[The role of IPA and dual diagnosis in psychiatry].

The co-occurrence of a psychiatric disorder and a substance use disorder affects many patients in adult psychiatry, with significant clinical and social repercussions. The fragmentation of healthcare systems hinders continuity of care. A survey of 147 nurses at two psychiatric facilities explored existing practices regarding dual diagnosis and defined the roles of advanced practice nurses. These nurses contribute to improving care and reducing disruptions in the care pathway.

[Addiction and schizophrenia. Nosological, clinical and therapeutic questions. 1. Alcoholism and schizophrenia].

The scientific discussion on the interrelationship of "alcoholism and psychosis" has been dominated by nosological problems for years. Alcohol hallucinosis, a comparatively rare psychosis in chronic alcoholism, can mimic the symptomatology of paranoid schizophrenia. The clinical features (vivid acoustic hallucinations and fear) are often inconclusive, as, unlike other organic psychoses, alcohol hallucinosis does not entail clouding of sensorium and disorientation. On the one hand follow-up studies suggest that alcohol hallucinosis in some cases can take a chronic schizophrenia-like course. Esp

Scope, quality and inclusivity of international clinical guidelines on mental health and substance abuse in relation to dual diagnosis, social and community outcomes: a systematic review.

OBJECTIVE: It is estimated that up to 75% of patients with severe mental illness (SMI) also have substance use disorder (SUD). The aim of this systematic review was to explore the scope, quality and inclusivity of international clinical guidelines on mental health and/or substance abuse in relation to diagnosis and treatment of co-existing disorders and considerations for wider social and contextual factors in treatment recommendations. METHOD: A protocol (PROSPERO CRD42020187094) driven systematic review was conducted. A systematic search was undertaken using six databases including MEDLINE,

Divalproex sodium in the treatment of adults with bipolar disorder.

Divalproex sodium (Depakote, Abbott Laboratories) is an anticonvulsant with well-established efficacy in the treatment of bipolar I disorder, manic or mixed episode. Over the past 10 years, an increased number of studies indicate that divalproex has a broad spectrum of activity in the treatment of bipolar depression and rapid cycling, psychotic symptoms, impulsive aggression and post-traumatic stress disorder. Newer formulations of divalproex, namely the extended-release formulation, are now available. The improved tolerability and convenience of the once-daily extended-release formulation has

Antipsychotic efficacy in psychosis with co-morbid cannabis misuse: A systematic review.

The prevalence of cannabis use in patients with psychotic mental illness is known to be high and is suspected to exacerbate symptoms and worsen prognosis. We aimed to evaluate evidence of antipsychotic efficacy in reducing the burden of psychotic symptoms and cannabis use in individuals with psychotic mental illness and co-morbid cannabis use. A systematic review was conducted of antipsychotic treatment in those with psychotic mental illness and co-morbid cannabis use. Quality of evidence for each study and outcomes were rated using the 'GRADE' approach. Twenty-two studies were identified: 13

[A case of primary ciliary dyskinesia who had been treated as asthma].

We report a case of 18-old girl with primary ciliary dyskinesia (PCD) who had been diagnosed as asthma. Since birth, she had presented with unexplained productive cough, sputum, rhinorrhea, and stridor with situs solitus. Her familial history was negative for PCD. At 2 years of age, ciliary beat frequency and beat pattern were normal. She was diagnosed as rhinosinusitis, chronic secretory otitis media. At 3 years of age, she was diagnosed as asthma because of wheezing not associated with respiratory infection. Various asthma medications were then administered, including DSCG, inhaled corticost

Adolescent substance abuse and psychiatric comorbidities.

The use and abuse of substances-including alcohol, nicotine, marijuana, inhalants, and other drugs-are commonly found to be comorbid with psychiatric conditions in adolescents. This dual diagnosis requires special attention and treatment, especially as substance use often begins during this developmental period. Adolescents may be diagnosed with substance abuse, substance dependence, or substance use disorder not otherwise specified, which indicates a developing substance use problem that includes symptoms of but does not meet criteria for substance dependence. Psychiatric comorbidity in adole

Interventions targeting patients with co-occuring severe mental illness and substance use (dual diagnosis) in general practice settings - a scoping review of the literature.

BACKGROUND: People with dual diagnosis die prematurely compared to the general population, and general practice might serve as a setting in the healthcare system to mend this gap in health inequity. However, little is known about which interventions that have been tested in this setting. AIM: To scope the literature on interventions targeting patients with dual diagnosis in a general practice setting, the outcomes used, and the findings. DESIGN AND SETTING: A scoping review of patients with dual diagnosis in general practice. METHODS: From a predeveloped search string, we used PubMed (Medli

Precision Psychiatry and Dual Disorders.

OBJECTIVE: Addiction co-occurs with distinct pathological personality traits, other psychiatric disorders or symptoms and cognitive impairment, which are known as dual disorders or co-occurring disorders. This symptomatic high concurrency suggests that both conditions are in some ways causally linked. Research is ongoing to identify distinctive neurobehavioral mechanisms and endophenotypes that predispose individuals to compulsive drug use and other mental disorders. Research is also providing new revelations about the diverse effects of substances on individuals, including differences accordi

[Social anxiety disorder and alcohol use disorders].

Social anxiety disorder is a mental condition with anxiety and avoidance in social settings and is frequently accompanied by self-medication with alcohol. Epidemiological studies demonstrate that the risk that patients with social anxiety disorder will develop alcohol dependence is more than two-fold. We will discuss epidemiological research findings about this comorbidity, as well as reasons for the comorbidity and the various treatments available for these conditions. Very few studies have analyzed the efficacy of treatments for the combination of these disorders and it is not yet clear whet

12-step participation among dually-diagnosed individuals: a review of individual and contextual factors.

The frequent co-occurrence of substance abuse disorders along with psychiatric disorders creates a number of complexities and needs in terms of long-term treatment for individuals. 12-step groups might provide unique mechanisms by which dually-diagnosed individuals can maintain their abstinence and improve their psychological functioning. This paper reviews the literature on outpatient community 12-step participation among dually-diagnosed individuals, and also focuses on individual factors that may interact with treatment: homelessness, legal status, and ethnicity. A total of 59 articles was

Delineation of dual molecular diagnosis in patients with skeletal deformity.

BACKGROUND: Skeletal deformity is characterized by an abnormal anatomical structure of bone and cartilage. In our previous studies, we have found that a substantial proportion of patients with skeletal deformity could be explained by monogenic disorders. More recently, complex phenotypes caused by more than one genetic defect (i.e., dual molecular diagnosis) have also been reported in skeletal deformities and may complicate the diagnostic odyssey of patients. In this study, we report the molecular and phenotypic characteristics of patients with dual molecular diagnosis and variable skeletal de

Early prenatal diagnosis of causative homozygous variants in ASCC1 in a fetus with cystic hygroma and additional homozygous variants of unknown significance associated with a neurological phenotype not visible in early gestation: Dual diagnosis or not?

A consanguineous couple was referred at 10 weeks of gestation (WG) for prenatal genetic investigations due to isolated cystic hygroma. Prenatal trio exome sequencing identified causative homozygous truncating variants in ASCC1 previously implicated in spinal muscular atrophy with congenital bone fractures. Prenatal manifestations in ASCC1 can usually include hydramnios, fetal hypo-/akinesia, arthrogryposis, contractures and limb deformities, hydrops fetalis and cystic hygroma. An additional truncating variant was identified in CSPP1 associated with Joubert syndrome. Presentations in CSPP1 incl

[Patients with a diagnosis of pancreatitis and "dual diagnosis"].

"Dual diagnosis"is usually understood as the co-occurrence of mental illness and addiction to psychoactive substances. In the last two decades can be seen an increased interest of researchers and practitioners in theoretical aspects of dual diagnosis, their means of diagnosis and planning treatment programs. Establishing a medical diagnosis may be questioned whether psychopathological symptoms are due to psychoactive substance use or are caused by mental illness. Difficulty in recognizing the problem of dual diagnosis may be questioned whether psychopathological symptoms are due to psychoactiv

The prevalence and correlates of dual diagnosis among adults in custody: A systematic review and meta-analysis.

BACKGROUND: Incarcerated individuals experience mental illness (MI), substance use disorders (SUD), and their co-occurrence - dual diagnosis - at higher rates than the general population. By systematically reviewing the literature on dual diagnosis in custody, we aimed to (1) estimate the pooled prevalence of dual diagnosis among adults in custody, and (2) identify the psychosocial, health-related, and criminal justice correlates of dual diagnosis. METHOD: We searched CINAHL, CINCH, Embase, Medline, PsycINFO, and Web of Science for studies investigating dual diagnosis among adults in custody.
