Tremor and other hyperkinetic movements (New York, N.Y.)2013eng
BACKGROUND: Adult-onset focal dystonias (AOFDs) are non-task-specific or task-specific and may spread to other body segments of affected patients.
CASE REPORT: We report the case of a barber with non-task-specific craniocervical dystonia and a new occupational focal hand dystonia (while using scissors).
DISCUSSION: Different AOFDs may develop and coexist in the same "vulnerable" patient. Hairdresser's dystonia is a rare task-specific dystonia.

Early onset torsion dystonia (EOTD) is a rare movement disorder characterized by involuntary, repetitive, sustained muscle contractions or postures involving one or more sites of the body. A US study estimated the prevalence at approximately 1 in 30,000. The estimated prevalence in the general population of Europe seems to be lower, ranging from 1 in 330,000 to 1 in 200,000, although precise numbers are currently not available. The estimated prevalence in the Ashkenazi Jewish population is approximately five to ten times higher, due to a founder mutation. Symptoms of EOTD typically develop fir

Dystonia is a movement disorder characterized by sustained or intermittent muscle contractions causing abnormal, often repetitive, movements or postures. Currently, dystonia is defined as a network disorder involving the basal ganglia, cerebellum, thalamus, and sensorimotor cortex. Considering that it is refractory to medical therapy, functional neurosurgery is indispensable in the treatment strategy for dystonia. Functional neurosurgery may improve dystonic symptoms by suppressing abnormal neuronal activity in the motor loop network. Deep brain stimulation(DBS)of the globus pallidus internus(

This review presents the newest classification of dystonia, new evidence regarding aetiology, the diagnosis of dystonia, and the most common forms of treatment. Dystonia is a rare and heterogeneous movement disorder defined as a syn-drome of sustained muscle contractions with twisting and abnormal postures, and tremor. Dystonia is a collective term for different syndromes, primary idiopathic forms as well as genetic and secondary forms.

PURPOSE OF REVIEW: The purpose of this review is to provide an update on the classification, phenomenology, pathophysiology, and treatment of dystonia.
RECENT FINDINGS: A revised definition based on the main phenomenologic features of dystonia has recently been developed in an expert consensus approach. Classification is based on two main axes: clinical features and etiology. Currently, genes have been reported for 14 types of monogenic isolated and combined dystonia. Isolated dystonia (with dystonic tremor) can be caused by mutations in TOR1A (DYT1), TUBB4 (DYT4), THAP1 (DYT6), PRKRA (DYT16)

Focal dystonias such as writer's cramp or blepharospasm are treatable with botulinum toxin injections and medications, but both therapies provide largely symptomatic relief. Because the basic abnormality in dystonia is at the synaptic level, brain modulating therapies with repetitive transcranial magnetic stimulation (rTMS) may well be able to produce lasting clinical improvement. Low-frequency threshold or subthreshold rTMS over the premotor cortex or anterior cingulate cortex, for hand dystonia and blepharospasm, respectively, could in the future become a more curative treatment, perhaps in

Many different disorders have dystonia as the only or primary sign. The list of causes for dystonia increases yearly and now includes three mapped loci for primary torsion dystonia, although other susceptibility genes are suspected. Study of one of these primary torsion dystonia loci (DYT1) has culminated in the cloning of a gene which codes for a novel protein, torsin A. Physiological and positron emission tomography analyses suggest that dystonia results from impaired inhibition at cortical and subcortical levels; these physiological changes may in turn be due to striatal dysfunction and a m

OBJECTIVE: This article focuses on the epidemiology, diagnostic criteria, and clinical features of dystonia. The treatment of dystonia and controversies in the field are also addressed.
LATEST DEVELOPMENTS: The latest international classification is in development to further refine diagnostic criteria for dystonia. This emphasis on classification highlights the importance of accurately recognizing clinical signs in a disorder requiring a diagnosis without a clear biomarker. Accurate workup and diagnosis facilitate appropriate treatment. The field of dystonia therapy now includes both neurosti

Dystonia is a neurological condition characterized by abnormal involuntary movements or postures owing to sustained or intermittent muscle contractions. Dystonia can be the manifesting neurological sign of many disorders, either in isolation (isolated dystonia) or with additional signs (combined dystonia). The main focus of this Primer is forms of isolated dystonia of idiopathic or genetic aetiology. These disorders differ in manifestations and severity but can affect all age groups and lead to substantial disability and impaired quality of life. The discovery of genes underlying the mendelian

This chapter presents a review of the literature for falls and abnormalities of balance and gait in isolated dystonia syndromes. Balance appears to be relatively preserved in primary and focal dystonias, except when postural abnormalities significantly displace the center of gravity of the body. Gait can be affected by abnormal dystonic movements and limb deformities, and is commonly abnormal in generalized dystonia. Impairment of gait can also be seen in patients with cervical dystonia when it results in abnormal posturing, and severe cases of blepharospasm leading to functional blindness. Ga

Dystonia is defined as a syndrome of sustained involuntary muscle contractions, frequently causing twisting, repetitive movements and abnormal postures. Categorisation of the dystonia syndromes is variable and depends on etiology, age at onset, distribution of symptoms or genetic causes, reflecting the variability of the clinical spectrum of these diseases and the many underlying causes. The prevalence of the dystonias is largely unknown, as few epidemiological studies have been carried out and many of these have been service based rather than community based. This article reviews the phenomen

Therapy for most people with dystonia is symptomatic, directed at lessening the intensity of the dystonic contractions. For a small minority of patients (eg, those with dopa-responsive dystonia, Wilson's disease, or psychogenic dystonia), specific therapy directed at one of the many causes of dystonia is available. Before initiating treatment, clinicians need to decide if a patient has a form of dystonia amenable to such therapy. The most sensitive and least costly method to diagnose DRD is a therapeutic trial of levodopa. It is, therefore, recommended to treat all those with dystonia beginnin

Dystonia can be considered either as a symptom, or as a disease. An initial classification of dystonia can be made according to the localization and the severity of the spasms or the associated movement disorders such as myoclonus. A second classification differentiates idiopathic dystonia and secondary dystonia. Personnel medical history, familial cases, neurological symptoms such as pyramidal, cerebellar, oculomotor signs are helpful clues in the diagnosis strategy. Drugs, botulinum toxin, physiotherapy are often combined symptomatic treatment regardless of the cause of dystonia.

Dystonia has moved from the status of a rare disease or an uncommon clinical manifestation to the center stage of research in movement disorders. Recent developments have expanded our knowledge of the genetics and understanding of its pathophysiology, and have resulted in significant improvements in the clinical management of this group of disorders. These, as well as the varied and multifaceted phenomenology are reviewed.

Dystonia is a persistent attitude or posture in one or other of the extremes of athetoid movement. It may take the form of an over-extension or over-flexion of the hand, torsion of the spine, with arching and twisting of the back or forceful closure of the eyes and a fixed grimace. Dystonia is classified into idiopathic and symptomatic dystonia. Idiopathic dystonia is further divided into generalized, focal and segmental dystonia. Generalized dystonia covers classical torsion dystonia, paradoxical dystonia, myoclonic dystonia, dystonia with diurnal variation and Dopa-responsive dystonia. Dysto

To understand better the features and mechanisms distinguishing tremor in dystonia, we reviewed the epidemiologic, clinical, and neurophysiologic data in patients with dystonia and tremor. Clinical studies suggest that tremor starts at or after dystonia onset in body parts affected or unaffected by dystonia. Tremor in dystonia manifests during posture or voluntary movements even though some dystonic patients may have tremor at rest. Prevalence rates for tremor in dystonia are higher in patients with adult-onset dystonia and cervical dystonia than in other dystonias and highest in patients in w

INTRODUCTION: The genetic basis of autosomal-recessive dystonia remains poorly understood. Our objective was to report identification of additional individuals with variants in AOPEP, a recently described gene for recessively inherited dystonic disorders (OMIM:619565).
METHODS: Ongoing analysis on a high-throughput genetic platform and international case-recruitment efforts were undertaken.
RESULTS: Novel biallelic, likely pathogenic loss-of-function alleles were identified in two pedigrees of different ethnic background. Two members of a consanguineous Iranian family shared a homozygous c.1
