Postpartum depression.

Though postpartum depression has been observed for years, research into it has increased recently, especially in England. Distinguishing the rarer, severe puerperal psychosis from milder, more frequent "maternity blues" has provided a clearer framework for new research. Hormonal changes, biogenic amines, stress, and lack of social support have been proposed as explanations for postpartum depression, but the etiology is still not clear. Hormones, tranquilizers, and antidepressants are some modern therapeutic measures employed, whereas electroconvulsive shock therapy was once the most frequent t

Postpartum Respiratory Depression.

Postpartum respiratory depression is a complex, multifactorial issue that encompasses a patient's baseline preexisting conditions, certain pregnancy-specific conditions or complications, as well as the iatrogenic element of various medications given in the peripartum period. In this review, we discuss many of these factors including obesity, sleep-disordered breathing, chronic lung disease, neuromuscular disorders, opioids, preeclampsia, peripartum cardiomyopathy, postpartum hemorrhage, amniotic fluid embolism, sepsis, acute respiratory distress syndrome (ARDS), and medications such as analges

Management of postpartum depression.

The mainstays of treatment for peripartum depression are psychotherapy and antidepressant medications. More research is needed to understand which treatments are safe, preferable, and effective. Postpartum depression, now termed peripartum depression by the DSM-V, is one of the most common complications in the postpartum period and has potentially significant negative consequences for mothers and their families. This article highlights common clinical challenges in the treatment of peripartum depression and reviews the evidence for currently available treatment options. Psychotherapy is the fi

Identification of postpartum depression.

Postpartum depression (PPD) is the most common medical complication of childbearing. Universal screening maximizes the likelihood of prompt identification of PPD. Obstetrician-gynecologists routinely evaluate postpartum women for a general health examination and review of family planning options at approximately 6 weeks after birth; therefore, they are well positioned to identify PPD. In this study, we review the diagnostic criteria for postpartum depressive disorders and clinical risk factors predictive of PPD. We examine depression screening tools, appropriate cut-points associated with posi

Epilepsy and postpartum depression.

PURPOSE: Postpartum depression occurs in 10-20% of women who have recently given birth, but less than half of cases are recognized. The purpose of this study was to examine whether there was a difference in the rate of postpartum depression between patients with epilepsy and healthy controls, and to determine the relationship between epilepsy and postpartum depression. METHODS: The sample consisted of 55 patients with epilepsy and 55 patients without epilepsy. All patients were recruited at the Regional Epilepsy Center and at the Department of Obstetrics and Gynecology, University of Milan, S

The neurobiology of postpartum depression.

Postpartum depression (PPD) is the most common complication of pregnancy, posing significant health risks for both women and their children. Unlike other subtypes of major depressive disorder (MDD), PPD is thought to be biologically relatively homogenous, as it is precipitated by a specific biological trigger - the profound hormonal shifts associated with pregnancy and delivery. However, the underlying pathophysiology of PPD is still unclear, with a variety of biological factors likely at play. In this review, we examine recent biological findings related to neuroimaging, the hypothalamic-pitu

Universal screening of postpartum depression with Edinburgh Postpartum Depression Scale: A prospective observational study.

OBJECTIVES: To assess the feasibility of universal screening of postpartum depression (PPD), using the Edinburgh Postpartum Depression Scale (EPDS) in the general population. To investigate the proportion of women identified as being at risk of PPD and with confirmed PPD or other mental disorders after a psychiatric consultation. METHODS: A multicenter prospective cohort study in four French maternities conducted between 2020 and 2023. All women aged over 18 years, who delivered following a singleton pregnancy after 37 weeks of gestation were eligible for inclusion. The exclusion criteria wer

Association between Postpartum Nutritional Status and Postpartum Depression Symptoms.

Taiwanese women may practice traditional confinement after childbirth, and no study has investigated the nutritional status and the effects of postpartum depression on such women. The aim of this study was to investigate the association between nutritional status and postpartum depression at 6-8 weeks postpartum. A cross-sectional study was conducted on postpartum women who returned to the obstetrics and gynecology clinic for routine examination from January 2016 to September 2017. A total of 344 women received assessments based on the Edinburgh Postnatal Depression Scale (EPDS). An EPDS score

Familial risk of postpartum depression.

OBJECTIVE: Many psychiatric diseases have a strong familial aggregation, but it is unknown whether postpartum depression (PPD) without prior psychiatric history aggregates in families. METHODS: Based on Danish national registers, we constructed a cohort with information on 848,544 singleton deliveries (1996-2017). Women with an episode of PPD were defined as having used antidepressant medication and/or had a hospital contact for depression within 6 months after delivery. Those with psychiatric history prior to the delivery were excluded. We estimated relative risk (RR) of PPD, comparing women

Postpartum Depression: A Metaphorical Analysis.

BACKGROUND: The reported incidence of postpartum depression in healthy women without a history of depression was 12%, and the prevalence was 17%. A silent health crisis in America is the undiagnosed and untreated mental health of women related to childbirth. Unfamiliar with medical terminology, persons may communicate more effectively by means of metaphors to describe what they are experiencing. AIMS: To examine the language women used to describe their experiences of postpartum depression with metaphors that can provide a valuable source of insight for psychiatric nurses and other clinicians

Home visiting for postpartum depression.

This is a protocol for a Cochrane Review (intervention). The objectives are as follows: To assess the benefits of home visiting models for postpartum depression amongst mothers of young children, where either the mothers or the children are enrolled in early childhood home visiting programs or interventions. To identify core components essential for a home visiting program to effectively address postpartum depression in mothers of young children. The hypothesized core components of such a program include the priority level given to PPD intervention, the type of home visitors, the intensity of

Postpartum Depression in Correctional Populations.

This viewpoint was developed to revisit the burden and risks associated with postpartum depression (PPD) among incarcerated women based on a thematic review of the literature. Around one third of incarcerated pregnant women have symptoms of moderate to severe depression perinatally. In particular, PPD negatively impacts the well-being of parents and their children. Mitigating the consequences of PPD through screening, promotion of protective factors, and early identification coupled with treatment may have a substantial impact on the overall well-being of the affected children and postpartum i

Risk Factors of Postpartum Depression.

Postpartum depression (PPD) is a widespread mental health problem and one of the prime causes of maternal suffering and ill health. On a global level, the prevalence of the disorder is about 10 to 15%. Symptoms generally appear within the first four to six weeks, which is the high-risk period. However, it may develop up to one year post-delivery. PPD presents with symptoms of classical depression, including mood fluctuations, bouts of crying, lack of interest in the child, and even thoughts of suicide. PPD not only has adverse effects on the mother's health but also hampers the growth and deve

Sad dads: paternal postpartum depression.

The postpartum period is associated with many adjustments to fathers that pose risks for depression. Estimates of the prevalence of paternal postpartum depression (PPD) in the first two months postpartum vary in the postpartum period from 4 to 25 percent. Paternal PPD has high comorbidity with maternal PPD and might also be associated with other postpartum psychiatric disorders. Studies so far have only used diagnostic criteria for maternal PPD to investigate paternal PPD, so there is an urgent need to study the validity of these scales for men and develop accurate diagnostic tools for paterna

Transcultural analysis of postpartum depression.

Postpartum depression (PPD) strikes about 1 in 10 Western women. Studies of Western women have demonstrated that this emotional experience can occur during pregnancy and/or after delivery, and even in women who adopt an infant. Is PPD a universal experience or a morbid condition that is culture specific to Western/industrialized countries? The purpose of this literature review was to examine other cultures to ascertain whether PPD is a universal experience. The literature describes women throughout the world experiencing degrees of sadness postnatally, which persist up to 1 year. The risk fact

Dementia presenting as postpartum depression.

BACKGROUND: Other conditions that can mimic postpartum depression are rare but must be considered. CASE: A 37-year-old woman developed mood symptoms as well as progressive hyperphagia, hypersexuality, disinhibition, and impairment of judgment after delivery of her third child. She was unresponsive to multiple treatments for depression and was evaluated for frontal lobe syndromes. CONCLUSION: Frontotemporal dementia, formerly known as Pick disease, is a primary degenerative dementia for which no cause is clearly established. Family history or genetic abnormalities are found in about 50% of ca

Screening for Postpartum Depression using Edinburg Postnatal Depression Scale.

BACKGROUND: Postpartum depression (PPD) is a non-psychotic depressive episode within one year post-delivery. If left untreated PPD can lead to chronic depression, neglect, substance abuse, or suicidal ideation. It is common and often undiagnosed complication of childbirth because routine screening is not standard during postnatal care. The Edinburgh Postnatal Depression Scale (EPDS) is a widely used screening tool recommended for early detection. This study assesses the prevalence of PPD and its association with socio-demographic, obstetric, perinatal, and psychiatric factors. METHODS: A cros

Delineating the Association between Heavy Postpartum Haemorrhage and Postpartum Depression.

OBJECTIVES: To explore the association between postpartum haemorrhage (PPH) and postpartum depression (PPD), taking into account the role of postpartum anaemia, delivery experience and psychiatric history. METHODS: A nested cohort study (n = 446), based on two population-based cohorts in Uppsala, Sweden. Exposed individuals were defined as having a bleeding of ≥1000 ml (n = 196) at delivery, and non-exposed individuals as having bleeding of <650 ml (n = 250). Logistic regression models with PPD symptoms (Edinburgh Postnatal Depression scale (EPDS) score ≥ 12) as the outcome variable and PPH,

Temporal Association Between Maternal Depression and Paternal Postpartum Depression.

INTRODUCTION: Paternal postpartum depression may affect 28% of fathers, but its temporal association with maternal postpartum depression is not well understood. This study aimed to quantify the temporal association between maternal postpartum depression and paternal postpartum depression, considering mother's depression history and other clinical factors. METHODS: Data for this retrospective cohort study were obtained from Kaiser Permanente Colorado health records between 2008 and 2019. The cohort included father-mother dyads having live-birth pregnancies. Diagnostic codes identified postpart
