Evaluating an impedance-based smart drill system in a preliminary human cadaver model.

Dental implant placement requires a great level of accuracy given the risk of iatrogenic injury to the inferior alveolar nerve and maxillary sinus. Real-time impedance sensing of the surrounding tissues by the drill tip can provide feedback to the surgeon for improved outcomes and reduced risk of injury. Previous studies have shown impedance sensing to successfully discriminate between cortical and cancellous bone in ex vivo and in situ large animal tissue. Understanding electrical properties of tissues in a cadaver model is essential to developing human-centered sensor and tissue classificati

[Noise risk due to dental turbines].

Sound level measurements and frequency analyses were carried out on dental turbines in order to assess the risk of hearing defects due to the noise caused by such turbines. The measured values were related to other factors (exposure time, noise pauses) and the overall situation was evaluated. A major leasion of the auditory organ due to the noise caused by dental turbines seems unlikely.

Improving the visibility of tooth cementum annulations by adjustment of the cutting angle of microscopic sections.

Age at death assessments by counting tooth cementum annulations (TCAs) in unstained undecalcified microscopic ground sections of (single rooted) teeth is, amongst others, problematic because of the unclear distinction between the bright and darker annulations. Counting is hampered by optical superimposition of the tangentially positioned layers of cementum in the section since 'regular transverse sections' run perpendicular to the axis of a cone-shaped root with its yearly deposited cone-shaped layers of cementum. This study demonstrates that to improve the visibility of the annulations, the c

Remodeling along the bone-screw interface.

OBJECTIVE: The stability of cortical miniscrews depends on the contact between bone and implant. The aim of this study was to compare bone remodeling along the contact surface between the screw and bone with and without pre-drilling. MATERIALS AND METHODS: Twenty-four FAMI miniscrews (length 10 mm, diameter 2 mm) were inserted into the mandibular alveolar process apical to the premolars of four miniature pigs. Each pig received six screws, three on the one side with pre-drilling, three on the other side without pre-drilling. Polychrome fluorescent labels were used to assess the bone remodelin

Comparison of the effects of periodontal rotary instruments and Gracey curettes on root surface characteristics: an in vivo SEM study.

OBJECTIVE: Preparing the root surface by planing is an important procedure in the treatment and prevention of periodontal disease. The present study was designed to compare root surface characteristics following root planing with hand and power-driven instruments. METHOD AND MATERIALS: In the present study, 40 single-rooted teeth indicated for extraction were selected and divided into two groups according to whether hand or rotary instruments were used (group I or II, respectively). Teeth were randomly selected. Scaling was performed with ultrasonic instruments. A mucoperiosteal flap was rais

Ultrasonic versus high-speed cavity preparation: analysis of increases in pulpal temperature and time to complete preparation.

STATEMENT OF PROBLEM: The use of ultrasonic tips has become an alternative for cavity preparation. However, there are concerns about this type of device, particularly with respect to intrapulpal temperatures and cavity preparation time. PURPOSE: The purpose of this study was to analyze pulpal temperature increases generated by an ultrasonic cavity preparation with chemical vapor deposition (CVD) tips, in comparison to preparation with a high-speed handpiece with a diamond rotary cutting instrument. The time required to complete the cavity preparation with each system was also evaluated. MATE

Tooth preparation and pulp degeneration.

Conscious attention must be directed toward providing atraumatic tooth preparations for crowns, and to constructing adequate provisional restorations. If meticulous care is not taken, pulpal damage or death can occur. Patients should be warned that pulpal death and endodontic therapy can result from crown placement.

Automated sterile saline dispenser in lieu of an assistant for surgical bone cutting to remove impacted teeth.

Surgery for removal of wisdom teeth requires bone cutting under a constant stream of saline to cool the site of surgery. Any interruption in irrigation may predispose to thermal necrosis of bone which delays healing. A gravity-assisted saline irrigation system is easily fabricated from a sterile intravenous drip-set. Economics, availability, ease of use and sterility are its advantages as compared to expensive electromechanical saline dispenser pumps. The system also permits the surgeon's assistant to concentrate on other tasks during the operation.

Iatrogenic subcutaneous facial emphysema secondary to a Class V dental restoration: a case report.

Subcutaneous facial emphysema (SFE) following routine dental operative procedure is an uncommon but potentially life-threatening complication. The present case details a Class V restoration where air was introduced into the fascial tissue planes via the gingival sulcus from the use of an air-driven dental handpiece. Although the SFE is usually self-limiting within 3-10 days, such instances should be regarded as a medical emergency as in severe cases, the air may spread to the neck, mediastinum and thorax to result in cervicofacial emphysema with potential pneumomediastinum and pneumothorax.

The use of CVD diamond burs for ultraconservative cavity preparations: a report of two cases.

UNLABELLED: During the past decades, scientific developments in cutting instruments have changed the conventional techniques used to remove caries lesions. Ultrasound emerged as an alternative for caries removal since the 1950s. However, the conventional technology for diamond powder aggregation with nickel metallic binders could not withstand ultrasonic power. Around 5 years ago, an alternative approach using chemical vapor deposition (CVD) resulted in synthetic diamond technology. CVD diamond burs are obtained with high adherence of the diamond as a unique stone on the metallic surface with

Using chemomechanically assisted diamond bur cutting for improved efficiency.

BACKGROUND: Surface active agents added to handpiece coolants enhance bur cutting rates, or CRs, through chemomechanical effects, or CMEs. The authors evaluate the effect of CMEs on long-term cutting by diamond burs. METHODS: The authors tested medium-grit diamonds in a high-speed handpiece under a 147.5-gram load at 350,000 revolutions per minute, with 22 milliliters per minute coolant flow consisting of distilled water or a 1:10 mouthwash/water mixture. Repeated 6-millimeter-long edge cuts were made through machinable glass ceramic bars until the CR (determined as the time to transect the b

Otologic complications following the use of a high-speed air-turbine handpiece.

The use of high-speed instruments may result in subcutaneous air in the face and neck. Hearing loss and otalgia have been added to the previously reported sequelae. Careful use of air-driven high-speed handpieces around areas of weakened and disrupted oral tissues may help avoid subcutaneous emphysema. Awareness of this entity plays a significant role in appropriate diagnosis and treatment.

Sudden Onset of Subcutaneous Air Emphysema After the Application of Air to a Maxillary Premolar Located in a Nonsurgical Field.

Although rare, subcutaneous air emphysema can occur during dental procedures such as endodontic treatment, surgical extractions, and preparing a tooth for an indirect or direct dental restoration. We report the development of a subcutaneous air emphysema that was introduced through the periodontal ligament of an untreated premolar after the use of an air syringe to dry the tooth.

Histologic evaluation of the pulpal response to temperature probe placement in the Macaca fascicularis monkey.

A method to measure intrapulpal temperatures during cavity preparation and restorative procedures has been developed. It involves placement of temperature probes to the dentinoenamel and dentinopulpal junctions. After placement of the probes, the pulps of the test teeth were examined histologically to determine whether their placement caused a pulpal response. The test teeth displayed no adverse response in the areas of the pulp opposite or adjacent to the position of the probes. Therefore the developed method of temperature probe placement to measure intrapulpal temperatures does not interfer

Surgical Emphysema: A Rare Complication of a Simple Surgical Dental Extraction Without the Use of an Air-Driven Rotor.

Surgical emphysema is a rare complication of dental extractions, often associated with the use of high-speed air rotors. This report describes a case of extensive surgical emphysema following a simple surgical extraction of a LL6 under local anaesthetic. There was no use of air-driven handpieces during the procedure. The patient developed extensive surgical emphysema bi-laterally in both cervical neck and facial planes. After prophylactic antibiotics with careful monitoring in a secondary care setting, the patient made a full unremarkable recovery. Clinical relevance: Simple extraction of teet

Fiber optic probe augmented sonic scaling versus conventional sonic scaling.

Several factors, including access and visualization problems, make total deposit removal during scaling and root planing procedures extremely difficult. This study examined the effectiveness of a mode of therapy designed to improve access and visualization for sonic scaling compared to closed sonic instrumentation. Teeth with moderate to deep probing depths in six patients scheduled to receive immediate dentures were divided into three experimental groups: Group I, sonic scaling with access augmented by interdental papilla reflection and fiber optic illumination/transillumination (34 surfaces)

Preparation time and sealing effect of cavities prepared by an ultrasonic device and a high-speed diamond rotary cutting system.

The purpose of this study was to compare a high-speed handpiece to an ultrasonic device regarding the time taken for cavity preparation in bovine incisor teeth (n = 12), and to evaluate microleakage after the restorations. Two cylindrical cavities each were prepared on the labial surfaces of 12 teeth. One of them was made with a diamond tip in group 1 (G(1) = 12) coupled with a high-speed handpiece, and the other with a chemical vapor deposition (CVD) tip in group 2 (G(2) = 12) coupled with an ultra-sound device (n = 24). The time taken for each preparation was measured with a stopwatch. The c
