Acute Colonic Diverticulitis.

Acute colonic diverticulitis is a gastrointestinal condition that is frequently encountered by primary care and emergency department practitioners, hospitalists, surgeons, and gastroenterologists. Clinical presentation ranges from mild abdominal pain to peritonitis with sepsis. It is often diagnosed on the basis of clinical features alone, but imaging is necessary in more severe presentations to rule out such complications as abscess and perforation. Treatment depends on the severity of the presentation, the presence of complications, and underlying comorbid conditions. Medical and surgical tr

Acute colonic diverticulitis.

Colonic diverticulosis is truly a disease of the 20th century. A direct correlation is thought to exist between the incidence of diverticular disease and the amount of dietary fiber. Acute colonic diverticulitis occurs in approximately 25 per cent of the patients with diverticula, and 20 per cent of the patients with diverticulitis will ultimately require surgical intervention. Because of the often virulent nature of the disease in younger patients and the prevalence in the geriatric population, an aggressive approach is advocated. Primary resection of the involved segment of colon is advocate

Sonography of colonic diverticulitis.

Sonographic findings in 16 patients with diverticulitis of the colon are described. The wall of the inflamed segment of the colon appeared hypoechoic and thickened. Maximum thickness of the wall ranged from 5 to 17 mm. Length of the most severely inflamed segment of the colon ranged from 6 to 9 cm. In addition, adjacent contiguous segments of the colon, less involved with inflammation, ranging in length from 7 to 15 cm, were demonstrated on sonographic examination of five patients. Long segments of the inflamed colon could be demonstrated by oblique scanning. The appearance of the mucosal refl

Complications of colonic diverticulitis.

About half of the patients in whom diverticulitis develops have subsequent attacks. Recurrence increases the likelihood both of complications, such as perforation, abscess, fistula, obstruction, and hemorrhage, and of the necessity for surgical intervention. For these reasons, patients who have had two or more attacks of acute diverticulitis should be considered candidates for prophylactic resection.

[Surgical treatment of colonic diverticulitis].

BACKGROUND: Colonic diverticulitis is a serious disease that may require surgical treatment. Early diagnosis and management are necessary to reduce the rates of morbidity-mortality. PURPOSE: This review article was performed in order to discuss the most recent advances in the diagnosis and management of colonic diverticulitis. RESULTS: Patients with acute diverticulitis should be categorized based upon the presence or absence of complications. Patients with complicated diverticulitis should be further categorized into I) pericolonic abscess, II) distant abscess (retroperitoneum or pelvis), I

Computerized tomography of colonic diverticulitis.

In this review article, the value of computerized tomography (CT) vs. contrast enema (CE) in patients with acute diverticulitis is discussed. Acute diverticulitis is a frequent differential diagnosis in abdominal disease. Approximately 15 to 30% of all patients with diverticulosis develop diverticulitis, whereby approximately 21% of these must undergo surgery. The clinical as well as the radiographical differential diagnosis between diverticulosis and diverticulitis often poses a great problem. While the contrast enema demonstrates the changes of the intestinal lumen, the true extent of the in

ACG Clinical Guideline: Colonic Diverticulitis.

Colonic diverticulitis is a common and costly disease, with a lifetime risk of 3%-5%, that drives substantial healthcare use. This guideline provides practical, evidence-based recommendations for the management of diverticulitis in the outpatient gastroenterology setting. Consultations should address diagnosis, typical clinical course, risk factors, the role of antibiotics, and prevention. Computed tomography imaging is essential to confirm the diagnosis of diverticulitis, particularly at the first presentation and in severe cases. After recovery, colonoscopy is recommended for patients with c

Imaging update: acute colonic diverticulitis.

Because the incidence of colonic diverticulosis is high in the general population, incidental asymptomatic diverticulosis is commonly seen on radiology imaging studies. However, diagnostic imaging performed specifically for diverticular disease is essentially limited to imaging of suspected acute colonic diverticulitis (ACD) and its complications. The clinical diagnosis of ACD can be challenging, and imaging has become an essential tool to aid in diagnosis, assess severity of disease, and aid in treatment planning. Computed tomography (CT) has replaced contrast enema as the imaging procedure o

[Computed tomography of colonic diverticulitis].

33 patients with clinically suspected diverticulitis of the colon were studied prospectively by CT. The predictive value of symptoms, such as thickening of the colonic wall (86.6%), inflammatory changes of the pericolic fatty tissue (87.5%), the presence of diverticula (73.3%) and abscess formation (100%), were examined separately and their significance was evaluated. Our study was performed mainly on clinically less severe cases of diverticulitis. True positive results by CT were reached in 20/21 cases (sensitivity = 95.2%), true negative findings in 9/12 (specificity = 75.0%). The results of

Colonic cancer and associated diverticulitis.

In a total group of 385 cases of colonic cancer, there was evidence of associated diverticulitis in 42 cases. Of these, 92% of the patients were over 60 years of age. In 45% of the cases the coexisting lesions were located in the sigmoid colon. It is suggested that cancer of the colon contributes to the development of diverticulosis and diverticulitis; this is confirmed by the finding of 52.8% of the total group of diverticulosis (98 cases) complicated by diverticulitis. Signs of diverticulitis may mask the presence of cancer, and the diagnosis of associated lesions is sometimes difficult. The

[Colonic diverticulitis: diverticulitis with complications and its treatment (author's transl)].

Complications (stenosis, intestinal obstruction, abscess or fistula formation, inflammation, etc.) of colonic diverticulitis are seen in 35% of patients affected. Multiple operative procedures involve less risk than surgery performed in a single session. The mortality in emergency surgery is reported to be up to 40%, while subsequent resection of the diseased colon involves the usual mortality of surgery on the colon, i.e. 1.5-9%. The vital factor in this illness is the earliest possible surgical treatment of the diseased diverticulum, so that the extremely problematical complications can be a

Surgical management of complicated colonic diverticulitis.

BACKGROUND: Improvements in surgical techniques, advances in intensive care medicine and progress in the management of peritoneal sepsis have recently favoured colonic resection with primary anastomosis in the treatment of complicated diverticulitis. METHODS: Some 224 patients with complicated diverticulitis who had undergone surgery in the preceding 22 years were reviewed. Complications present on admission included acute phlegmon without pus formation (92 patients), paracolic abscess and/or localized peritonitis (99), diffuse purulent peritonitis (33), complete obstruction of the sigmoid co

Emergency surgery of complicated colonic diverticulitis.

Colon diverticular disease is an increasingly frequent disorder especially in Western populations characterized by high living standards. In 30-40% of subjects over 60 years of age barium enema detects the presence of diverticula in the sigma. 10-20% of patients affected by colon diverticular disease develop complications such as inflammation or haemorrhage and 20-30% of these patients undergo surgery (60% of patients aged less than 40). Emergency surgery is performed in 50% of cases and it is currently burdened by high mortality rates. We decided to review our cases history to clarify the ind

Pathogenesis of colonic diverticulitis and diverticulosis.

Colonic diverticula result from herniation of the mucosa through weak spots in the muscular wall. Clinically manifested diverticulitis has been thought to have its pathologic basis in an abscessed diverticulum obstructed by a fecalith, but studies of resected sigmoids have failed to produce evidence to support this view. Instead, the outstanding lesion was found to be a perforation in the fundus of a diverticulum, with surrounding peridiverticular or pericolic inflammation. Another surprising finding in pathologic studies was that one out of three sigmoids resected for "diverticulitis" showed
