Atopic dermatitis/atopic eczema.

Atopic dermatitis was described in 1933 but exists since antiquity. We review descriptions of a childhood skin disease compatible with our modern diagnosis of atopic dermatitis, in ancient medicine and in nineteenth century dermatology texts. We identify Hebra's prurigo and Besnier's diathetic prurigo as forerunners of atopic dermatitis, the latter being a synthesis of infantile eczema and prurigo. The pathogenic theories which link atopic dermatitis to humoralistic medicine, to digestive diseases, to allergy may have had consequences on today's reluctance to consider atopic dermatitis as a sk

Atopic dermatitis.

Atopic dermatitis (AD) is a common, chronic skin disorder that can significantly impact the quality of life (QoL) of affected individuals as well as their families. Although the pathogenesis of the disorder is not yet completely understood, it appears to result from the complex interplay between defects in skin barrier function, environmental and infectious agents, and immune dysregulation. There are no diagnostic tests for AD; therefore, the diagnosis is based on specific clinical criteria that take into account the patient's history and clinical manifestations. Successful management of the d

[Atopic dermatitis].

Atopic dermatitis (AD), or atopic eczema, is a pruriginous inflammatory dermatosis with a genetic predisposition evolving in recurrent flare-ups on a background of chronicity, affecting essentially infants and small children. The clinical phenotype of AD results from interactions between genes and susceptibility, a defect in the cutaneous barrier function (abnormalities in filaggrin and the proteins of the epidermic differentiation complex) and dysfunction of the innate and/or adaptive immune response. With its multifactorial origin, it is dependent on the TH2 lymphocyte system, with a tendenc

[Atopic dermatitis].

For the past decades, the prevalence of atopic dermatitis is on the rise. Three phases of the disease are recognized according to the age of the patient, either in the newborn age, in infancy or after puberty. A partial enzymatic defect in delta-6 desaturase has been reported. Such metabolic anomaly would be responsible for the alterations in both the skin barrier function and in the Th2 inflammatory reactions in part mediated through IgE. Some microorganisms colonizing the skin play an important causal role in the clinical exacerbations. The influence of food allergy on atopic dermatitis rema

Atopic dermatitis.

Atopic dermatitis (AD) is the most common inflammatory skin disease and carries the highest disability-adjusted life-years burden, ranking 15th among all non-fatal diseases globally. It is characterized by intensely itchy skin and is associated with multiple comorbidities, such as food allergy, asthma, allergic rhinitis and eosinophilic oesophagitis, which are mainly driven by type 2 immune responses. Other comorbidities include mental health disorders, disordered bone health, and cutaneous and extracutaneous infections. AD is also associated with other immune-mediated inflammatory diseases, i

Atopic dermatitis.

Atopic dermatitis is the most common chronic inflammatory skin disease globally. Key features include an eczematous eruption accompanied by intense itch, which can have an enormous negative effect on patients' quality of life, especially in those with moderate-to-severe disease. Atopic dermatitis is part of a spectrum of atopic conditions that can also include several non-cutaneous organs such as respiratory (eg, allergic rhinitis and asthma) and gastrointestinal (eg, food allergy) systems. For decades, long-term disease control and maintenance were particularly challenging given that treatmen

Atopic dermatitis.

Atopic dermatitis (AD), also known as atopic eczema, is a chronic relapsing inflammatory dermatosis characterized by pruritus, xerosis, and a close association with IgE mediated sensitization to aeroallergens and foods. More than 60% of children with AD are at risk to develop allergic rhinitis or asthma (the atopic march). The distribution of lesions varies by age. Infants tend to have lesions on the cheeks and scalp, and very young children typically have involvement over the extremities, cheeks, forehead, and neck. A rash in the diaper area of infants is rarely AD. Lesions in older children

[Atopic dermatitis].

Atopic dermatitis is a common chronic inflammatory skin disease. It is a disease mostly of childhood, but also of adult life. The classical course of childhood eczema shows improvement in 80% of the cases. The pathogenesis of the disease is now better understood but the world wide prevalence of atopic dermatitis has increased two to threefold over the past 30 years. Atopic dermatitis affects considerably quality of life as well in children as in adults. Important clues in the understanding of the pathogenesis of the disease are genetic factors, skin barrier dysfunction and immune dysregulation

Atopic dermatitis.

Atopic dermatitis (AD) is a chronic inflammatory skin disease characterized by intense pruritus and frequent relapsing courses. It occurs mostly in patients who have a personal or family history of other atopic conditions, such as asthma or allergic rhinitis. The prevalence of AD is high, particularly in children, with rapidly increasing numbers in the past few decades. The chronicity of this disease, along with its relapsing nature, presents treatment and management challenges for clinicians and frustration for patients and their families.

Atopic dermatitis.

Atopic dermatitis is a chronically relapsing inflammatory skin disease with altered immune and pharmacologic responses. Elevated serum IgE probably reflects defective immune regulation. Various other cellular immune defects rise and fall exacerbations and remissions of skin inflammation. Increased responsiveness to cholinergic and alpha adrenergic agents may relate to abnormalities of cyclic nucleotide regulation. Recent observations of abnormal cyclic adenosine monophosphate (cAMP)-phosphodiesterase activity in atopic dermatitis may provide new insights into the pathogenesis and treatment of

How atopic is atopic dermatitis?

BACKGROUND: The extent to which the phenotype of atopic dermatitis (AD) is truly atopic has been the subject of much debate. OBJECTIVE: We sought to systematically evaluate the evidence for the value of measurement of IgE antibodies in diagnosing AD and whether knowledge of IgE sensitization increases clinical diagnostic and predictive ability. METHODS: We searched Medline from its inception until September 2003. Only studies that measured atopy as either skin prick test positivity or IgE-antibody sensitization to environmental allergens were included within a descriptive analysis. Because t

Atopic keratoconjunctivitis and atopic dermatitis.

PURPOSE OF REVIEW: This review will focus on the diagnostic features of atopic keratoconjunctivitis (AKC), its relationship to atopic dermatitis, the immunopathogenesis, and therapy, and will include strategies used for the management of severe disease unresponsive to conventional therapy. RECENT FINDINGS: Recent research has demonstrated the importance of various cytokines (IL-33), proteins (thymic stromal lymphopoetin) and effector cells (conjunctival epithelial cells, eosinophils and basophils) in the pathogenesis of chronic ocular inflammation. Current evidence supports the use of tacroli

Use of textiles in atopic dermatitis: care of atopic dermatitis.

Atopic dermatitis (AD) is a chronic relapsing inflammatory skin disease which usually starts during the first years of life. In the management of AD, the correct approach requires a combination of multiple treatments to identify and eliminate trigger factors, and to improve the alteration of the skin barrier. In this article we try to explain the importance of skin care in the management of AD in relation to the use of textiles: they may be useful to improve disrupted skin but they are also a possible cause of triggering or worsening the lesions. Garments are in direct contact with the skin al

[Study on the genesis of atopic dermatitis--atopic dermatitis and IgE].

For the purpose of investigating how IgE is correlated in the genesis of Atopic dermatitis (abbreviated as AD) micronizing method of RIST (necessary serum 10 microliter) was utilized. Serum IgE obtained from 369 cases AD, age ranged from 1 month old to 52 years old were studied. In 78 cases, IgE of the objects' parents were also studied as well. Serum IgE of the healthy control, age ranging from 3 days old to 49 years old, were studied and made comparison with those of the objects. Analysis was made in accordance with the factors like age, complication, disease type and prognosis. RAST (house

[Herpetiform dermatitis and atopic dermatitis].

History data and clinical pattern were analysed in 33 children with herpetiform dermatitis (DH) and in 34 children with atopic dermatitis (DA). The differences are stressed between DH and DA as which it is frequently misdiagnosed, which is the cause of delayed treatment in DH (in the studied group 2.8 years on the average). The development of skin changes with accompanying itching in a child at preschool age should suggest the supposition of DH.

Atopic Dermatitis: Pathophysiology.

The pathophysiology of atopic dermatitis is complex and multifactorial, involving elements of barrier dysfunction, alterations in cell-mediated immune responses, IgE-mediated hypersensitivity, and environmental factors. Loss-of-function mutations in filaggrin have been implicated in severe atopic dermatitis due to a potential increase in trans-epidermal water loss, pH alterations, and dehydration. Other genetic changes have also been identified, which may alter the skin's barrier function, resulting in an atopic dermatitis phenotype. The imbalance of Th2 to Th1 cytokines observed in atopic der

Managing atopic dermatitis.

"Itchy" rashes occurring in patients (usually an infant or child) with a personal and/or family history of itching, sneezing, and/or wheezing should be considered part of the spectrum of skin problems labeled "atopic dermatitis" (AD). Multiple factors influence AD, and effective management requires that none be overlooked. The complicated nature of treatment means parents and family members require repeated reinforcement and education, particularly in avoiding "triggers of itch;" and use of medications. When all the aspects of AD are adequately addressed, more than 90% of patients with AD can

Atopic contact dermatitis.

Contact urticaria appears either as wheal and flare reactions or as dermatitis resembling other types of contact dermatitis. A nonimmunologic form of contact urticaria is seen more frequently than the allergic form of the disease in experimental conditions. However, the immunologic form of contact urticaria is clinically more important. It is often seen in atopic persons working in the food industry or in kitchens. We can prevent many cases of occupational anaphylactic hand eczema by choosing proper jobs for atopic persons. Especially handling food seems to produce hypersensitivity reactions i
