Thoracic endovascular aortic repair for ruptured descending thoracic aortic aneurysm.

BACKGROUND: We evaluated clinical outcomes of thoracic endovascular aortic repair (TEVAR) for ruptured descending thoracic aortic aneurysm (rDTAA). METHODS: Twenty-three patients with rDTAA (mean age, 76.8 ± 8.8 years) underwent TEVAR at our center between January 2008 and April 2013. RESULTS: In twenty-three patients, five patients (21.7%) were in shock before surgery. Technical success was achieved in 21 patients. After TEVAR, retrograde Type A aortic dissection occurred in one patient, Type I endoleak in one patient, and Type II endoleak in three patients. The 30-day mortality rate was 4.

Giant Descending Thoracic Aortic Aneurysm: A Case Report.

BACKGROUND: Giant descending thoracic aortic aneurysm (GDTAA) is a rare vascular disease characterized by an aortic diameter exceeding 10 cm. GDTAA carries a significant risk of rupture and mortality and requires timely diagnosis and intervention. Despite the clinical severity of the disease, the literature on GDTAA remains sparse, particularly in cases with extreme aneurysmal dilatation. CASE PRESENTATION: We present the case of a 68-year-old man with a GDTAA of 14.08 × 10.04 cm, one of the largest ever reported. The patient initially presented with recurrent syncope, chronic cough and fatig

Descending thoracic aortic aneurysm: case presentation and review.

Descending thoracic aortic aneurysms, similar to other aneurysms, are often incidentally diagnosed in patients with unrelated complaints. Management of these aneurysms is largely dependent on their size and anatomy. Most individuals with asymptomatic descending thoracic aortic aneurysms may be safely managed with cardiovascular risk factor modification until the aneurysm size reaches 6 cm. A subset of individuals, such as those whose descending thoracic aortic aneurysm measures > 6 cm or in cases of rapid growth, should be offered repair, increasingly performed via an endovascular approach. Descending Thoracic Aortic Aneurysm due to Brucella melitensis.

Brucellosis is an endemic infection mainly in the Middle East and the Mediterranean region that can involve any system. However, cardiovascular involvement commonly seen as endocarditis is a rare occurrence, but it is one of the main causes of mortality and morbidity. Brucella mycotic aneurysms are extremely rare and carry a higher morbidity and mortality. Here, we present a case of Brucella mycotic aneurysms in the descending aorta complicated by an aortoesophageal fistula in a 52-year-old diabetic man. The diagnosis was made by thoracic CT angiogram showing a saccular aneurysm arising from t

Endovascular Repair of Descending Thoracic Aortic Aneurysms.

Descending thoracic aortic aneurysms (DTAAs) are an important cause of morbidity and mortality in the elderly. Once diagnosed, they should be surveilled and then repaired at a diameter of 5.5 to 6 cm, depending on the individual patient's physiologic and anatomic risk of repair. Thoracic endovascular aortic repair (TEVAR) is the preferred approach for repair and there are multiple procedural adjuncts that can expand indications for and use of TEVAR. Spinal cord injuries are an important and highly morbid complication after TEVAR and it is imperative to mitigate this risk.

Endovascular repair of a ruptured descending thoracic aortic aneurysm.

Endovascular aneurysm repair has considerable potential advantages over the surgical approach as a treatment for thoracic aortic rupture, in part because open surgical repair of ruptured thoracic aortic aneurysms is associated with high mortality and morbidity rates. We describe the successful endovascular deployment of stent-grafts to repair a contained rupture of a descending thoracic aortic aneurysm in an 86-year-old man whose comorbidities prohibited surgery. Two months after the procedure, magnetic resonance angiography showed a patent stent-graft, a patent left subclavian artery, and com

[Adjunctive methods for surgery of descending thoracic aortic aneurysm].

During the last 6 years, we employed partial extracorporeal circulation (PEC) in 7 cases and left heart bypass (LHB) in 9 cases as adjunctive methods for surgery of descending thoracic aortic aneurysm. During the aortic clamping, systemic heparinization was performed to keep an ACT more than 400 seconds in a PEC group, while it was kept from 200 to 250 seconds in LHB group. The bypass flow was controlled to maintain the distal arterial pressure over 50 mmHg in both groups. Hemodynamics during the aortic clamping were stable in both groups and significant differences were not found between them

Staged Repair of Concomitant Aortic Regurgitation and Descending Thoracic Aortic Aneurysm.

Descending thoracic aortic (DTA) aneurysms causing left main bronchus compression can be surgically repaired under left heart bypass (LHB). Safe LHB requires a competent aortic valve. Some patients present with concomitant DTA aneurysms and severe aortic regurgitation (AR), precluding LHB as an adjunct for aortic surgery. The authors present such a case and outline the management. AR can safely be addressed first in an immediate staged surgical approach, providing adequate left ventricular function.

Emergency Endovascular Interventions for Ruptured Descending Thoracic Aortic Aneurysm.

BACKGROUND: Ruptured descending thoracic aortic aneurysm (rDAA) is a rare but devastating condition. Open aortic surgery which uses cardiopulmonary bypass is associated with a high mortality. Thoracic endovascular aortic repair (TEVAR) is a less-invasive approach for which it remains unclear whether outcomes are superior or equivalent to open aortic surgery. In this study, we report our early and midterm outcomes with TEVAR for rDAA. METHODS: This is an observational, retrospective, single-center study which included patients with rDAA and treated by TEVAR. The main objective was the 30-day i

[Endovascular repair of descending thoracic aortic aneurysm: preliminary experience].

OBJECTIVE: To report an initial experience with the endovascular repair of descending thoracic aortic aneurysm (DTAA). METHODS: Endoprostheses were placed into 41 patients with DTAA between January 2001 and July 2007 which were retrospectively analyzed. The preliminary right-left carotid and left carotid-subclavian bypass was performed in 4 cases in which the distances from the proximal aneurysm to the origin of the left common carotid artery were no longer than 15 mm. EVAR was conducted 1 week after the bypass or immediately. RESULTS: All stent grafts were deployed in proper position. There

Aortic debranching for descending thoracic aortic aneurysm repair by stent grafts.

Endovascular repair of a descending thoracic aortic aneurysm may result in covering the ostia of the left carotid or left subclavian artery for proper proximal landing zones, and the celiac artery or superior mesenteric artery ostia in the abdomen for distal landing zones. To prevent possible complications of occluding the ostia of these vessels, the authors performed an innominate to left common carotid and left subclavian artery bypass as the first procedure in one patient. In the second patient they performed an aortoceliac and aortomesenteric bypass before stent graft placement. The stent

Risk of rupture or dissection in descending thoracic aortic aneurysm.

BACKGROUND: Current practice guidelines recommend surgical repair of large thoracic aortic aneurysms to prevent fatal aortic dissection or rupture, but limited natural history data exist to support clinical criteria for timely intervention. METHODS AND RESULTS: Of 3247 patients with thoracic aortic aneurysm registered in our institutional Thoracic Aortic Center Database, we identified and reviewed 257 nonsyndromic patients (age, 72.4±10.5 years; 143 female) with descending thoracic or thoracoabdominal aortic aneurysm without a history of aortic dissection in whom surgical intervention was not

Endovascular treatment of descending thoracic aortic aneurysms and dissections.

Various endovascular techniques have become viable therapeutic alternatives in the treatment of patients with many types of descending thoracic aortic pathology and aortic dissections. Descending thoracic aortic aneurysms can be successfully treated using stent grafts. This technique is less invasive and is associated with acceptable morbidity and mortality rates. Patients who are particularly likely to benefit include the very elderly population; those with markedly compromised cardiac, pulmonary, or renal status; and individuals who have previously undergone complex operations on the thoraci

A rapidly expanding descending thoracic aortic aneurysm: an unusual complication.

The rate of enlargement of a descending thoracic aortic aneurysm is usually low, related to initial size and anatomical position. We report a case of an old, diabetic woman, with previous history of ischemic heart disease, admitted for a broncopneumia, in which a small descending thoracic aortic aneurysm was detected by a routine computed tomography (CT) scan. Because of the presence of comorbid condition, especially in the woman with an infectious respiratory disease, a closer follow-up by a 3-month CT scan was programmed. An impressive enlargement of the aneurysm, occurred in the next 2 mont

Repair of descending thoracic aortic aneurysms with Ankura Thoracic Stent Graft.

OBJECTIVE: The aim of the study was to present the results for patients with atherosclerotic aneurysm of the descending thoracic aorta (DTA) treated with a novel thoracic stent graft. METHODS: A single-center retrospective review of prospectively collected data was performed. We extracted demographic variables as well as atherosclerotic comorbidities and operation-related and imaging-related data from patients' medical records. We estimated technical success rate, in-hospital and 30-day mortality, and mortality at the end of follow-up as well as complication and reintervention rate in our stu

Natural history of descending thoracic and thoracoabdominal aortic aneurysms.

OBJECTIVES: Elucidating critical aortic diameters at which natural complications (rupture, dissection, and death) occur is of paramount importance to guide timely surgical intervention. Natural history knowledge for descending thoracic and thoracoabdominal aortic aneurysms is sparse. Our small early studies recommended repairing descending thoracic and thoracoabdominal aortic aneurysms before a critical diameter of 7.0 cm. We focus exclusively on a large number of descending thoracic and thoracoabdominal aortic aneurysms followed over time, enabling a more detailed analysis with greater granul
