Dementia: Dementia Types.

Dementia, also called major neurocognitive disorder, is characterized by a chronic progressive loss of cognitive function in the absence of fluctuating consciousness. It represents a primarily geriatric syndrome that may be caused by one of several underlying conditions. There is insufficient evidence to support universal screening for cognitive impairment in older adults; however, clinicians should be alert to patient and caregiver concerns about cognitive changes and investigate such concerns with validated cognitive assessment tools. Alzheimer disease is the leading cause and prototypical f

Depressive dementia: a "transitional dementia"?

This review comprises a historical, clinical, and empirical examination of the dementia spectrum of depression. The primary focus of the article is to evaluate the usual dichotomy between depressive dementia as functional-reversible and degenerative dementia as organic-irreversible. It is proposed that depression, cognitive impairment, and degenerative dementia be viewed as intersecting continua. Five prototypical groups are defined along these continua: (1) major depression without depressive dementia, (2) depressive dementia, (3) degenerative dementia without depression, (4) depression of de

[Reversible dementias and treatable dementias].

INTRODUCTION: When studying patients with dementia the search of reversible or treatable entities has a primordial role. Many patients do not recover the previous cognitive level; however, that does not signify that the disease is not treatable with partial recovery or prevention of secondary progression. DEVELOPMENT: The prevalence of reversible dementias is around 15% in most series. Rutine studies include complete blood cell count, erythrocyte sedimentation rate, electrolytes and chemistry panel, liver function tests, vitamin B12 and folate serum levels, urianalysis and syphilis serology.

Dementia.

Transcranial magnetic stimulation (TMS) has been used extensively to characterize motor system pathophysiology in Alzheimer's disease (AD) and other forms of dementia, as well to monitor the effects of certain pharmacological agents. Among the studies focusing on motor cortical excitability measures, the most consistent finding is a significant reduction of short-latency afferent inhibition (SAI) in AD and other forms of dementia in which the cholinergic system is affected, such as dementia with Lewy bodies. SAI evaluation may thus provide a reliable biomarker of cortical cholinergic dysfuncti

Dementia.

Alzheimer's disease is the most common cause of dementia, but often several other conditions causing dementia are present on brain autopsies. Palliative care medical issues are similar in all late stage progressive degenerative dementias and include; cardiopulmonary resuscitation, transfer to acute care setting, antibiotic therapy, and tube feeding. Behavioral symptoms of dementia include agitation and resistiveness to care. Quality of life of individuals suffering from dementia is enhanced by availability of meaningful activities and by avoidance of restraints. Family support and involvement

Lewy Body Dementias: Dementia With Lewy Bodies and Parkinson Disease Dementia.

PURPOSE OF REVIEW: This article provides an overview of the clinical features, neuropathologic findings, diagnostic criteria, and management of dementia with Lewy bodies (DLB) and Parkinson disease dementia (PDD), together known as the Lewy body dementias. RECENT FINDINGS: DLB and PDD are common, clinically similar syndromes that share characteristic neuropathologic changes, including deposition of α-synuclein in Lewy bodies and neurites and loss of tegmental dopamine cell populations and basal forebrain cholinergic populations, often with a variable degree of coexisting Alzheimer pathology.

Spousal Dementia Exposure and Risk of Dementia.

IMPORTANCE: Previous studies suggest that spouses of individuals with dementia may be at increased risk of developing dementia, but evidence regarding subgroup variation and absolute risk remains limited. OBJECTIVE: To examine whether spouses of individuals with dementia have an increased risk of developing dementia and to assess whether this association is modified by demographic, socioeconomic, and family context factors. DESIGN, SETTING, AND PARTICIPANTS: This nationwide population-based cohort study used Taiwan's National Health Insurance Research Database from 1998 to 2022, with follow-

[Lewy body dementia and Parkinson disease dementia].

Lewy body dementia and Parkinson disease dementia are frequent causes of degenerative dementia: 20% of the dementias in patients older than 65 years are caused by the former and nearly 80% of patients with advanced Parkinson disease develop the latter. Symptoms of Lewy body dementia include fluctuations of cognitive performance, frontal and visuospatial impairment, visual hallucinations, and parkinsonism. Parkinson disease dementia could be differentiated in two subtypes: a "subcortical" subtype, characterized by frontal impairment with apathy and dullness and a "cortical" subtype with symptom

[Parkinson's disease dementia and Lewy body dementia].

To specify the existing relationship between Parkinson's disease dementia (PDD) and Lewy body dementia (LBD), it is necessary to retrace the natural history of an histopathological lesion, which, although being non-specific, is essential for a precise diagnosis, the Lewy body. The occurrence of Lewy bodies in Parkinson's disease, as in the other two types of dementia, unveils a potential continuum between these affections, which could be reunited under the term of alpha-synucleinopathy. However, defining the modalities of various types of alpha-synucleinopathy has not been historically based o

Multi-infarct dementia, subcortical dementia, and hydrocephalus.

The diagnostic evaluation of dementia is directed toward the identification of treatable causes. It can be facilitated by classification of the dementia into one of four categories: attentional, amnestic, cognitive, and intentional. Intentional dementia reflects dysfunction of frontal lobe systems, components of which include the frontal cortex, basal ganglia, thalamus, limbic structures, and subcortical white matter. Disorders that affect one or more of these components and produce intentional dementia include Binswanger's disease, Parkinson's disease, Huntington's disease, HIV infection, clo

Frontotemporal dementia: a different kind of dementia.

Frontotemporal dementia (FTD) is an umbrella term for a number of uncommon illnesses, including Pick's disease, which affect the frontal and/or temporal lobes of the brain and produce a dementia syndrome that is quite characteristic but unfamiliar to most clinicians. It is the third most common cause of cortical dementia. An overview of clinical manifestations, diagnosis, and management of FTD is provided with some specific information in relation to Pick's disease and a case example. Caregivers, in particular, need the education and support that could be provided by informed nurses.

The dementias.

Dementia affects about 5% of the elderly population over age 65 years and has an unexplained predominance in women and a low rate in some cultures. Different forms of dementia are now distinguished-Alzheimer's disease, dementia with Lewy bodies, frontotemporal dementia, and dementia secondary to disease, such as AIDS dementia. However, such nosological boundaries are being re-evaluated because different dementias are believed to have common underlying neuropathology. Neurochemical and neurobiological research has led to advances in understanding causes of dementia, and functional imaging has a

Dementias.

Senile dementia of the Alzheimer type is becoming one of the most common of the malignant diseases as our society ages. Currently, research has identified several pathophysiological changes, including the bihelical filament and the loss of the enzyme choline acetyltransferase from the cortex. Although genetic factors play some role in this disease, the important environmental risk factors have not yet been identified and there is, at present, no specific treatment. The second most common cause of dementia, cerebrovascular disease, produces dementia only when there is destruction of brain tissu

[The dementias].

Statistics indicate that in Europe people over 65 years of age will be 23.5% at the beginning of the next century and prevalence of severe dementias is estimated to be 5% in this population group. Clinically, dementia is a syndrome characterized by memory loss, other cognitive dysfunctions and loss of ability for self-care. This organic syndrome, that can have many different causes, must be differentiated from other similar psychiatric disorders of the elderly such as the pseudo-dementias. The most important of these causes are Alzheimer's disease (AD) and vascular dementias. Other causes are

Automated Imaging Differentiation for Dementia: Including Alzheimer Disease Dementia and Dementia with Lewy Bodies.

BACKGROUND AND OBJECTIVES: Differentiation of Alzheimer's disease dementia (ADD) and dementia with Lewy bodies (DLB) remains a challenge. Free-water imaging has been investigated in neurodegenerative diseases and was found to be associated with neurodegeneration and neuroinflammation. This retrospective cohort study tested whether Automated Imaging Differentiation for Dementia (AIDD), combining diffusion free-water imaging (FWI) and support vector machine, predicts ADD vs DLB with high accuracy. METHODS: Diffusion MRI data was rendered from ADNI, NACC, and PDBP. Free-water and free-water corr

[Senses and dementia : sensory loss and dementia risk].

In the course of several neurodegenerative conditions, the presence of a sensory alteration, even before the appearance of cognitive disorders or complaints concerning autonomy, can potentially lead to earlier diagnosis and more efficient management of these complex patients. The mechanisms underlying the association between sensory impairment and the risk of progression to dementia are increasingly better understood and should ultimately improve the accuracy of the diagnosis process for the most common neurodegenerative conditions, but also participate in the differential diagnosis between th
