New inhalers have been developed because of difficulty of using the conventional metered dose inhaler (MDI) and because of the impending ban on CFC's. Dry powder inhalers (DPI's) in general are easier to use than the MDI and cause fewer irritant effects. Unlike the MDI few patients develop a poor inhalation technique with continued use of DPI's. Comparisons of multidose DPI's have shown that they achieve a similar degree of bronchodilatation to the MDI. Comparisons of the Diskhaler and Turbuhaler have shown little difference in therapeutic efficacy, but most indicate patient preference for the

INTRODUCTION: Inhalation of herbs and other compounds has a long history but habits for medical treatment are intertwined with rituals to obtain hallucinatory effects and pleasurable sensations. Several examples of inhaled herbs, and the diseases they were used for, based on early translations of ancient manuscripts related to inhalation were found to be speculative and inconsistent with each other in literature. They needed to be reconsidered and verified with the original sources of information.
AREAS COVERED: Examples of ancient inhalation and the development of early dry powder inhalers u

Dry powder inhalers (DPIs) are essential for pulmonary drug delivery, yet how adhesive formulation powders are converted into well-dispersed aerosols remains poorly resolved, which is highly device- and formulation-dependent. Here we integrate device and formulation characterization, theoretical analysis, numerical simulations, and machine learning to map airflow-particle dynamics across DPIs. Device geometry sets internal flow fields, which modulate drag, swirl, and impact, thereby mobilizing powders and driving transient deagglomeration. For fine particle-dominated DPI formulations, gravity

Asthma affects an estimated 4.8 million children. The pressurized metered-dose inhaler (pMDI), despite problems associated with its use and concern that most of these inhalers contain ozone-damaging chlorofluorocarbons (CFCs), is currently the device most frequently used to deliver inhaled medication. Concerns regarding pMDIs that contain CFCs have led to further development of alternative delivery devices, including CFC-free pMDIs, breath-actuated devices, and dry powder inhalers (DPIs). Advantages and disadvantages of these devices are discussed briefly, with emphasis on the new DPIs. A brie

Dry powder inhalers (DPIs) are the newer devices whereby medicine is dispensed as a fine powder inhaled deeply into the lungs, which is more simplified and stable compared to the pressurized metered dose inhalers. DPIs were developed primarily for respiratory disorders such as asthma and chronic obstructive pulmonary disease, but are being investigated for other uses. This review explicates the features of DPIs and how they are made, looking at their effectiveness in respiratory therapy, noting features such as usability, stability of the drugs, and flexibility. It also discusses the potential

Chlorofluorocarbons (CFCs), used in metered dose inhalers (MDIs), have been identified as being deleterious to the environment leading to a ban on their production. Dry powder inhalers (DPIs) are a widely used alternative to MDIs. One disadvantage of DPIs is that in vivo lung deposition can be influenced by the patient's inspiratory flow rate. The ASTA Medica multi-dose dry powder inhaler (AM-MDPI) has been designed to offer low resistance on inhalation, so that asthmatic patients can achieve inhaled flow rates of approximately 90 L x min(-1). The aim of the study was to evaluate the in vivo d

Lung cancer is the leading cause of cancer-related deaths. Traditional chemotherapy causes serious toxicity due to the wide bodily distribution of these drugs. Curcumin is a potential anticancer agent but its low water solubility, poor bioavailability and rapid metabolism significantly limits clinical applications. Here we developed a liposomal curcumin dry powder inhaler (LCD) for inhalation treatment of primary lung cancer. LCDs were obtained from curcumin liposomes after freeze-drying. The LCDs had a mass mean aerodynamic diameter of 5.81 μm and a fine particle fraction of 46.71%, suitable

Ivacaftor, a CFTR potentiator, is clinically effective in treating cystic fibrosis (CF), but it has poor lung targeting, variable oral bioavailability, and poor aqueous solubility. The objective of the study was to develop and optimize an inhalable proliposomal dry powder inhaler (DPI) of ivacaftor for pulmonary delivery. The optimized ivacaftor-loaded proliposomes were optimized using a face-centered central composite design (α = 1) by varying soy phosphatidylcholine (SPC) concentration and drug: cholesterol ratio, with particle size, zeta potential, and entrapment efficiency. The optimized f

Invasive pulmonary aspergillosis poses a significant threat to immunocompromised patients, characterized by high mortality rates. Posaconazole (PSZ), a second-generation triazole antifungal, exhibits broad-spectrum activity but suffers from limited pulmonary concentrations and notable systemic side effects when administered orally or intravenously. This study focuses on optimizing PSZ nanocrystals-agglomerated particles for dry powder inhalers (DPIs) to enhance solubility, dissolution rates, and pulmonary deposition, ultimately improving therapeutic efficacy while minimizing systemic adverse e

Idiopathic pulmonary fibrosis (IPF) is a common disease of pulmonary fibrosis with an increasing incidence. Silk fibroin peptides (SFPs) are bioactive substances rich in essential amino acids and possess antioxidant and anti-inflammatory properties; however, their application is limited by drawbacks such as low bioavailability and susceptibility to protein degradation. In this study, spray-drying technology was employed to prepare a silk fibroin peptides dry powder inhaler (SFPs-DPI), followed by characterisation of its physicochemical properties and preliminary in vitro and in vivo studies. Respiratory tract infections remain a significant global health challenge, with inhalation therapies playing a critical role in targeted drug delivery. This study investigates the application of Deep Reinforcement Learning (DRL) to determine the physical parameters of inhalers, aerosol particles, and their dissemination flow that maximise lung deposition for oral inhaled drug delivery. Results from this study suggest that DRL implemented with Proximal Policy Optimisation (PPO) works well for highly multivariate cases. In this study, the DRL performance in cases designed with the maximum number

Despite great promise, application of mRNA therapeutics in the lung has proven challenging. Many groups have reported success instilling liquid mRNA formulations in animal models, but direct intratracheal administration of large liquid quantities to the human lung presents significant safety and distribution concerns. To accomplish safe and effective mRNA delivery to the lung, formulations must be prepared for dosing via inhalation. An inhaled mRNA delivery system for the lung must be both robust enough to survive inhalation conditions and potent enough to deliver mRNA upon reaching the lung.

The global incidence of lung diseases associated with high mortality and morbidity rates is projected to rise in the coming years. Conventional therapies face limitations such as low bioavailability and systemic side effects with oral administration, as well as poor patient compliance and frequent dosing with traditional inhalers. These shortcomings have created an urgent need for novel pulmonary drug delivery systems. In recent years, nanoparticulate drug carriers have gained significance for their potential to enable targeted lung delivery and reduced dosing frequency. Among them, solid lipi

Dry powder inhalers (DPI) are frequently used by asthmatic patients, and the usage rate increases every year. The pharmacists at our hospital provided initial inhalation instructions on how the inhaler must be used but did not elaborate on the cleaning of the device. Therefore, the cleaning status of the inhaler is unknown, and there is a possibility of bacterial growth. This study investigated the cleaning status and hygiene of steroid drug inhalers used by elderly asthma patients. We administered a questionnaire to investigate the inhaler cleaning status after inhalation, and conducted a cro

This study employs high-resolution Optical Coherence Tomography (OCT) to characterize aerosol deposition within a model Dry Powder Inhaler (DPI). Deposition was examined for five in-house formulations containing different mass fractions of coarse lactose carrier (SV010), fine lactose (LH300), and Salbutamol Sulphate (SS), and for two commercial formulations, namely Aridol® (mannitol) and Pulmicort Flexhaler™ (budesonide). Experiments were performed at dispersion flow rates of 30 LPM and 50 LPM to evaluate the influence of formulation composition and aerodynamic conditions. Three-dimensional OC

BACKGROUND: The optimal management of chronic obstructive pulmonary disease (COPD) requires effective drug delivery through dry powder inhalers (DPIs). Maximum forced inspiratory flow (FIFmax), easily obtained from routine spirometry, can reflect inspiratory capacity and serve as a physiological marker associated with exacerbation risk. However, its clinical implications have not been fully established.
METHODS: This retrospective cohort study analyzed COPD patients treated with DPIs over a 5-year observational period. Baseline and follow-up FIFmax values were measured through repeated spirom

High-efficiency dry powder inhalers (DPIs) were developed and tested for use with carrier-free formulations across a range of different inhalation flow rates. Performance of a previously reported DPI was compared with two new designs in terms of emitted dose (ED) and aerosolization characteristics using in vitro experiments. The two new designs oriented the capsule chamber (CC) at different angles to the main flow passage, which contained a three-dimensional (3D) rod array for aerosol deaggregation. Computational fluid dynamics simulations of a previously developed deaggregation parameter, the

A series of co-engineered macrolide-mannitol particles were successfully prepared using azithromycin (AZ) as a model drug. The formulation was designed to target local inflammation and bacterial colonization, via the macrolide component, while the mannitol acted as mucolytic and taste-masking agent. The engineered particles were evaluated in terms of their physico-chemical properties and aerosol performance when delivered via a novel high-payload dry powder Orbital(™) inhaler device that operates via multiple inhalation manoeuvres. All formulations prepared were of suitable size for inhalation
