Dysthymic disorder.

Preview Dysthymic disorder, or dysthymia, is a mild to moderate depression that smolders for years despite brief moments of emotional reprieve in which the individual experiences a normal mood. These fleeting euthymic periods do not extend beyond a couple of months at a time. The up-and-down course is as potentially debilitating as that of major depression, yet dysthymia is surprisingly underdiagnosed. The authors review the diagnostic characteristics, comorbid conditions, and clinical approach to management of this insidious mood disorder.

[Dysthymic disorders].

Long lasting, low intensity depressive episodes have been diversely integrated according to the classifications types or the psychodynamic points of view. The concept of anxious persistent lasting depression, neurotic depressive states, neurotic depression ... have been unified into the dysthymic disorder category of the DSM classification. This concept unification have been a topic of dispute considering that dysthymic disorder was a restrictive, heterogeneous an extensively comorbid diagnosis. Nevertheless the definition of this category offers the opportunity to place the notions of tempera

Dysthymic disorder: forlorn and overlooked?

This ongoing column is dedicated to the challenging clinical interface between psychiatry and primary care-two fields that are inexorably linked.Dysthymic disorder is a smoldering mood disturbance characterized by a long duration (at least two years in adults) as well as transient periods of normal mood. The disorder is fairly common in the US general population (3-6%) as well as in primary care (7%) and mental health settings (up to one-third of psychiatric outpatients). While the etiology of dysthymia remains unknown, there appears to be a genetic susceptibility, which may manifest in the pr

[Dysthymic disorder and its treatment].

Dysthymia is a new nosographic category in the latest American and international classification of mental disorders. Dysthymics were previously considered either as "neurotic depressive characters" or "characterologic depressives." Today it has been demonstrated that the personality and affective traits of these patients are in fact simply different expressions of a constitutive predisposition to clinical depression. Some studies have shown biological and neurophysiological relationships between dysthymia and major mood disorders. We therefore submit that dysthymia constitutes a subaffective e

Diagnostic criteria for dysthymic disorder.

The DSM-III (American Psychiatric Association 1980) criteria for dysthymic disorder selected a heterogeneous group of patients who overlapped with major depression and personality disorders in ways that were difficult to interpret. DSM-III-R (American Psychiatric Association 1987) revised the dysthymia criteria by (1) distinguishing early from late age of onset; (2) providing separate designations for primary and secondary states of dysthymia; (3) including a category of chronic major depression; and (4) revising the specific content of criteria. The performance characteristics of the new crit

Dysthymic disorder: the chronic depression.

Dysthymic disorder is defined as chronic depression of a mild to moderate degree for at least two years' duration. The disorder tends to be underdiagnosed despite a prevalence of 5 to 15 percent in primary care settings. Both the diagnosis and treatment of dysthymic disorder may be complicated by a variety of comorbid psychiatric and medical conditions as well as chronic stressors. Treatment may be determined by the accompanying comorbid condition. Antidepressant drugs are moderately efficacious in the treatment of dysthymia, with selective serotonin reuptake inhibitors preferred over tricycli

Dysthymic disorder in the elderly population.

The diagnosis of dysthymic disorder was created in DSM-III and maintained in DSM-IV to describe a depressive syndrome of mild to moderate severity of at least two years' duration that did not meet criteria for major depressive disorder. The prevalence of dysthymic disorder is approximately 2% in the elderly population where subsyndromal depressions of lesser severity are more common. Dysthymic disorder was replaced in DSM-V by the diagnosis of "persistent depressive disorder" that includes chronic major depression and dysthymic disorder. In older adults, epidemiological and clinical evidence s

Recognition and management of dysthymic disorder.

Dysthymic disorder, a chronic disturbance of mood, manifests as depressed mood for most of the day, more days than not, for at least two years. In children, dysthymia may present as irritable mood, and a duration of symptoms of only one year is required to make the diagnosis. By definition, there is no history of a major depressive disorder. Associated symptoms include poor appetite or overeating, insomnia or hypersomnia, poor concentration or difficulty making decisions, low energy, low self-esteem and feelings of hopelessness. Because of the chronic nature of this disorder, treatment require

Dysthymic disorder: the depression that never quits.

Dysthymic disorder, an insidious and chronic depressive mood disorder that waxes and wanes in intensity over several years, is fairly prevalent in healthcare settings. Although the explicit etiology is unknown, serotonergic dysfunction may be involved. Dysthymia appears to have a high rate of comorbidity, including both psychiatric and medical disorders. The primary care physician should maintain an awareness of this mood disorder, be able to screen efficiently for signs and symptoms, and be able to differentiate major depression from dysthymia. The foundation of treatment is pharmacotherapy,

Citalopram in the treatment of dysthymic disorder.

This study aimed to provide preliminary data on the tolerability and effectiveness of citalopram for patients with dysthymic disorder. Twenty-one adult subjects meeting DSM-IV criteria for dysthymic disorder were enrolled in this 12-week open-label study, of whom 15 had pure dysthymia (e.g. no major depression in the past 2 years). Citalopram was initiated at 20 mg/day, and increased to a maximum of 60 mg/day. Response was defined as 50% or greater drop in score on the Hamilton Depression Rating Scale (HDRS) and a Clinical Global Impressions-I score of 1 ('very much improved') or 2 ('much impr

Current pharmacotherapeutic approaches for dysthymic disorder and persistent depressive disorder.

Introduction: Persistent Depressive Disorder (PDD) is a nosological entity introduced with DSM-5, encompassing numerous different conditions including Dysthymia, recurrent Major Depressive Disorder, Double Depression and Chronic Major Depression. PDD is a particularly significant cause of disease burden in the general population. Areas covered: In the present paper, the authors explore the controversies surrounding the definition of PDD, the current approach to its treatment endorsed by the major scientific bodies, along with the available evidence on the efficacy of said treatments. Platelet MAO activity in patients with dysthymic disorder.

The aim of the present study was to examine possible associations between platelet monoamine oxidase (MAO) activity and primary dysthymic disorder. For that purpose we estimated the enzyme activity in 58 patients (15 males and 43 females) selected according to DSM-III-R criteria and in 61 healthy controls (30 males and 31 females). Platelet MAO activities were found significantly lower in the female patients compared to female controls. Moreover, the enzyme activities were found to be even lower in the female patients who had attempted suicide. These differences did not exist in the male popul

Comparison of prototypic cases of depressive personality disorder and dysthymic disorder.

Although depressive personality disorder (DPD) was included in the Diagnostic and Statistical Manual of Mental Disorders, Fourth Edition (DSM-IV; American Psychiatric Association, 1994) appendix as a proposed category needing further research, there are concerns that it overlaps excessively with dysthymic disorder (DD). The purpose of this investigation was to identify the characteristic features of representative patients with DPD and patients with DD, and to determine whether they could be distinguished by their symptoms. Two matched samples of psychologists (n=57, n=48) identified a prototy

A critical discussion of DSM-III dysthymic disorder.

The authors review the history of the concept of dysthymia and the literature on the epidemiology, course, and treatment of chronic depression. They present a critical discussion of DSM-III and DSM-III-R criteria for dysthymic disorder. On the basis of this review, they suggest that future revisions of the nomenclature include further subcategorization of chronic depressive disorders and that the term "dysthymic disorder" be reserved for chronic depressive disorders with an insidious onset at an early age. The relationships between dysthymic disorders and personality disorders and the response

Personality profiles of patients with dysthymic and panic disorder.

BACKGROUND: The research objective was to identify personality characteristics as well as similarities between, and differences in personality profiles of persons suffering from Dysthymic (DD) and Panic Disorder (with/without Agoraphobia) (PD/PDA). SUBJECTS AND METHODS: Three groups (N=120) were analysed: DD, PD/PDA, and a healthy control group, matched by socio-demographic characteristics and classified in sub-groups according to gender. Diagnoses were made using the Structured Clinical Interview for DSM-IV DD and PD/PDA, and the personality assessment was made using the Minnesota Multiphasi

Duration of periods of euthymia in patients with dysthymic disorder.

OBJECTIVE: The purpose of this study was to assess the duration of periods of euthymia in patients with dysthymia. METHOD: All patients with dysthymia who came to the Center for Anxiety and Depression over a 10-month period (N = 22) were interviewed by the author regarding the duration of their euthymic episodes. RESULTS: The 22 patients with dysthymia reported euthymic periods from 2 to 30 days (mean = 8.0 days, SD = 6.6). CONCLUSIONS: The euthymic period of up to 2 months that is specified in DSM-IV for dysthymic disorder might confound the results of clinical trials. Data from additional

Dysthymic disorder: clinical characteristics in relation to age at onset.

BACKGROUND: The variability in the clinical presentation of dysthymia has given rise to a rich debate in literature, and various hypotheses have been proposed. One is that the clinical presentation differs in relation to age at onset. The aim of the study was to evaluate differences in socio-demographic and clinical characteristics in a sample of patients with dysthymia (DSM-IV), in relation to age at onset. METHOD: 84 consecutive outpatients with a diagnosis of dysthymia (DSM-IV) were studied. All subjects were evaluated by a semistructured clinical interview and the following rating scales:
