The relationship between peripapillary duodenal diverticulum and benign biliary tract disease was studied. Peripapillary duodenal diverticulum could be classified pathophysiologically into three types. Type I represents the disease not directly affecting the biliary tract. Type II shows the elevation of bile duct pressure directly caused by intraduodenal pressure loading. Type III includes patients in whom the diverticulum is small and is prone to cause papillitis or mechanical stimulation. Various stresses are considered to be involved in the development of gastric and duodenal ulcers. We interviewed patients of either gastric ulcer group (GU) or duodenal ulcer group (DU) with regular occupation who were admitted to our Psychosomatic Illness Department in order to examine the differences in the characteristics of their stress and developmental factors of their diseases between the two groups. Examination of 124 pancreatico-duodenal preparations obtained during surgery and of 250 pancreatic preparations at post-mortem has shown, that patients with chronic pancreatitis do have rather frequently cysts of the duodenal wall (38,5%). The work is devoted to the results of the treatment of 535 patients with diverticula of the duodenum, associated diseases being found in 15% of cases. The clinical picture is dependent on the relationship of the diseases: which of them is the leading one at the moment, are they concomitant or competing. The author comes to the conclusion that planned operation of full volume is necessary and expedient. The conclusion is supported by good immediate and late results.

Endoscopic duodenal biopsies were taken from 27 patients with suspected coeliac disease and compared with intubation capsule jejunal biopsies. The specimens were reported without knowledge of the patients' names or symptoms. In 24 patients (89%), coeliac disease could either be diagnosed or excluded with 100% accuracy, despite the inability to orientate the biopsies correctly. The condition of the major duodenal papilla and the periampullary region was studied in 1,113 patients with diseases of the biliary tract and pancreas. In 88.6% of cases complex examination revealed various pathological changes which were considered to be important etiological factors of the development of the diseases. These changes were found to be particularly significant in patients with the postcholecystectomy syndrome. The authors present the modern methods for correction of pathology of the major duodenal papilla, among which endoscopic interventions are most valuable.

X-ray examination of 123 patients showed duodenal diverticula seated primarily in the descending part. In 60% of the patients, they were located in the area of the major duodenal papilla. An experience with transduodenal operations on the major duodenal papilla in 123 patients is described, in 91% the operative interventions were performed on strict indications (stenosis of the papilla, incarceration of concrements in it). Complications of different degree without lethal outcomes were noted in 17.1% of the patients. Lethality was 3.2%. Long-term results from 6 months to 9 years were studied in 101 patients. Excellent and good results were observed in 80.2%, satisfactory--in 16.8%, bad--in 3% of the patients.
