[Software for polygraphs used in scientific research].

Polygraphs--devices for simultaneous detection of several physiological parameters--are currently used in medicine in psychology. Special polygraphs are also used in scientific research. The process of data acquisition and processing in such polygraphs is usually computer-controlled. The goal of this work was to describe software for detection of polygraph signals used in cardiorespiratory system research. The software supports the experiment script language, allows hyperventilation testing, and makes it possible for the examination subject to control the data detection process.

Pulmonary hypertension.

The modern era in cardiopulmonary medicine began in the 1940s, when Cournand and Richards pioneered right-heart catheterization. Until that time, no direct measurement of central vascular pressure had been performed in humans. Right-heart catheterization ignited an explosion of insights into function and dysfunction of the pulmonary circulation, cardiac performance, ventilation-perfusion relationships, lung-heart interactions, valvular function, and congenital heart disease. It marked the beginnings of angiocardiography with its diagnostic implications for diseases of the left heart and periph

[The syndrome of increased upper airways resistance: What are the clinical features and diagnostic procedures?].

The upper airway resistance syndrome "UARS" is a poorly defined entity, often described as a moderate variant of the obstructive sleep apnea syndrome. It is associated with respiratory effort-related arousal, absence of obstructive sleep apnea, and absence of significant desaturation. It is a relatively common condition that predominantly affects non-obese young adults, with no predominance in either sex. The degree of upper airway collapsibility during sleep of patients with UARS is intermediate between that of normal subjects and that of patients with mild-to-moderate sleep apnea syndrome. C

Olfaction in rhinology--methods of assessing the sense of smell.

Olfactory disorders frequently occur in rhinological disease. Different subjective and objective test methods are available to assess the sense of olfaction. Among the subjective methods, screening tests and threshold measurements are commonly used to quantify hyposmia or anosmia. Qualitative methods are available using discrimination and identification tests. Objective methods are used in research and in some medicolegal situations. Objective tests include olfactory evoked potentials, functional Magnetic Resonance Imaging and functional Positron Emission Tomography. The measurement of the sen

[The modern approach to diagnostics and treatment of mycotic lesions in ear, nose, and throat].

The analysis of the prevalence of mycoses based on the literature data and original observations gives evidence of the increase in the relative frequency of mycotic lesions in the overall ENT morbidity. The occurrence of otomycosis among otitides of different etiology is estimated at 25% and that of pharyngomycosis in the patients presenting with chronic pharygitis and tonsillitis up to 28%. The mycotic lesions in the larynx are diagnosed in almost 15% of the patients with chronic laryngitis. Chronic inflammation of intranasal cavity and paranasal sinuses is associated with mycosis in 7% of th

[Bronchiectasis imaging].

Bronchiectasis are defined as an irreversible focal or diffuse dilatation of the bronchi and can be associated with significant morbidity. The prevalence is currently increasing, probably due to an increased use of thoracic computed tomography (CT). Indeed, the diagnosis relies on imaging and chest CT is the gold standard technique. The main diagnosis criterion is an increased bronchial diameter as compared to that of the companion artery. However, false positives are possible when the artery diameter is decreased, which is called pseudo-bronchiectasis. Other features such as the lack of bronc

[Childrens' chronic cough revisited].

Chronic cough is a common symptom in childhood and very often the expression of a cold like respiratory tract infection. Chronic cough may follow an acute cough lasting more than 4-8 weeks or may be the primary symptom of an underlying severe respiratory disease. It is important to find out the exact origin of chronic cough to allow accurate investigation steps and treatment. In childhood, adults like diagnostic approach is not appropriate and differential diagnosis must encompass children specific age dependent diagnosis. This article aims to stress a specific paediatric approach in prolonged

Update on empty nose syndrome: disease mechanisms, diagnostic tools, and treatment strategies.

PURPOSE OF REVIEW: To discuss the proposed pathophysiology of empty nose syndrome (ENS), summarize and evaluate the role of validated diagnostic tools in the diagnosis of ENS, and review the medical and surgical treatment strategies for patients with ENS. RECENT FINDINGS: Historically, ENS has been associated with a reduction in nasal turbinate size; new data suggest that impaired trigeminal nerve function may also play a role in the pathophysiology of the disease. The newly validated empty nose syndrome 6 item questionnaires and Cotton test are steps forward to standardize the diagnosis of E

Diagnosis issues in sarcoidosis.

Multiple problems may be encountered during the diagnosis of sarcoidosis: at first diagnose sarcoidosis in an appropriate clinical setting, secondly, identify any manifestation to be linked to sarcoidosis at diagnosis work-up and during evolution; thirdly, recognize "danger" in sarcoidosis and parasarcoidosis syndromes, and finally, diagnose sarcoidosis recovery. Diagnosis is often delayed as presentation may be diverse, non-specific, or atypical. Diagnosis of sarcoidosis is based on three criteria: a compatible presentation; evidence of non-caseating granulomas and exclusion of any alternativ

Determining respiratory impairment in connective tissue disease-associated interstitial lung disease.

Connective tissue diseases (CTDs) can affect the lungs through diseases of the chest wall, pleura, vasculature, airways, and parenchyma. Interstitial lung disease (ILD) is a common complication of CTD associated with increased morbidity and mortality. This article describes the evaluation of respiratory impairment in patients with CTD and summarizes the evidence that guides diagnosis and management of CTD-ILD. Patients with CTD with suspected ILD should undergo clinical, physiologic, and radiologic studies to evaluate for the presence of ILD, and these results should be integrated in a multidi

Diagnostic Methods in Primary Ciliary Dyskinesia.

Diagnosing primary ciliary dyskinesia is difficult. With no reference standard, a combination of tests is needed; most tests require expensive equipment and specialist scientists. We review the advances in diagnostic testing over the past hundred years, with emphasis on recent advances. We particularly focus on use of high-speed video analysis, transmission electron microscopy, nasal nitric oxide and genetic testing. We discuss the international efforts that are in place to advance the evidence base for diagnostic tests.

[Pulmonary lesions in Wegener's granulomatosis].

Pulmonary lesions are the commonest and prognostically unfavourable visceral pathology in Wegener's granulomatosis. The author discusses clinically and reongenologically detectable lesions associated with this systemic vasculitis with special reference to its most severe forms (alveolar bleeding and infectious complications frequently caused by active immunosuppressive therapy). Diagnostic (including early) and modem therapeutic modalities of Wegener's granulomatosis are considered.

Recent Advances in CRISPR/Cas Systems for Respiratory Pathogen Diagnostics.

Early, rapid, and accurate detection is essential for clinical management and epidemiological control of acute respiratory infections caused by pathogens. Traditional testing methods such as microbial culture, serological testing, and PCR are restrictive in terms of operation and logistics and are therefore not easily used in point-of-care settings. The CRISPR/Cas system is an adaptive prokaryotic immune system composed of clustered regularly interspaced short palindromic repeats and their associated proteins, which has been used as a nucleic acid diagnostic platform with programmable sequence

Rapid Point-of-Care Detection of Pathogens for Lower Respiratory Tract Infections in ICU Patients.

Lower respiratory tract infections (LRTIs) are among the leading causes of clinical deterioration and death in critically ill patients. In the intensive care unit (ICU), timely identification of the causative pathogen is essential for appropriate antimicrobial therapy; however, conventional culture methods take 3 to 5 days and have suboptimal positivity rates, particularly for fungi. Molecular approaches such as PCR shorten turnaround time but still depend on manual nucleic acid extraction and thermocycling, limiting their suitability for bedside use. This protocol describes a point-of-care wo

[Significance of the sampling points and the aerobic microbiological culture for the diagnosis of respiratory infections in reptiles].

Respiratory diseases play an important role in reptiles kept in captivity. Microbiological examinations are described as an essential part of the diagnostic possibilities. Therefore the aim of this study was to collect data on the usefulness of results obtained after aerobic culture (sheepblood, brilliantgreen, sabouraud's agar) of swabs and tracheal lavages following standardized sampling. Respiratory symptoms were found in 24.3% of the snakes, 16.5% of the tortoises/turtles and 1.6% of the lizards presented in the clinic for birds and reptiles at the university Leipzig. Altogether, 52% of th

Health system implementation of clinical decision support to optimize respiratory pathogen profile utilization.

UNLABELLED: Overuse of respiratory pathogen profiles (RPPs) in immunocompetent patients offers limited clinical benefit and may contribute to unnecessary hospital costs. Evidence supports RPP use primarily in severely ill, immunosuppressed, or pediatric patients, while smaller, targeted tests are more appropriate for routine cases . The goal of this study was to evaluate the impact of a clinical decision support (CDS) intervention on RPP utilization and associated reagent costs. We reviewed RPP utilization across the health system and implemented a CDS tool with pre-approved clinical indicatio

Mucosal wave measurement and visualization techniques.

Organized vibration of the vocal folds is critical for high-quality voice production. When the vocal folds oscillate, the superficial tissue of the vocal fold is displaced in a wave-like fashion, creating the so-called "mucosal wave." Because the mucosal wave is dependent on vocal fold structure, physical alterations of that structure cause mucosal wave abnormalities. Visualization and quantification of mucosal wave properties have become useful parameters in diagnosing and managing vocal fold pathology. Mucosal wave measurement provides information about vocal fold characteristics that cannot
