European journal of trauma and emergency surgery : official publication of the European Trauma Society2011eng
PURPOSE: Most gastrointestinal fistulae commonly occur following surgery. A minority is caused by a myriad of other etiologies and is termed by some as "uncommon fistulae". The aim of this study was to review these fistulae and their treatment.
METHODS: A literature review was carried out. Searches were conducted in Pubmed and related references reviewed.
RESULTS: Except for Crohn's disease and diverticulitis, "uncommon fistulae" are described in case reports or very small case series. Most of the patients were treated by surgery.
CONCLUSIONS: The anatomic features of the fistula and the et

Isolated iliac aneurysms are often discovered when they rupture, sometimes into a neighbouring organ. We report 2 cases of aneurysm of the common iliac artery, one which ruptured into the right common iliac vein and the other in to the sigmoid. Diagnosis was made at laparotomy in the first case and on the CT-scan of the pelvis in the second. The first case was treated with an exclusion graft using ilio-iliac bypass with suture of the venous breach via the aneurysm. In the second case, the aneurysm was excluded by ligature of the iliac arteries with extra-anatomic crossed femoro-femoral bypass,

Although infrequent, digestive fistulae in HIV/AIDS patients have been reported throughout the digestive tract from the esophagus to the anus, with predominance of esophageal fistulae. AIDS/HIV-associated opportunistic infections may invade the digestive system and lead to fistula formation. Tuberculosis is the most common infection associated with these esophageal fistulae. We report here one case of bile duct-duodenal fistula in a female AIDS patient with associated abdominal Mycobacterium tuberculosis infection compromising lymph nodes of the hepatic pedicle where the fistula was found. Appropriate treatment strategy for congenital heart disease may be influenced by coexisting congenital anomalies of respiratory or digestive system. Typical congenital anomalies that coexist frequently with congenital heart diseases and have influence on treatment strategy of congenital heart disease include trecheoesophageal fistula, diaphragmatic hernia, and tracheal stenosis. In neonates with congenital heart disease and tracheoesophageal fistula (the fistula) should be immediately repaired after birth, followed by cardiac surgery to ameliorate heart failure or hypoxemia if feasible. In neo

Based on the principle of magnetic anastomosis technique, the design of magnetic anastomosis system for endoscopic tissue clamping is proposed. The system includes a semi-ring magnet, a special structure transparent cap and a detachable push rod. With the help of the existing digestive endoscopy and endoscopic tissue gripper, the endoscopic close clamping and anastomosis of the bleeding or perforated tissue can be completed. After the anastomosis, the magnet falls off and is discharged through the digestive tract. Animal experiments showed that the system was easy to use, the fistula was clamp

The cystectomy with substitute enterocystoplasty because of the bladder cancer is a surgical technique which is the origin of several problems that the medicosurgical team faces. The aim of this study is to evaluate the postoperative complications of this surgical technique. We have realized a retrospective study of 56 patients hospitalized in intensive care service between 1994 and 1997. The complications observed were dominated by postoperative peritonitis (5.2%), urinary fistula (3.5%), digestive fistula (1.7%). Other general complications were recorded as pneumopathy (3.5%), digestive hemo

Experience in surgical treatment of 53 patients with internal biliodigestive fistulas is analysed. The formation of the fistula in all cases was caused by cholelithiasis with obstruction of the bile ducts, purulent cholangitis, angiocholitis, cholangiolytic abscesses of the liver, pancreatitis, hepatitis, as well as cholelithic ileus. Cholecystectomy, removal of the stones from the bile ducts, restoration of the main route of bile drainage, elimination of the pathological communication, and closure of the defect in a hollow organ should be considered the operation of choice. In cholelithic ile

Fourteen cases of digestive fistulas, complicated by a dehiscence of the laparotomy wound, are described. Ten patients have a single fistula and 4 have multiple digestive fistulas. There were eleven survivors. This represents a mortality of 21.4%. In 9 cases, the spontaneous recovery of the fistulated zone is followed by a losing of the laparotomy wound. In 2 cases, surgical intervention was necessary to permit the recovery of the fistula and of the dehiscence of the laparotomy. The treatment of such patients requires a mean hospitalization in an intensive care ward of about 48 days +/- 25.8 (

Nephro-intestinal fistulas involve the GI tract and the kidney or pyelocalyceal system. The main causes may be urologic or digestive, the site diverse. Reno-colic fistulas, although rare, are more frequently observed. We describe a case of left reno-colic fistula secondary to lithiasic pyonephrosis that presented as sepsis. Diagnosis was confirmed by retrograde pyelography. Treatment was by surgery (left nephrectomy, closure of the fistula, and colonorhaphy).

The appendiceal diseases, particularly appendicitis, are the most common disorders in the digestive system localized at the right lower quadrant area. However, appendiceal carcinoma with vesico-appendiceal fistula is a rare clinical phenomenon. Lacking specific symptoms, appendiceal carcinomas with fistula formations are often misdiagnosed as acute appendicitis cases. The purpose of this study is to increase awareness of appendiceal neoplasms and appendicovesical fistulas. We reported our experiences in three complex cases related to digestive and urological systems, and reviewed the literatur

In this study, the authors present a technical tip for better and easier visualization of rectourethral fistula both during dissection and fistula closure. In this technique, a small-caliber ureteral catheter is used for catheterization of the fistulous tract under direct urethroscopic guidance. Five cases were treated using this technique. All cases had either brachytherapy as a monotherapy or combined brachytherapy and external beam radiotherapy. All cases had the fistula completely healed except for one failed case that had a 2-cm fistula.

BACKGROUND: Spontaneous cutaneous fistula of hydatid liver cysts is a rare complication. Its genesis involves anatomic factors as well as other local factors.
AIMS: An exhaustive literature review was conducted to identify the characteristics of this complication, treatment modalities and the results obtained.
PATIENTS AND METHODS: An exhaustive bibliographic search was made for all articles published in French and English relating to parietal complications of hydatid liver cyst, from which we retained only those involving cases of cutaneous cyst fistulas (communicating rupture), to which we

Fistulas represent abnormal communications between two epithelialized surfaces, which can involve organs, vessels, or the skin. While common fistulas are frequently encountered, uncommon fistulas in the thoracic and abdominal cavities pose significant diagnostic challenges due to their varied etiologies and presentations. Imaging plays a pivotal role in the detection, characterization, and management planning of these complex entities. This pictorial review will explore the spectrum of uncommon thoracic and abdominal fistulas, emphasizing the crucial contributions of various imaging modalities

Anorectal fistulas are common maladies. The challenge in therapy of perianal fistulas balances between the best possible cure and the preservation of continence. Complex fistulous disease challenges even the most experienced surgical specialists. The management options in these groups of patients are inadequate, with treatment often requiring multiple procedures, causing a risk for continued symptoms and fecal incontinence. This has lead to a serious search for newer and safer treatment options. Use of different types of setons and advancement flaps have their own advantages and pitfalls. Inva
