Ribbon-like hypomineralization in human dental enamel.

Human dental enamel is a uniquely hard and tough biological realization of the inherently brittle apatite crystal. In sound enamel both structure and chemistry are organically fine-tuned, while developmental disorders - such as hypomineralization - often lead to severe enamel alterations resulting in weaker mechanical performance and shorter lifetime as in case of molar-incisor hypomineralization (MIH) which has a global prevalence of 13.5 %. However, causes of the development of hypomineralization disorder remain unclear up to date. In the present paper, a new form of dental hypomineralizatio

Hypomineralized Enamel Alters Trigeminal Sensory Afferent Architecture, Transcriptome, and Dental Injury Responses.

At barrier tissues such as the skin and gut, sensory neurons interface with environmental stimuli and coordinate with neighboring epithelial and immune cells to detect tissue perturbations. In teeth, however, the sensory dentin-pulp complex is insulated from the oral environment by highly mineralized enamel. Although dentin-pulp responses to severe injury have been studied in models with direct pulp exposure, it remains unclear whether enamel barrier dysfunction alone alters pulpal and neuronal homeostasis. A quantitative backscattered electron imaging study of hypomineralization and hypoplasia in fluorosed dental enamel of deer.

Mineral content and distribution of fluorosed and unfluorosed (control) dental enamel of roe deer and red deer cheek teeth were analyzed using digital backscattered electron (BSE) imaging of PMMA-embedded specimens. Compared to the controls, the fluorosed enamel exhibited various aberrations resulting from a fluoride-induced disturbance of the processes involved in enamel formation. Thus, the presence of surface hypoplasias and an enhancement of the incremental pattern in the fluorosed enamel are evidence of a fluoride impact on the secretory ameloblasts, whereas a (subsurface) hypomineralizat

Does molar-incisor hypomineralization (MIH) affect only permanent first molars and incisors? New observations on permanent second molars.

BACKGROUND: The second permanent molar plays an important role when considering extraction of first permanent molars severely affected by molar-incisor hypomineralization (MIH). AIM: To assess the association between MIH and enamel hypomineralization of the second permanent molars in terms of presence and severity. DESIGN: In this retrospective cross-sectional study, permanent teeth were evaluated in 453 patients between the ages of 13 and 16 at a dental clinic, using intraoral photographs. A calibrated examiner classified enamel hypomineralization using the MIH index and dental caries exper

Developmental and Post-Eruptive Defects in Molar Enamel of Free-Ranging Eastern Grey Kangaroos (Macropus giganteus) Exposed to High Environmental Levels of Fluoride.

Dental fluorosis has recently been diagnosed in wild marsupials inhabiting a high-fluoride area in Victoria, Australia. Information on the histopathology of fluorotic marsupial enamel has thus far not been available. This study analyzed the developmental and post-eruptive defects in fluorotic molar enamel of eastern grey kangaroos (Macropus giganteus) from the same high-fluoride area using light microscopy and backscattered electron imaging in the scanning electron microscope. The fluorotic enamel exhibited a brownish to blackish discolouration due to post-eruptive infiltration of stains from

Optical coherence tomography and gray scale digital analysis as noninvasive techniques for evaluating molar-incisor hypomineralization severity: A comparative study with microcomputed tomography.

Molar-incisor hypomineralization (MIH) is a qualitative defect of dental enamel characterized by demarcated opacities present in permanent first molars and other teeth. It is considered a major clinical challenge in dentistry because it makes affected teeth more susceptible to fractures and dental caries. Its diagnosis is mainly clinical and there are few technological resources that allow for a more accurate diagnosis, especially with respect to the depth of the defect in the dental enamel. In this context, optical coherence tomography (OCT), which is routinely used in ophthalmology, can prod

Chronic fluoride toxicity: dental fluorosis.

Dental fluorosis occurs as a result of excess fluoride ingestion during tooth formation. Enamel fluorosis and primary dentin fluorosis can only occur when teeth are forming, and therefore fluoride exposure (as it relates to dental fluorosis) occurs during childhood. In the permanent dentition, this would begin with the lower incisors, which complete mineralization at approximately 2-3 years of age, and end after mineralization of the third molars. The white opaque appearance of fluorosed enamel is caused by a hypomineralized enamel subsurface. With more severe dental fluorosis, pitting and a l

Creation and characterization of novel knock-in mouse models of hypophosphatasia.

Hypophosphatasia (HPP) is a rare inherited disorder caused by inactivating variants in the ALPL gene, resulting in defective mineralization of bone and teeth. Dental manifestations include early exfoliation of fully rooted primary teeth as a result of cementum dysplasia, premature loss of permanent teeth, and hypomineralization of enamel, dentin, and jawbone. However, genotype-dependent dental phenotypes and severity-based treatment strategies remain poorly defined. The aim of this study was to generate knock-in mouse models harboring clinically relevant ALPL variants and to characterize their

Dental tissue effects of fluoride.

It is now well-established that a linear relationship exists between fluoride dose and enamel fluorosis in human populations. With increasing severity, the subsurface enamel all along the tooth becomes increasingly porous (hypomineralized), and the lesion extends toward the inner enamel. In dentin, hypomineralization results in an enhancement of the incremental lines. After eruption, the more severe forms are subject to extensive mechanical breakdown of the surface. The continuum of fluoride-induced changes can best be classified by the TF index, which reflects, on an ordinal scale, the histop

G protein-coupled receptor Gpr115 (Adgrf4) is required for enamel mineralization mediated by ameloblasts.

Dental enamel, the hardest tissue in the human body, is derived from dental epithelial cell ameloblast-secreted enamel matrices. Enamel mineralization occurs in a strictly synchronized manner along with ameloblast maturation in association with ion transport and pH balance, and any disruption of these processes results in enamel hypomineralization. G protein-coupled receptors (GPCRs) function as transducers of external signals by activating associated G proteins and regulate cellular physiology. Tissue-specific GPCRs play important roles in organ development, although their activities in tooth

Posteruptive changes in human dental fluorosis--a histological and ultrastructural study.

The aim of the present study was to describe the structural features characterizing the severe grades of human fluorotic enamel (TF scores 5-9) with particular emphasis on the posteruptive changes in severely fluorosed teeth. Dental fluorosis is a subsurface hypomineralized lesion deep to a well-mineralized outer enamel surface, which in severe cases breaks apart shortly after eruption. Early signs of posteruptive changes comprise small defects corresponding to the opening of striae of Retzius. The enamel pits which develop after eruption in more severe cases exhibit an increase in mineral con

Mechanism and timing of fluoride effects on developing enamel.

Fluoride appears to specifically interact with mineralizing tissues, causing an alteration of the mineralization process. In enamel, fluorosis results in a subsurface hypomineralization. This hypomineralized enamel appears to be directly related to a delay in the removal of amelogenins at the early-maturation stage of enamel formation. The specific cause for this delay is not known, although existing evidence points to reduced proteolytic activity of proteinases that hydrolyze amelogenin. This delay in hydrolysis of amelogenins could be due to a direct effect of fluoride on proteinase secretio

Prevalence of Enamel Hypomineralisation/Molar Incisor Hypomineralisation: A Scoping Review.

The aim of this scoping review was to provide an up-to-date assessment of the prevalence of enamel hypomineralisation (EH) and, in particular, molar incisor hypomineralisation (MIH). Epidemiological studies published from 2010 to 2023 were analyzed. After screening the primary literature, a total of 139 studies were included. According to the available epidemiological trials of children and adolescents from Europe (n = 39), the average prevalence of EH/MIH was 16.74%. The data from Asian studies (n = 50) showed an average prevalence of 15.6%. For South America (n = 33 studies), the mean preval

Dental fluorosis in the European roe deer (Capreolus capreolus): A review of the pathological changes in the enamel of fluorotic cheek teeth and the abnormal pattern of dental wear in affected dentitions.

This article reviews the pathological changes in the enamel of permanent mandibular cheek teeth and their sequelae in European roe deer from regions polluted by anthropogenic fluoride emissions. The primary (developmental) changes of fluorotic roe deer enamel are hypomineralization and microstructural aberrations, including enamel hypoplasia. The hypomineralized fluorotic enamel is whitish-opaque on eruption and gets stained following tooth eruption. Moreover, it is prone to excessive wear and mechanical breakdown, resulting in posteruptive enamel lesions. These posteruptive lesions can morpho

Evaluation of developmental defects of enamel and differential diagnosis from dental caries using quantitative light-induced fluorescence technology: A cross-sectional investigation.

OBJECTIVES: Dental caries and developmental defects of enamel (DDE) are common conditions that impact oral health. Quantitative light-induced fluorescence (QLF) is widely used for detecting dental caries, but its ability to identify DDE remains unclear. This study aimed to investigate fluorescence differences among sound, carious, and DDE-affected teeth using QLF, and to differentiate white hypomineralization from incipient caries. METHODS: Data from 704 teeth in 137 patients under 20 years old were analyzed. QLF images were acquired, and fluorescence parameters were measured. Statistical tes
