrhage remains speculative, loss of cerebrospinal fluid may play an important role. Cerebellar hemorrhage must the

Delayed emergence from anesthesia associated with absent brainstem reflexes following suboccipital craniotomy.

One of the most feared complications after intracranial surgery is development of acute intracranial pathology, which may result in hypoperfusion and brain injury. Thus, early neurological assessment, performed in the operating room immediately after emergence from anesthesia, is a practice that may contribute to timely diagnosis of neurosurgical complications. Failure to awake after general anesthesia precludes conductance of neurological assessment. We report a patient who failed to emerge from anesthesia after suboccipital craniotomy and had absent brain-stem reflexes with fixed and dilated

Delayed Emergence From Anesthesia After Esophagogastroduodenoscopy/Colonoscopy in an Adult Patient With Narcolepsy.

Endoscopic procedures such as esophagogastroduodenoscopy and colonoscopy are commonly performed under propofol-based anesthesia due to its rapid onset and recovery. However, delayed emergence is a rare but clinically significant concern, especially in patients with neurological disorders such as narcolepsy. We report a 28-year-old woman (body mass index: 35.8) who exhibited delayed emergence approximately 10 hours after esophagogastroduodenoscopy and colonoscopy for evaluation of iron deficiency anemia. She received lidocaine 80 mg IV, followed by propofol 900 mg IV over 45 minutes. Postproced

Independent predictors of delay in emergence from general anesthesia.

Some patients with intellectual disabilities spend longer than others in emergence from ambulatory general anesthesia for dental treatment. Although antiepileptic drugs and anesthetics might be involved, an independent predictor for delay of the emergence remains unclear. Thus, a purpose of this study is to identify independent factors affecting the delay of emergence from general anesthesia. This was a retrospective cohort study in dental patients with intellectual disabilities. Patients in need of sedative premedication were removed from participants. The outcome was time until emergence fro

Delayed Emergence From Anesthesia Due to Posterior Reversible Encephalopathy Syndrome (PRES): A Case Report.

Posterior reversible encephalopathy syndrome (PRES) is a rare clinical and radiological syndrome that presents as rapid onset of neurological symptoms such as headache, visual loss, impaired mental status, and seizure activity associated with characteristic focal white matter vasogenic edema. When promptly recognized and managed, these changes are usually reversible. PRES is most commonly associated with hypertensive crises, renal insufficiency, and the use of immunosuppressive therapies, though it may arise in various clinical contexts. Despite its significance, reports of PRES in the field o

Delayed emergence from propofol anesthesia in a patient with Lesch-Nyhan syndrome: A case report.

RATIONALE: Lesch-Nyhan syndrome (LNS) is an X-linked recessive disorder presenting with uric acid overproduction, neurocognitive disability, and behavioral disturbances. Inhalational anesthesia has been frequently used in LNS patients undergoing surgery. Characteristic compulsive self-injurious behavior and high risk of emesis may hinder inhalational induction. Propofol may be beneficial for these patients because of its easy and rapid titration for anesthetic depth during induction, early recovery from anesthesia, and antiemetic effect as well as uricosuric effect. PATIENT CONCERNS: A 16-yea

Blood concentration of propofol in a patient with delayed emergence from propofol-nitrous oxide anesthesia.

We measured the blood concentration of propofol in a patient with delayed emergence from propofol-nitrous oxide anesthesia. A 78-year-old man underwent subtotal gastrectomy under both epidural and propofol-nitrous oxide anesthesia and did not regain consciousness soon after termination of propofol infusion. Preoperative laboratory examination revealed anemia and a low blood total protein concentration, but there was no evidence of impaired liver function. While the anesthesiologists were waiting for the patient to regain consciousness, a surgeon mentioned that the common hepatic artery might h

[Improvement of consciousness by administration of flumazenil in a patient with unexpectedly delayed emergence from general anesthesia].

A 64-year-old woman was scheduled for the removal of hepatic cystadenoarcinoma. The preoperative examination did not reveal any neurological disorders. Anesthesia was induced with midazolam (5 mg) and remifentanil (0.1.ag x kg-1 x min-1) and the trachea was intubated following administration of rocuronium. Anesthesia was maintained with propofol (1.2-4.0 mg x kg-1 x hr-1), remifentanil (0.1-0.4microg kg 1 x min-1), and rocuronium (10 mg) as needed. The dose of propofol was controlled so that bispectral index (BIS) ranged between 40 and 60 during surgery. The duration of surgery was 10 hr 29 mi

Delayed Emergence from Total Intravenous Anesthesia Following Posterior Spinal Correction and Fusion for Scoliosis: A Case Report.

BACKGROUND Postoperative acute liver failure, a complication following spine surgery, can cause delayed emergence from total intravenous anesthesia. Here, we report a case of acute severe postoperative liver failure following posterior spinal correction and fusion in a patient with congenital scoliosis. CASE REPORT A girl's congenital scoliosis worsened, and posterior spinal correction and fusion was scheduled. General anesthesia was induced with sevoflurane, fentanyl, target-controlled-infusion with propofol, and rocuronium. General anesthesia was maintained using target-controlled-infusion w

Reanimation of rodents: an animal model for cognitive recovery from anaesthesia.

Outside of the specialty, there is often a simplified perception that anaesthesiology focuses solely on inducing and maintaining an anaesthetised state. However, patients and their families never seek a one-way trip into the abyss of general anaesthesia, a point emphasised by a senior colleague who quipped, 'any fool can put someone to sleep, it's being able to wake them up again that's important'. When emergence goes awry, in the best cases, it manifests as a mere temporal delay to normal wakefulness. However, aberrant emergence can portend underlying neuropathology including emergence and po

Sugammadex: A Comprehensive Review of the Published Human Science, Including Renal Studies.

Although neuromuscular block (NMB) allows immobility for airway management and surgical exposure, termination of its effect is limited by and associated with side effects of acetylcholinesterase inhibitors. Sugammadex is a selective relaxant binding agent that has been shown to reverse deep NMB, even when administered 3 minutes following a 1.2 mg/kg dose of rocuronium. This novel drug is a modified gamma cyclodextrin, that through encapsulation process terminates the effects of rocuronium and vecuronium (aminosteroid muscle relaxants), and enables the anesthesiologists rapidly to reverse profo

An educational intervention to reduce the incidence of postoperative residual curarisation: a cluster randomised crossover trial in patients undergoing general anaesthesia.

BACKGROUND: The incidence of postoperative residual curarisation remains unacceptably high. We assessed whether an educational intervention on perioperative neuromuscular block management can reduce it. METHODS: In this multicentre, cluster randomised crossover trial, centres were allocated to receive an educational intervention either in a first or a second period. The educational intervention consisted of a lecture about neuromuscular management key points, including quantitative neuromuscular monitoring and use of reversal agents. The lecture was streamed to allow repetition. Additionally,

Residual neuromuscular blockade in the ICU: a prospective observational study and national survey.

Residual neuromuscular blockade is associated with significant morbidity. It has been widely studied in anaesthesia; however, the incidence of residual neuromuscular blockade in patients managed in the ICU is unknown. We conducted a prospective observational study in a tertiary ICU to determine the incidence of residual neuromuscular blockade using quantitative accelerographic monitoring. We tested for residual neuromuscular blockade (defined as a train-of-four ratio < 0.9) before cessation of sedation in anticipation of tracheal extubation. We also surveyed 16 other ICUs in New Zealand to det

Required dose of sugammadex or neostigmine for reversal of vecuronium-induced shallow residual neuromuscular block at a train-of-four ratio of 0.3.

Residual shallow neuromuscular block (NMB) is potentially harmful and contributes to critical respiratory events. Evidence for the optimal dose of sugammadex required to reverse vecuronium-induced shallow NMB is scarce. The aims of the present study were to find suitable doses of sugammadex and neostigmine to reverse a residual vecuronium-induced NMB from a time of flight (TOF) ratio of 0.3-0.9 and evaluate their safety and efficacy. In total, 121 patients aged 18-65 years were randomly assigned to 11 groups to receive placebo, sugammadex (doses of 0.125, 0.25, 0.5, 1.0, or 2.0 mg/kg), or neos

Epidemiology and outcomes of residual neuromuscular blockade: A systematic review of observational studies.

OBJECTIVES: Complete reversal of neuromuscular blockade (NMB) is important for patient safety and prognosis following surgical procedures involving NMB agents (NMBAs). Published evidence on the epidemiology and consequences of residual neuromuscular blockade (rNMB; incomplete neuromuscular recovery) in real-world clinical settings is lacking with advances in NMB management. Therefore, we aimed to examine the burden of rNMB and its associated clinical, economic and humanistic outcomes using a systematic review framework. REVIEW METHODS: Electronic and conference database searches were performe
