[Percutaneous lumbar diskectomy].

At the Department of Neurosurgery in Trondheim, 17 patients with low backpain and sciatica have been operated upon using percutaneous lumbal discectomy. The procedure was carried out under local anaesthesia and all patients were discharged on the first day after operation. Seven patients recovered completely, four patients showed some improvement, and six patients showed no change. All the patients who reported a complete recovery experienced pre-operative stress-dependent radicular pain with mild neurological deficits. In contrast, the group of patients who showed no change suffered more cons

[Percutaneous lumbar diskectomy].

The results after percutaneous lumbar discectomy were evaluated in 52 patients with low back pain corresponding to the L5 or S1 root. All patients were examined with a median follow-up time of 14 weeks. The patients were divided in two groups. Group 1: patients with an obvious indication for surgery (29 patients) and group 2: patients with a relative indication for surgery (23 patients). The follow-up results for patients in group 1 were comparable with the results after conventional discectomy. The results concerning follow-up low back pain in groups 1 and 2 were good or excellent in 69% and

[Discitis after lumbar diskectomy].

The aim of this study was to examine the frequency and clinical course for early diagnosis (earlier signs-index of suspicion, later signs) and different clinical presentation of disctitis at deep infection wound (DIW) and superficially infection wound (SIW). The medical histories of adult patients, which were all hospitalized after lumbar discectomy at the Department of Rehabilitation during a 5-year period (1986-1990) was retrospectively analysed. The incidence of discitis by 231 patients was 0.43%. DIW 0.43%, and SIW 0.86% is incidence by 231 patients. Infection group with seven patients (th

[Antero-lateral, cervical microsurgical diskectomy].

We report 25 cases of cervical disc lesion, treated with antero-lateral microsurgical discectomy without intercorporal graft. The posterior osteophyts were removed and the posterior longitudinal ligament lateral resected. In the control period of 6 months to 4 years we have in 84% of cases excellent/good results clinically and radiologically and concerning the working capacity. Comparing the results with 51 other cases operated on with the fusions technique after Cloward, there are no significant differences. We give a short description of the technique and review the literature.

Effect of Nicotine Dependence and Smoking on Revision Diskectomy After Single-Level Lumbar Diskectomy.

Removal of a herniated disk that is causing neural compression is among the most common indications for spinal surgery. Previous population database studies of risk factors for reoperation after this procedure analyzed small to medium numbers of patients. To date, no study has concurrently assessed the effect of modifiable risk factors, such as smoking and nicotine dependence, with a large number of patients. Data were obtained with commercially available software that houses de-identified data for several major US health care systems. A database search was conducted to find all patients who h

A comparison of the effects of automated percutaneous diskectomy and conventional diskectomy on intradiscal pressure, disk geometry, and stiffness.

Diskectomy, chemonucleolysis, percutaneous diskectomy, and laser ablation are used to treat patients with sciatica. The effects of percutaneous diskectomy on the intradiscal pressure of the human disk are not known. Our aims were to determine (a) whether removal of nucleus through automated percutaneous diskectomy significantly reduces intradiscal pressure without altering the disk geometry and stiffness, and if so, how much nucleus removal is required to achieve these goals; and (b) whether the effects of conventional diskectomy on these same parameters are equivalent to removal of nucleus th

Surgical outcome of percutaneous endoscopic interlaminar lumbar diskectomy for recurrent disk herniation after open diskectomy.

STUDY DESIGN: Technical report. OBJECTIVE: To present a detailed surgical technique of percutaneous endoscopic interlaminar diskectomy (PEID) for recurrent lumbar disk herniation and present features of postoperative magnetic resonance images that were unavailable in previous studies. SUMMARY OF BACKGROUND DATA: Revision lumbar diskectomy is troublesome because of the difficulty in dissecting a surgical scar. Endoscopic diskectomy is regarded as an alternative method with comparable clinical outcome and less complication. Technically, a transforaminal approach is similar to a virgin operatio

Nightmare of lumbar diskectomy: aorta laceration.

STUDY DESIGN: Case report. OBJECTIVE: To present a rare case of a patient who during posterior lumbar spinal decompression and diskectomy underwent a laceration of the aorta. The patient survived with prompt treatment. SUMMARY OF BACKGROUND DATA: Intraoperative injuries of the prevertebral and pelvic vessels are infrequent but serious complications of posterior lumbar disc surgery. Injury may cause laceration of the vessel with acute life-threatening retroperitoneal hemorrhage, which is usually massive. The aortic bifurcation is closely approximated to the anterior surface of the L4-L5 disc.

Video-assisted thoracoscopic diskectomy and fusion.

Thoracoscopic diskectomy has been described in adults as an alternative to thoracotomy for access to the anterior spine for correction of scoliosis, but its use in children for correction of spinal deformities has not been reported. Eight patients have undergone video-assisted thoracoscopic diskectomy with fusion before posterior instrumentation. In five patients, the posterior fusion and instrumentation followed the thoracoscopic procedure under the same anesthesia; in three patients it was staged and performed 1 week later. The mean time required for the thoracoscopic procedure was 174 minut

Percutaneous laser diskectomy. An alternative method.

1. The percutaneous posterior approach with laser to a herniated lumbar intervertebral disc--heralded as a "minimal invasion" treatment for the symptomatic relief of discogenic back pain and sciatica--is an alternative method for the treatment of protrusion of a lumbar disc and its associated radiculopathy. 2. Percutaneous diskectomy is considered in patients who have unremitting radiculopathy with a contained disc herniation. The particular criteria for patient selection includes: evaluation of the patient's pain, performance status, medication dependence, and duration of symptoms. The patien

Lumbar diskectomy for recurrent disk herniation.

Eighty-two patients who underwent reoperation using a lumbar microdiskectomy approach for recurrent back and leg pain were evaluated retrospectively. Patients were entered consecutively except for exclusion of those who had a spinal fusion in addition to the diskectomy or those where a diskectomy was performed as an adjunct to decompression for spinal stenosis. As a percentage of all diskectomies performed by us over the 13 years of the study, the overall rate of reoperation (including all patients who underwent more than one diskectomy procedure regardless of vertebral level and side) was 7.4

[Imaging of the lumbar spine after diskectomy].

The radiological investigation of persistent or recurrent sciatica after lumbar diskectomy essentially consists of demonstrating recurrent disk herniation. Comparison between plain and contrast enhanced CT or MR examinations at the level of the diskectomy is the main step of the radiological survey. The meanings of the various radiological findings are discussed. Other lesions that may induce persistent sciatica after lumbar diskectomy include degenerative narrowing of the lateral recess, spinal instability, stress fracture of the remaining neural arch, pseudo-meningomyelocele after laminectom

[Laser puncture diskectomy in discogenic lumbosacral radiculitis].

Percutaneous laser discectomy (PLD) was performed in 273 patients with herniated disc-induced lumbosacral radiculitis. Indications for PLD were based on clinical and neurological findings and results of MR tomography: no benefit from conservative treatment, pain syndrome in the lumbago stage and sciatica not presenting with severe paresis and with herniated intervertebral discs down to 6 mm in size across.

Magnetic resonance evaluation after temporomandibular joint diskectomy.

The aim of this study was to investigate the value of magnetic resonance imaging after diskectomy of the temporomandibular joint. Magnetic resonance images were obtained before and 12 months after unilateral diskectomy without disk replacement. Magnetic resonance findings at follow-up were correlated to residual pain. At the follow-up, 20 of 28 patients were free of pain in the joint that had been surgically treated, four patients had mild pain, and four patients had significant residual pain. The magnetic resonance images at follow-up showed that the joint space was filled with soft tissue af

Radiographic morphology in the temporomandibular joint after diskectomy.

The purpose of the study was to evaluate the radiographic appearance in the temporomandibular joint after diskectomy in patients refractory to conservative treatment. The diskectomies were performed in patients in which the common diagnosis were internal derangement or symptoms of osteoarthrosis. Postoperative radiographs were taken in 25 patients after 1-17 yrs and compared with the preoperative from both the operated and the nonoperated joints. Moderate to severe osteoarthrotic changes were found on the operated side in all but two patients in contrast to on the nonoperated side where such c
