Juvenile discitis.

Over a period of three years four girls and two boys presented with discitis. All were less than 5 years old at presentation, and each had a short history of symptoms. Three were initially thought to have pathological defects of the abdomen. All children showed abnormal posturing with exaggerated lumbar lordosis. Diagnosis was essentially clinical. All cultures were sterile. The erythrocyte sedimentation rate was increased in all the children and all had mild pyrexia. Symptoms lasted from two to 8 weeks. Discitis should be considered in any child with fever, abnormal posturing, and refusal to

[Postoperative lumbar discitis].

The authors report our experience on 19 cases of discitis developed after operations for herniated lumbar disc. Because of the negativity of the neuroradiological studies in the acute stage, the recognition of the typical syndrome (severe back pain, spasm of the paravertebral lumbar muscles, limited spinal motility, fever) beginning 3-30 days post-operatively and the study of some laboratory tests (elevated Erythrocyte sedimentation rate (ESR) and midly to moderately elevated white blood cells (WBC) are very important for diagnosis. The first radiographic findings (disc space narrowing, ecc.)

Cervical Discitis in Children.

Cervical discitis, though rare, should be included in the differential diagnosis of torticollis, neck pain and neurodevelopmental regression in motor skills in children and infants. Magnetic resonance imaging is the diagnostic method of choice. Treatment should be conservative with antibiotics only. The aim of this study was to describe the 10-year experience of a tertiary pediatric medical center with cervical discitis.

[Discitis after lumbar diskectomy].

The aim of this study was to examine the frequency and clinical course for early diagnosis (earlier signs-index of suspicion, later signs) and different clinical presentation of disctitis at deep infection wound (DIW) and superficially infection wound (SIW). The medical histories of adult patients, which were all hospitalized after lumbar discectomy at the Department of Rehabilitation during a 5-year period (1986-1990) was retrospectively analysed. The incidence of discitis by 231 patients was 0.43%. DIW 0.43%, and SIW 0.86% is incidence by 231 patients. Infection group with seven patients (th

[Intervertebral discitis in children].

Intervertebral discitis is a rare disorder which is easily missed. It presents with non-specific symptoms such as irritability, abdominal pain, decreased appetite and limping. The infection parameters can be normal, and blood cultures are often negative. The pathogenesis is not established but infectious, and inflammatory aetiologies have been suggested. Diagnostic golden standard is magnetic resonance imaging. The treatment is immobilization, anti-inflammatory drugs and often antibiotics. Early treatment is important to reduce the risk of complications such as nerve damage and spine fusion.

Discitis in young children.

Discitis is uncommon in children and presents in different ways at different ages. It is most difficult to diagnose in the uncommunicative toddler of one to three years of age. We present 11 consecutive cases. The non-specific clinical features included refusal to walk (63%), back pain (27%), inability to flex the lower back (50%) and a loss of lumbar lordosis (40%). Laboratory tests were unhelpful and cultures of blood and disc tissue were negative. MRI reduces the diagnostic delay and may help to avoid the requirement for a biopsy. In 75% of cases it demonstrated a paravertebral inflammatory

Secondary Discitis Masquerading as Treatment Failure of Primary Discitis: Case Report and Review of the Literature.

BACKGROUND: Vertebral osteomyelitis often presents with localized back pain at the site of infection and elevated inflammatory markers. It can generally be treated conservatively with antibiotics targeted at the causative microorganism, but failure of medical treatment often necessitates surgical debridement. There are no reports in the literature that describe a secondary infection masquerading as treatment failure of the primary infection. METHODS: We present a case of a 29-year-old male with a history of intravenous drug abuse who was treated with antibiotics for methicillin-sensitive Stap

Thoracic Discitis in Ankylosing Spondylitis.

Chronic ankylosing spondylitis can lead to several rare long-term complications including cauda equina syndrome and inflammatory discitis especially without treatment. These complications are uncommon, but there is evidence that they can be treated with anti-tumor necrosis factor (TNF) inhibitors. We present a case of a 52-year-old male with a 30-year history of undiagnosed ankylosing spondylitis with cauda equina syndrome on initial outpatient presentation with a negative lumbosacral magnetic resonance imaging (MRI). He was admitted later that month and was found to have thoracic discitis fro

Discitis due to Clostridium perfringens.

INTRODUCTION: A combination of disk space narrowing and vacuum phenomenon on radiographs of the spine is usually considered a reliable indicator of degenerative disk disease. We report a case in which vacuum phenomenon was related to Clostridium perfringens discitis. METHODS: A 79-year-old woman was admitted for inflammatory low back pain that had worsened steadily over the last 2 months. Her body temperature was normal, laboratory tests showed inflammation (erythrocyte sedimentation rate, 61 mm/h; and C-reactive protein, 13 mg/L), and blood cultures were negative. Imaging studies (radiograph

[Upper cervical discitis: case report].

A 48-year-old man complained of severe neck pain. T1-weighted magnetic resonance imaging (MRI) after intravenous injection of Gadolinium disclosed homogeneously enhanced masses in the retropharyngeal soft tissue, in the inter-vertebral disc space between C3 and C4, and in the spinal epidural space behind the vertebral body. The symptoms of the patient improved smoothly after intravenous injection of antibiotics without surgical procedure. In this case, MRI was very important and useful in the prompt diagnosis of the disease.

Infectious Discitis and Spondylodiscitis in Children.

In children, infectious discitis (D) and infectious spondylodiscitis (SD) are rare diseases that can cause significant clinical problems, including spinal deformities and segmental instabilities. Moreover, when the infection spreads into the spinal channel, D and SD can cause devastating neurologic complications. Early diagnosis and treatment may reduce these risks. The main aim of this paper is to discuss recent concepts regarding the epidemiology, microbiology, clinical presentation, diagnosis, and treatment of pediatric D and SD. It is highlighted that particular attention must be paid to t

An unusual cause of lumbar discitis.

STUDY DESIGN: Case report. OBJECTIVE: To identify an unusual cause of lumbar discitis. SUMMARY OF BACKGROUND DATA: Discitis resulting from a foreign body has not been reported in the English-language literature. METHOD: A case report is reviewed identifying the presence of this foreign object (titanium), erroneously implanted into the disc space at L5-S1. Imaging studies identify the location and surrounding reaction. RESULTS: The surgical exploration of this disc revealed an inflammatory response surrounding the disc with degeneration within the disc space. CONCLUSION: If this gynecologi

Contiguous discitis and osteomyelitis in children.

Magnetic resonance imaging of 16 patients with contiguous discitis and osteomyelitis provided a specific diagnosis and defined the anatomic extent of vertebral and soft-tissue involvement. Altered signal changes were evident in the disc, adjacent vertebra in the end plate and metaphyseal equivalent regions, and the anterior prevertebral tissues. Significant posterior spread and disc herniation were not evident. Fourteen patients had lumbar involvement; two had cervical involvement. The patients were followed-up for an average of 4 years 5 months. Scoliosis has developed in one patient, and fou

Pyogenic discitis due to Abiotrophia adiacens.

INTRODUCTION: Abiotrophia species have been referred to as nutritionally variant streptococci because of their fastidious nutritional requirements for growth. Abiotrophia species are difficult to identify with conventional solid culture. PRESENTATION OF CASE: A 48-year-old woman was admitted to our hospital with severe low back pain and body temperature of 38.2°C. Magnetic resonance imaging revealed edema and contrast enhancement of the L4 and L5 vertebral bodies with high signal intensity in the L3-4 and L4-5 intervertebral discs on the T2-weighted images. The patient underwent needle biopsy

Complicated septic cervical and lumbar discitis.

A 69-year-old lady presented with back pain for 5 days associated with spiking temperatures, lower limb weakness and urinary retention. Urgent MRI showed discitis at the disc between cervical vertebra seven (C7), thoracic vertebra one (T1) and lumbar vertebra three and four (L3-4), associated dural inflammation, stenosis of the cervical spinal canal and cervical cord oedema at the level of C3. No definite epidural abscess was seen. She was transferred to the spinal unit for observation. Following transfer she rapidly developed respiratory compromise and required emergency spinal decompression
