A multidisciplinary dental program for community-living adults with chronic mental illness.

Only recently have reports described specialized large-scale dental programs for community-living adults with chronic mental illness. In response to requests from community mental health center case managers, a multidisciplinary team from Central Sydney Area Health Service dentistry, mental health, and transport services joined together to develop a pilot dental program. The aim was to profile the dental needs of community-living clients with chronic mental illness and to improve clients' utilization of inner Sydney public dental services. A review of clients' dental problems and access is pre

Oral Health Needs and Experiences of Medicaid Enrollees With Serious Mental Illness.

INTRODUCTION: Chronic dental diseases are among the most prevalent chronic conditions in the U.S., despite being largely preventable. Individuals with mental illness experience multiple risk factors for poor oral health and need targeted intervention. This study investigated experiences of Kansas Medicaid enrollees with serious mental illness in accessing dental services, examined their oral health risk factors, and identified oral health needs and outcomes. METHODS: Survey data were collected from October 2016 through February 2017 from 186 individuals in Kansas with serious mental illness e

Hyperventilation in the dental chair.

An anxious patient may experience an uncontrollable increase in ventilatory effort, far in excess of the body's metabolic needs, to such a degree that profound changes are induced in the cardiovascular and neurological systems. This abnormal drive to breathe rapidly leads to a reduction in the tension of carbon dioxide in alveolar gas and arterial blood and to mild alkalosis. The outward expression of these disturbances can be alarming for both patient and dentist, but for the physiologist searching for underlying mechanisms it is intriguing. The cursory coverage of this subject in some textbo

Palliative dental care: Ignored dimension of dentistry amidst COVID-19 pandemic.

Palliative care (PC) represents an approach that encompasses the procedures to relieve pain and distressing symptoms and maintain function as much as possible in the terminally ill patient until death. PC dentistry (PCD) is an approach for providing supportive and palliative dental care in patients with serious life-threatening illness (cancer, chronic heart failure, chronic obstructive pulmonary disease and cognitive impairment). The care is provided at different time points of diseases (i.e., from in-therapy care to survivorship care to end-of-life care). Dentists have a significant role to

Dental Caries in Older Adults: A Narrative Review for Non-Dental Providers.

Dental caries is one of the most prevalent chronic diseases affecting older adults and remains an important, underrecognized contributor to pain, impaired nutrition, systemic illness, and reduced quality of life. Older adults access medical care more often than dental care, positioning non-dental clinicians to identify risk, initiate prevention, and facilitate timely referral. A narrative review of English-language literature was conducted using PubMed and Google Scholar. Search terms included "dental caries," "root caries," and "older adults." Sources included original studies, clinical guide

Perioperative management of patients with myasthenia gravis: prevention, recognition, and treatment.

Myasthenia gravis (MG) is an autoimmune disease characterized by weakness and fatigability of skeletal muscles, with improvement following rest. The disease was so named because of a frequently fatal outcome. As recently as 30 years ago, 25% of patients with MG died of the disease. Treatment advances have dramatically changed the expected outcome, and with appropriate therapy, most patients with MG can now lead normal lives and have a relatively normal life expectancy. In this article we review the pathophysiology of MG, its signs and symptoms, treatment, the perioperative evaluation and prepa

Understanding the autistic dental patient.

Autism spectrum disorder (ASD) is one of many pervasive developmental disorders (PDD); others include Rett syndrome, childhood disintegrative disorder (also known as Heller's syndrome), pervasive developmental disorder not otherwise specified (PDD-NOS), and the higher functioning Asperger's syndrome. Because ASD is the most common of the developmental disabilities, it is not unusual for dentists to have ASD patients among their patient population. As the name indicates, ASD varies widely in its clinical manifestations; however, dentists are likely to encounter difficulties with communication a

Medications Affecting Treatment Outcomes in Dentistry: Part 2.

Today, it is common for medically complex patients who are receiving multiple medications, to seek routine and emergent dental care. It is essential for the practitioner to recognize and comprehend the impact of such medications on the patient's ability to tolerate the planned dental treatment and on dental treatment outcomes. An active appraisal of current literature is essential to stay abreast of emerging findings and understand their treatment implications. This article outlines the process of such active critical appraisal, illustrating key paradigms of the models that describe the impact
