The diving accident (decompression incident, DCI) occurs in the decompression phase of dives. The DCI can either be caused by an arterial gas embolism (AGE) subsequent to a pulmonary barotrauma or by the formation of inert gas bubbles subsequent to a reduction of ambient pressure during the ascent from depth. In contrast to the traditional assumption that decompression incidents only occur if decompression rules are neglected, recent data indicate that a vast amount of diving accidents occur even though divers adhered to the rules. Hence, there is a large inter- and intraindividual variability

Dear Readers, Welcome to this year's third issue of the International Maritime Health Magazine. Every medical specialty has a history, but not every history is preserved. Hyperbaric medicine in Latin America did not emerge from a single institution or a single pioneering individual. Rather, it evolved through diverse national experiences that were shaped by geography, maritime traditions, military and civilian medicine, engineering, scientific curiosity, and the dedication of countless, often silent, professionals. The articles in this issue offer more than historical accounts of individual co

Hyperbaric oxygen therapy (HBOT), i. e. breathing pure oxygen at elevated ambient pressure, remains the gold standard of care in treating air or gas embolism and decompression illness. Guidelines are less clear on the value of HBOT in acute management of carbon monoxide (CO) poisoning or clostridial necrosis. To evaluate the evidence of clinical efficacy of HBOT we performed a systematic literature review. Part 1 assesses acute indications such as air or gas embolism, decompression sickness, CO-poisoning, clostridialmyonecrosis, necrotizing problem wounds, acute traumatic wounds and arterial r

INTRODUCTION: Hyperbaric chambers and underwater environments are challenging and at risk of serious accidents. Personnel aiming to assist patients and subjects should be appropriately trained, and several courses have been established all over the world. In healthcare, simulation is an effective learning technique. However, there have been few peer-reviewed articles published in the medical literature describing its use in diving and hyperbaric medicine.
METHODS: We implemented the curriculum of the Master's degree in hyperbaric and diving medicine held at the University of Padova with emerg

Humans, led by their eternal wish to explore the unknown, have always wanted to perfect their diving skills and conquer the sea world. The adverse conditions experienced by divers brought about medical problems and a new field of medicine. Diving medicine serves the identification, treatment, and precautions against illnesses that are related to diving activities. While the development of diving equipment is advancing, divers have had the chance to reach greater depths for a longer time. Along with this success, a novel medical condition under the term 'decompression illness' (DCI) was introdu

As is well known, the origins and development of hyperbaric medicine are closely tied to the history of diving medicine. Our HBO2 studies stemming from diving medicine date back to 1972. We concentrated our early basic research on dysbaric osteonecrosis. There are now good indications that HBO2 is helpful in a variety of orthopedic conditions. However, hyperbaric medicine in orthopedics is still relatively new and some aspects of it remain controversial.

BACKGROUND: From the late 1960s until 1975, the Naval Physiology Department was established to focus on underwater medical research, serving the training needs of naval special forces. Since the 1980s, the Naval Medical Institute has applied recompression to treat some diving accidents, but the results have been limited. Some hospitals in Hanoi, Haiphong and Ho Chi Minh City were also equipped with monoplace and multiplace (4 people) hyperbaric chambers. However, clinical application research has not received much attention. In 2001, VINIMAM was established, and in 2007, the Center for Underwa

Psychotic disorders, characterised by impaired reality testing and a spectrum of symptoms, present significant challenges in assessing fitness for diving. While diving can be a safe and rewarding activity, the unique physiological and environmental stresses of hyperbaric conditions can exacerbate psychotic vulnerability or mimic psychotic symptoms. This article reviews the literature on psychosis and diving, exploring the implications of psychotic disorders, psychotropic medications, and hyperbaric effects. It highlights the critical importance of illness insight, the absence of comorbid condi

People dive out of fascination or for economic reasons. Today, technical devices allow us land-dwelling organisms to breathe underwater. The effect of water pressure has a significant impact on physiological processes in the organism and leads to specific pathophysiologies. A sound knowledge of this is the basis of good medical care for people who dive to depths below the surface. Based on a number of exemplary accident scenarios, the article will examine the important pathophysiologies in diving medicine. Various accident mechanisms produce gas bubbles in the body, which can lead to symptoms

INTRODUCTION: This case report describes an initially overlooked Type II decompression sickness (DCS) occurrence that was confused with a cerebral vascular accident in a patient with chronic atrial fibrillation (AF). The purpose of this case report is to reinforce the maxim that DCS needs to be suspected anytime a scuba diver experiences signs or symptoms compatible with DCS after completing a scuba dive.
METHODS: A 71-year-old scuba diver with a history of AF and who was taking warfarin made four dives, all with maximum depths less than 60 fsw (20 msw) over a 10-hour interval. Shoulder pain

INTRODUCTION: The Divers Alert Network (DAN) aims to provide safety information for all types of diving. Assessing the number of active closed-circuit rebreather (CCR) divers is difficult, as pertinent information is often not available. This review aims to give an overview of global use and safety of CCR diving equipment from 2013-2022.
METHODS: Data were combined and assessed from various DAN internal and public sources on CCR diver demographics, fatalities, and CCR sales.
RESULTS: Over the past 10 years, the number of certified CCR divers has increased from an estimated 2,000 in 2013, to

Background/Objectives: Hyperbaric oxygen therapy (HBOT) and mild hyperbaric oxygen therapy (mHBOT) are two terms commonly used by hyperbaric chamber sellers, hyperbaric oxygen therapy practitioners, and even by many scientists as if they were synonymous. The following review of hyperbaric oxygen therapy goes into the relevant aspects that relate to the clinical uses of mHBOT and HBOT, such as pressure, the percentage of oxygen in inspired gas, the amount of oxygen dissolved in plasma, and biological effects, as they apply to a broad range of diseases that can be treated with mHBOT and HBOT. INTRODUCTION: Decompression illness (DCI) is a recognised risk of freediving. When timely hyperbaric oxygen therapy (HBOT) is unavailable, oxygen-based in-water recompression (IWR) may be considered.
METHODS: A 49-year-old competitive freediver developed severe neurological DCI within minutes of surfacing from a dive to 117 metres of seawater at a sanctioned competition, with altered mental status, right facial droop, right hemiparesis and aphasia. With an anticipated 24-48 h delay to HBOT, emergency IWR was initiated with a full-face mask and 100% oxygen. Incremental ascent from 18 msw over

As a non-invasive and drug-free treatment method, hyperbaric oxygen therapy has been applied in cerebrovascular accidents to improve cerebral hypoxia, reduce ischemia-reperfusion injury following stroke and promote neural repair. However, its clinical efficacy remains controversial. This work aims to analyze the progress of research on the role of hyperbaric oxygen in stroke through bibliometric methods and takes relevant literature from the Web of Science core database from 2007 to December 31, 2024. The results showed that from 2007 to the present, the annual number of English publications i

BACKGROUND: Diving is a diffused recreational activity, and the number of divers carrying cardiac implanted devices is similarly growing. Due to the lack of guidelines or technical indications, the suitability of such devices for diving or the fitness to dive for these patients still needs to be determined.
OBJECTIVE: This work summarizes implantable cardiac devices' suitability for recreational diving, technical vulnerability factors, and recommendations to improve implanted divers' safety.
METHODS: Between May 1, 2021, and March 20, 2022, three interventional cardiologists retrieved the te

Unwitnessed diving fatalities present significant challenges for forensic investigation. We describe a fatal diving accident involving a 32-year-old experienced female diver during a deep night dive in freshwater. The diver became separated from her buddy and was recovered at depth in a head-down position, with evidence of entanglement of the central manifold valve of the twin-tank open-circuit breathing apparatus by a thin guideline. Autopsy findings were consistent with drowning. Technical examination of the diving equipment revealed no mechanical malfunction. Given the absence of eyewitness

BACKGROUND: Breath-hold diving (BHD, also referred to as freediving) represents an extreme physiological challenge, requiring adaptations to rapid changes in blood gas levels and hydrostatic pressure. Despite advances in understanding human responses to BHD, knowledge gaps remain. With this state-of-the-art review, research trends and progression were tracked to inform future investigation directions. METHODS: A structured literature search was conducted in PubMed and Scopus (2005–2025), selecting peer-reviewed studies on physiological, biochemical, and biomechanical aspects of BHD. Thematic a
