[Dissection and dissected aneurysm of the abdominal aorta].

Experience in successful treatment of 7 patients with isolated dissection of the abdominal aorta is presented. Over the last 10 years 165 patients with dissected aneurysms of different sites underwent surgery. In 7 (4.2%) of them isolated dissection of the abdominal aorta was diagnosed. Among 251 patients operated for abdominal aortic aneurysm dissection of its wall was seen in 2.8% cases. Morphologic study revealed atherosclerosis in 3 patients, degeneration of aortic wall - in 3, and combination of dysplasia with atherosclerosis - in 1. Linear prosthesis of abdominal aorta with replantation

Dissecting thoracic aorta and fusiform aneurysm of the abdominal aorta.

A 59-year-old woman with dissection of the thoracic aorta and a fusiform aneurysm of the abdominal aorta without evidence of Marfan's syndrome underwent aneurysmorrhaphy with a bifurcated expanded polytetrafluoroethylene graft. Histological specimens of the aneurysmal wall revealed the presence of idiopathic cystic medial necrosis. As typical findings of idiopathic cystic medial necrosis in the aortic wall are very rare except in cases of Marfan's syndrome, the present case is reported and the implications of this condition are discussed.

[Post-traumatic dissection of the abdominal aorta].

Dissections due to deceleration trauma are rarely limited to the infradiaphragmal aorta (only 2-3%) and are usually lethal. Here we report the unusual course of an abdominal aortic dissection with aneurysmatic enlargement of the false lumen. Based on diagnostic imaging, a therapeutic stent application was planned in order to close the entry and to prevent rupture. During the intervention sondation of the false lumen revealed that the left renal artery had a reentry. Due to the complexity of the entry-reentry situation of the left renal artery the intervention was not possible, and the patient

Spontaneous isolated dissection of the abdominal aorta.

Isolated spontaneous dissection of the abdominal aorta is such a rare entity and there are only a few cases reported in literature up to date. A 42-year old male was admitted to the hospital with mild pain in the lower abdomen and back that had began seven days prior to admission together with the sudden onset of the ischemic symptoms of the left leg (ischemic ulcers of the calf gangrenous toe and pallor foot). Patient denied any trauma, hypertension history was negative, while he was active cigarette smoker. MSCT and digital subtracted angiography have shown a dissection of the abdominal aort

Spontaneous dissection of the infrarenal abdominal aorta.

Spontaneous infrarenal abdominal aortic dissection is rare. We observed enlargement of a spontaneous infrarenal aortoiliac dissection in a 55-year-old hypertensive man. Open surgical repair with a bifurcated polyester graft was successful. A review of the English literature found 41 previously published cases. Mean age was 58 years, 74% of the patients were male, and 62% had hypertension. None had Marfan or Ehlers-Danlos syndrome. More than three fourths of the patients had symptoms, 6 patients (14%) presented with aortic rupture. Dissection was limited to the infrarenal aorta in 50% and exten

Dissecting aneurysm of the infrarenal abdominal aorta.

The aim of this study was to report the case of a patient with chronic dissecting infrarenal abdominal aortic aneurysm (AAA) and to review the literature for this rare vascular disorder. The preoperative assessment, surgical treatment, and postoperative course of a patient with a dissecting AAA and associated left iliac artery dissection were analyzed. The literature is reviewed with respect to etiology and pathogenesis as well as diagnostic and therapeutic management of infrarenal dissecting AAA. The preoperative diagnosis of dissecting infrarenal AAA was made by computed tomography and aorto

[Dissecting aneurysm of the abdominal aorta simulating acute abdominal diseases].

The paper is concerned with analysis of 9 case histories of patients with dissection and rupture of the abdominal aorta, simulating the acute abdomen. Diagnosis of this complication was very difficult: it was established in one patient only, in another patient it was suspected. In the other cases, a diagnosis was made at autopsy only. Physicians and surgeons of emergency care, outpatient clinics and hospitals should keep this pathology in mind.

Dissection of the abdominal aorta in a child with Takayasu's arteritis.

Takayasu's arteritis is a chronic inflammatory disease that primarily involves the aorta and its main branches. Varying degrees of narrowing, occlusion, or dilatation develop in the involved vessel segments. However, dissection of the aorta is quite rare in this disease, and it may develop particularly after angioplasty. We report a very rare case of Takayasu's arteritis with dissection of the abdominal aorta just distal to the origin of the inferior mesenteric artery in a 9-year-old girl. She was treated conservatively with close follow-up. At the end of 1 year's follow-up, the dissection of

[Imaging of aneurysms and dissections of the aorta. Part 1--Aneurysms of the abdominal aorta].

The authors review the main imaging findings of aneurysms and dissections of the aorta. Based on the experience and imaging techniques available in their institution (Conventional Radiology, Sonography and Computed Tomography), they try to make a decision tree of radiologic procedures in ambulatory and emergency patients. Nowadays, in the majority of situations, these techniques allow the Radiologist to make the correct diagnosis, detect the complications and evaluate the patients in the pre and post-surgical phases. Other imaging techniques not available in their institution, such as Magnetic

Aortic Dissection Limited to Abdominal Aorta: an Underrecognized Entity?

BACKGROUND: Aortic dissection limited to the abdominal aorta is a rare clinical entity with non-specific clinical features. Optimal treatment, as well as natural history and progression of the disease, remain unclear. In 1992 we -reported 5 cases of isolated abdominal aortic dissection (IAAD) and in the present paper we update our series with 5 additional patients. A concise literature review is also provided. METHODS: Between 1992 and 2014, we diagnosed 5 patients with IAAD (4 men, mean age 60.6 years, range 45-77). No patient presented with acute onset of symptoms. One patient was diagnosed

[Dynamic renal scintigraphy of dissecting aneurysm of abdominal aorta].

Dynamic renal scintigraphy on 15 patients of dissecting aneurysm of abdominal aorta extending below the renal arteries were retrospectively studied. The results were reviewed and classified into 3 types according to perfusion images and parenchymal tissue uptake: Type A: symmetric perfusion and parenchymal tissue uptake, Type B: asymmetrical perfusion and parenchymal tissue uptake, Type C: Delayed perfusion of one kidney and symmetric parenchymal tissue uptake. The number of patients who showed Type A, Type B, and Type C were 3 cases, 6 cases, and 6 cases, respectively. In all Type A and 3 of

Spontaneous isolated dissection of the abdominal aorta: single-center experience.

Isolated abdominal aortic dissection is a rare clinical entity. We report a single-center experience with six patients with acute isolated abdominal aortic dissection managed during a period of seven years. The patients' median age was 62 years and they were predominantly male. All patients presented acutely with abdominal pain, one with hypovolemic shock due to aortic rupture, two with acute lower limb ischemia and one with buttock necrosis. Interestingly, one major symptom was astasia observed in four patients that resolved postoperatively. Computed tomography scan of the thoracic and abdomi

Surgical experiences in acute spontaneous dissection of the infrarenal abdominal aorta.

OBJECTIVE: Spontaneous dissection of the abdominal aorta should be considered as a vascular surgical emergency. We report here our experience with this rare pathology. METHODS: At our hospital, we operated on 200 patients for abdominal aortic aneurysm over a 4-year period, and aortic dissection was the underlying pathology in eight of them. Diagnosis was based on history, imaging study findings and visualization at surgery. RESULTS: The mean size of the aorta at the time of diagnosis was 5.5 +/- 0.4 cm. Aortic dissection was complicated by acute ischemia of lower limb and paraplegia in 1 pat

[Surgical management of a chronic post traumatic dissection of the abdominal aorta].

The clinical case of a 34 years old male is reported, who was referred to our clinic complaining of disabling claudication of the lower extremities. No risk factors for atherosclerotic disease could be identified. Six years previously he sustained a severe automobile crush injury, resulting in cranio-encephalic and abdominal trauma, and fractures of the left knee and both legs. Appropriate medical and surgical care were delivered and he recovered completely, with no restrictions. Six months before the observation, he started to complain of intermittent claudication of both legs. An angio-CT ev

Replacement of the abdominal aorta with an aortic homograft in a patient with an aortic dissection.

The use of aortic and femoral homografts in early vascular surgery has been abandoned for the more successful and abundant synthetic substitutes. With the recent introduction of cryopreservation, homograft use has again met with improved success. A 40-year-old man who had a DeBakey Type I aortic dissection initially underwent replacement of the aortic root with a pulmonary homograft. Subsequently, in the presence of an intraabdominal infectious process, progressive mesenteric and lower limb ischemia was treated by replacing the abdominal aorta with an aortic homograft. Thirty-six months postop

Endovascular correction of a distal re-entry in an abdominal aorta dissection.

The re-entries are still a problem for the endovascular treatment of aortic dissections. A 60-year-old man was treated for an abdominal aortic dissection using aortic monoiliac endograft until the left iliac external artery and a femoro-femoral bypass with occlusion of the right common iliac artery and the left hypogastric artery. In his third year of follow-up, a re-entry tear in the right hypogastric ostium was diagnosed with pressurization of the aortic and common iliac aneurysmatic sac, that required correction. A self-expandable covered stent in a 'U' configuration was implanted, connecti
