The role of dopaminergic imaging in patients with symptoms of dopaminergic system neurodegeneration.

Diagnosis of neurological and psychiatric conditions associated with disturbances of dopaminergic functioning can be challenging, especially in the early stages, and may be assisted with biomarkers such as dopamine system imaging. Distinguishing between Alzheimer's disease and dementia with Lewy bodies is a major diagnostic challenge. Clinical diagnosis of Parkinson's disease is straightforward with classic presentation, but accurate distinction among Parkinsonian variants may be difficult; non-Parkinson's disease conditions are commonly misdiagnosed as Parkinson's disease, and ~20% of patient

Imaging Dopaminergic Neurotransmission in Neurodegenerative Disorders.

Imaging of dopaminergic transmission in neurodegenerative disorders such as Parkinson disease (PD) or dementia with Lewy bodies plays a major role in clinical practice and in clinical research. We here review the role of imaging of the nigrostriatal pathway, as well as of striatal receptors and dopamine release, in common neurodegenerative disorders in clinical practice and research. Imaging of the nigrostriatal pathway has a high diagnostic accuracy to detect nigrostriatal degeneration in disorders characterized by nigrostriatal degeneration, such as PD and dementia with Lewy bodies, and diso

PET tracers for imaging of the dopaminergic system.

The dopaminergic system plays a major role in neurological and psychiatric disorders such as Parkinson's disease, Huntington's disease, tardive dyskinea and schizophrenia. Knowledge on altered dopamine synthesis, receptor densities and status are important for understanding the mechanisms underlying the pathogenesis and therapy of diseases. PET provides a non-invasive tool to investigate these features in vivo, provided the availability of suitable radiopharmaceuticals. To investigate presynaptic function, PET-tracers have been developed to measure dopamine synthesis and transport. For the for

Functional imaging of dopaminergic neurotransmission in Tourette syndrome.

Tourette syndrome (TS) is a heritable neuropsychiatric disorder that presents in childhood with a constellation of motor and nonmotor symptoms. Many of the earliest efforts to elucidate the anatomic substrate of TS focused on the contribution of the basal ganglia to the disorder. Simultaneously, however, a substantial body of evidence has accrued to implicate dysfunction of regions outside the basal ganglia in the generation of tics. In particular, while activation studies have shown evidence for cortico-striato-thalamo-cortical circuit abnormalities in TS, several other functional imaging rep

Biological imaging and the molecular basis of dopaminergic diseases.

The development and validation of preclinical biological probes of nigrostriatal dysfunction are part of the next frontier for battling diseases involving dopamine deficiency. In this work, the quantitative relationship relationship between radiofluorinated L-DOPA, [e.g., L-3,4-dihydroxy-6-[18F]fluorophenylalanine (6-[18F]fluoro-L-DOPA, FDOPA)] kinetics measured with positron emission tomography and central dopamine biochemistry is discussed. A hypothesis of a possible "non-linearity" of FDOPA kinetics with dopaminergic cell losses is presented to explain apparent discrepancies in post-mortem

Dopaminergic imaging: clinical utility now and in the future.

Over the past ten years, dopaminergic imaging has become increasingly part of the assessment and diagnosis of dementia. There are numerous PET and SPECT ligands available that target different steps in the process of neurotransmission. Abnormalities in dopaminergic imaging measures are consistent features of dementia with Lewy bodies (DLB) and other parkinsonian syndromes, and can be used to facilitate diagnosis, particularly in distinguishing between DLB and Alzheimer's disease. This review summarizes present knowledge in this area and the implications for current and future clinical practice

Imaging beyond the striatonigral dopaminergic system in Parkinson's disease.

Parkinson 's disease (PD) is characterized by progressive loss of dopaminergic neurons in the nigrostriatal pathway, but this seems to constitute only part of the whole pathological process of the disease. Accumulating data have documented the concomitant degeneration of other dopaminergic pathways and of the serotonergic, cholinergic and noradrenergic neurotransmitter systems. In addition, pathologic process is not only restricted in the brain, since the spinal cord and the peripheral autonomic nervous system are also affected. The pathogenesis of PD remains unclear. The use of positron emiss

Multimodal dopaminergic and free-water imaging in Parkinson's disease.

INTRODUCTION: When using free-water diffusion imaging or positron emission tomography (PET), it is established that substania nigra microstructure and presynaptic dopamine activity are impaired in early PD. It is not well understood if these two forms of degeneration are redundant, or if they each provide a unique contribution to the clinical motor and cognitive symptoms. METHODS: A total of 129 PD and 75 control individuals underwent motor and cognitive evaluations, and in vivo [11C]dihydrotetrabenazine (DTBZ) monoaminergic brain PET imaging and diffusion imaging. Dopaminergic imaging separates normal pressure hydrocephalus from its mimics.

OBJECTIVE: Idiopathic normal pressure hydrocephalus (iNPH) and iNPH mimics (i.e., Parkinson's disease, progressive supranuclear palsy or dementia with Lewy bodies) share similar clinical features, and discrimination between both conditions relies on invasive time-consuming investigations. This study aims to compare [123I]FP-CIT SPECT imaging-visual rating and semiquantitative values-between iNPH and iNPH mimics. METHODS: Among 56 patients with a suspicion of iNPH (76.5 ± 6.1 years; 23.2% women), 26 fulfilled the iNPH diagnostic criteria and the remaining 30 were classified as iNPH mimics. Pat

SPECT and PET imaging of the dopaminergic system in Parkinson's disease.

This paper gives an overview of the clinical importance of SPECT and PET imaging of the dopaminergic system in the differential diagnosis and for the determination of the progression rate of Parkinson's disease (PD). D2 receptor imaging can help to differentiate multiple system atrophy (MSA) and progressive supranuclear palsy (PSP) from PD. In patients treated with neuroleptics it is possible to determine the rate of striatal D2 receptor blockade using this technique. This occupancy rate parallels the occurrence of parkinsonian side effects. Its measurement helps in the selection of newer atyp

Combined Striatal Dopaminergic and Cardiac Sympathetic Imaging in Parkinson's Disease.

Background/Objectives: Imaging of both cardiac sympathetic denervation and nigrostriatal dopaminergic degeneration can support the diagnosis of Parkinson's disease (PD). However, their temporal relationship and combined diagnostic value remain unclear. This review addresses (1) whether nigrostriatal degeneration and cardiac sympathetic denervation are correlated in PD and (2) the comparative and combined diagnostic accuracy of striatal dopaminergic and cardiac sympathetic imaging in PD. Methods: We searched PubMed (October 2025) for studies assessing both a striatal dopaminergic and a cardiac

Dopaminergic imaging in degenerative parkinsonisms, an established clinical diagnostic tool.

Parkinson's disease (PD) and other neurodegenerative parkinsonisms are characterised by loss of striatal dopaminergic neurons. Dopamine functional deficits can be measured in vivo using positron emission tomography (PET) and single-photon emission computed tomography (SPECT) ligands assessing either presynaptic (e.g. dopamine synthesis and storage, transporter density) or postsynaptic terminals (i.e. D2 receptors availability). Nuclear medicine imaging thus helps the clinician to separate degenerative forms of parkinsonism with other neurological conditions, e.g. essential tremor or drug-induc

Regional metabolic changes in parkinsonian patients with normal dopaminergic imaging.

Dopaminergic imaging has been found to be normal in approximately 15% of parkinsonian patients enrolled in neuroprotective trials. We used (18)F-fluorodeoxyglucose positron emission tomography (FDG PET) to determine the metabolic basis for this finding. We reviewed scans from 185 patients with clinical signs of Parkinson's disease (PD) who underwent (18)F-fluorodopa PET imaging for diagnostic confirmation. Of this group, 27 patients (14.6%) had quantitatively normal scans; 8 of these patients were additionally scanned with FDG PET. Pattern analysis was performed on an individual scan basis to

Cocaine analogue conjugated magnetic nanoparticles for labeling and imaging dopaminergic neurons.

Molecular imaging of the dopamine transporter (DAT) with Positron Emission Tomography (PET) or Single Photon Emission Computed Tomography (SPECT) has been widely used in studies of neurological and psychiatric disorders. Nevertheless, there is a great interest in expanding molecular imaging to include magnetic resonance technology, because of the superior spatial resolution this technology may provide. Here we present a magnetic nanoparticle (NP) that specifically targets dopaminergic neurons and allows DAT imaging with magnetic resonance imaging (MRI). The nanoparticle (namely, NP-DN) is comp

Multimodal brain and retinal imaging of dopaminergic degeneration in Parkinson disease.

Parkinson disease (PD) is a progressive disorder characterized by dopaminergic neurodegeneration in the brain. The development of parkinsonism is preceded by a long prodromal phase, and >50% of dopaminergic neurons can be lost from the substantia nigra by the time of the initial diagnosis. Therefore, validation of in vivo imaging biomarkers for early diagnosis and monitoring of disease progression is essential for future therapeutic developments. PET and single-photon emission CT targeting the presynaptic terminals of dopaminergic neurons can be used for early diagnosis by detecting axonal deg

[METABOLIC AND DOPAMINERGIC BRAIN FUNCTIONAL IMAGING IN DEMENTIA WITH LEWY BODIES].

Dementia with Lewy bodies (DLB) is the second most common neurodegenerative dementia of the elderly. Its early diagnosis is a clinical challenge due to overlapping symptoms of various neurodegenerative diseases. Early diagnosis is essential for patient management avoiding neuroleptic medications that harbor the risk of causing further deterioration. Functional imaging allows accurate diagnosis and improves the degree of confidence in the diagnosis. Its advantage is based on the ability to identify biochemical changes that precede structural changes, thereby providing valuable information at ea

Positron emission tomography molecular imaging of dopaminergic system in drug addiction.

Dopamine (DA) is involved in drug reinforcement, but its role in drug addiction remains unclear. Positron emission tomography (PET) is the first technology used for the direct measurement of components of the dopaminergic system in the living human brain. In this article, we reviewed the major findings of PET imaging studies on the involvement of DA in drug addiction, especially in heroin addiction. Furthermore, we summarized PET radiotracers that have been used to study the role of DA in drug addiction. To investigate presynaptic function in drug addiction, PET tracers have been developed to

Linking polygenic risk scores to dopaminergic neuron loss using neuromelanin-sensitive imaging.

Parkinson's disease is characterized by the degeneration of dopaminergic neurons in the substantia nigra pars compacta (SNc), and neuromelanin-sensitive MRI provides a biomarker to track this neuronal loss. Isolated REM sleep behaviour disorder (iRBD), associated with cognitive decline, may represent a distinct subtype of synucleinopathy. Polygenic risk scores for these conditions may be associated with neuronal degeneration. This study investigates whether genetic risk scores for Parkinson's disease (PGS000903) or iRBD (PGS003414) are associated with neuromelanin signal loss in the SNc in the

In vivo imaging of dopaminergic neurotransmission after transient focal ischemia in rats.

The precise biologic mechanisms involved in functional recovery processes in response to stroke such as dopaminergic neurotransmission are still largely unknown. For this purpose, we performed in parallel in vivo magnetic resonance imaging and positron emission tomography (PET) with [(18)F]fluorodeoxyglucose ([(18)F]FDG) and [(11)C]raclopride at 1, 3, 7, 14, 21, and 28 days after middle cerebral artery occlusion in rats. In the ischemic territory, PET [(18)F]FDG showed a initial decrease in cerebral metabolism followed by a time-dependent recovery to quasi-normal values at day 14 after ischemi
