To eat or not to eat: A role for ghrelin and LEAP2 in eating disorders?

Ghrelin is an orexigenic hormone that orchestrates many diverse behaviours of relevance for feeding control. It appears to operate as a hunger hormone, organizing food intake into meals and ensuring that we seek out and consume a variety of foods. With this physiological biography, the ghrelin system, including the pathways through which it operates, has been interrogated for its role in the aetiology, pathology and treatment of eating disorders. While common obesity does not appear to be a hyperghrelinemic state, it would be difficult to completely rule out enhanced ghrelin signalling. At the

Dysfunctional eating in the eating disorders.

Aberrant eating patterns in the eating disorders have been observed across various laboratory-based and clinical studies. It is now clear that problems in experiencing and expressing hunger, appetite, and satiety in anorexia and bulimia nervosa are likely to perpetuate the disorders once established. Whether problems in appetite regulation are primary or secondary to the development of the disorders is unknown. In studies examining indices of appetite regulation after treatment, there still remain significant levels of eating abnormality. This suggests that the main goals of treatment, includi

Hoodia gordonii: to eat, or not to eat.

ETHNOPHARMACOLOGICAL RELEVANCE: Hoodia gordonii (family Apocynaceae) has become known globally for its claimed effect of appetite suppression. Despite a relatively large body of evidence of the plant׳s chemical make-up, peer-reviewed studies to provide scientific information on physiological effects of Hoodia gordonii are relatively sparse. The role of the pregnane glycoside P57-commonly accepted to be responsible for appetite suppression-has been questioned recently. Furthermore, a variety of physiological side-effects associated with consumption of the plant in extracted form questions its s

Clinical pharmacology of eating and not eating.

FDA approval of Belviq and Qsymia seems to suggest that novel pharmacological targets to modulate human abnormal eating behaviours are still to be identified. However, a renewed translational approach opens new avenues on eating disorders and female vulnerability, highlighting the role of our reward pathway in obesity and binge eating and leading to potential novel targets. Nevertheless, the 'food addiction' hypothesis is still causing much scientific debate. In this context the interest in the modulation of ghrelin pathway is still very high although, so far, only ghrelin agonism has confirme

[Eating behavior, eating disorders and obesity].

Over the last 50 years, the nutritional and socioeconomic conditions have dramatically changed in all industrialized countries. As a consequence, there has been a sharp rise in the prevalence of obesity. Simultaneously, social and cultural pressures to maintain a thin body shape have significantly increased. This untoward situation is largely responsible for the steady increase of eating disorders, especially bulimia nervosa and binge-eating disorder, which are common disorders among normal or overweight individuals. Although the criteria for bulimia nervosa were first described in the DSM-III

Are Eccentric Eating Habits Eating Disorders?

BACKGROUND: The study deals with particular kinds of eating habits that are unusual, not focused on weight, different from eating disorders, and not pathological. They are characterized by features such as the kind, amount, manner and style of eating that deviate from the common ones in their family or culture. They would be included today under the DSM- 5 categories of Avoidant restricting food intake disorder (AR-FID), and unspecified feeding or eating disorders (USFED). The question was whether they are mild forms of eating disorders or an independent set of behaviors. The objective was to

Emotional Eating, Binge Eating and Animal Models of Binge-Type Eating Disorders.

PURPOSE OF REVIEW: The objective of this paper is to review the role that hedonic factors, emotions and self-regulation systems have over eating behaviours from animal models to humans. RECENT FINDINGS: Evidence has been found to suggest that for some high-risk individuals, obesity/binge eating may develop as an impulsive reaction to negative emotions that over time becomes a compulsive habit. Animal models highlight the neural mechanisms that might underlie this process and suggest similarities with substance use disorders. Emotional difficulties and neurobiological factors have a role in th

Eating disorder or disordered eating: undiagnosed inflammatory bowel disease mimicking eating disorder.

We describe the case of a patient with ongoing weight loss, low mood and previously undisclosed gastrointestinal (GI) symptoms initially diagnosed with an eating disorder and subsequently diagnosed with ulcerative colitis over a year following initial presentation. This patient exhibited disordered eating secondary to the worsening symptoms of undiagnosed inflammatory bowel disease (IBD) and had altered her eating habits to reduce the diarrhoea and rectal bleeding she was experiencing, contributing to significant weight loss. The implications of a delayed diagnosis of IBD or incorrect diagnosis

Editorial: To Eat or Not to Eat: Advancing the Neuroscience of Hedonic Versus Controlled Eating Across Weight and Eating Disorders.

Excessive weight and obesity, especially with childhood onset, is associated with long-term morbidity and mortality and places a major burden on the health care system. In the United States, 17% of children and adolescents are obese (32% overweight). By adulthood, the number rises to 34% or even 68% when also considering overweight individuals.1 Conventional nonsurgical treatments are often ineffective, and weight loss achieved with behaviorally oriented therapy programs is usually small (∼5%) and short-lived.2 A better understanding of the associated psychological mechanisms and their neurobi

Implicit attitudes toward eating stimuli differentiate eating disorder and non-eating disorder groups and predict eating disorder behaviors.

OBJECTIVE: The current study tested whether people with and without eating disorders (EDs) varied in their implicit attitudes toward ED-relevant stimuli. Additionally, the study tested whether implicit evaluations of ED-relevant stimuli predicted ED symptoms and behaviors over a 4-week interval. METHOD: Participants were people without EDs (N = 85) and people seeking treatment for EDs (N = 92). All participants completed self-report questionnaires and a version of the affect misattribution procedure (AMP) at baseline. The AMP indexed implicit evaluations of average body stimuli, eating stimul

[Eating disorders: anorexia nervosa, bulimia, binge eating].

We present an overview of three different eating disorders which seem to have an increasing prevalence, especially among young women between 12 and 20 years of age. Anorexia and bulimia nervosa are "threshold disorders" which usually become manifest for the first time during the transition from childhood to early adult life. Eating disorders are chronic disorders and often take a course of 6 or more years. Remission, improvements and symptom change can be expected even after many years of the disorder. Psychiatric comorbidity which occurs in more than 50% of eating disordered patients is of pr

[Nocturnal eating disorder--sleep or eating disorder?].

Nocturnal eating disorder (NED) is a rare syndrome that includes disorders of both eating and sleeping. It is characterized by awakening in the middle of the night, getting out of bed, and consuming large quantities of food quickly and uncontrollably, then returning to sleep. This may occur several times during the night. Some patients are fully conscious during their nocturnal eating, while some indicate total amnesia. The etiology of NED is still unclear, as research findings are contradictory. Those suffering from NED exhibit various levels of anxiety and depression, and many lead stressful

The importance of eating behavior in eating disorders.

A disturbance in eating behavior is the defining characteristic of the clinical eating disorders, Anorexia Nervosa, Bulimia Nervosa, and Binge Eating Disorder. Surprisingly little research has been devoted to assessing objectively the nature of the eating disturbances in these disorders, to elucidating what factors contribute to the development and persistence of these disturbances, or to describing how they change with treatment. This review, which is based on a Mars lecture delivered at the 2010 meeting of the Society for the Study of Ingestive Behavior, reviews objective information about t

Binge eating disorder and night eating syndrome: a comparative study of disordered eating.

The authors compared eating patterns, disordered eating, features of eating disorders, and depressive symptoms in persons with binge eating disorder (BED; n = 177), with night eating syndrome (NES; n = 68), and in an overweight comparison group without BED or NES (comparison; n = 45). Participants completed semistructured interviews and several established measures. Depressive symptoms were greater in the BED and NES groups than in the comparison group. NES participants ate fewer meals during the day and more during the night than BED and comparison participants, whereas BED participants ate m

Food addiction, eating addiction and eating disorders.

The concept of food addiction is currently a highly debated subject within both the general public and the scientific communities. The term food addiction suggests that individuals may experience addictive-like responses to food, similar to those seen with classic substances of abuse. An increasing number of studies have established the prevalence and correlates of food addiction. Moreover, food addiction may be associated with obesity and disordered eating. Thus, intervening on food addiction may be helpful in the prevention and therapy of obesity and eating disorders. However, controversy ex

[Eating psychopathology and personality in eating disorders].

OBJECTIVE: The question of how many psychopathologic factors are involved in Anorexia Nervosa (AN) and Bulimia Nervosa (BN) has no definite answer. The combination of psychopathology and personality research may shed a light upon the determinants of eating pathology. METHODS: The study consists in the administration of the Temperament and Character Inventory (TCI) and the Eating Disorder Inventory 2 (EDI-2) to 95 outpatient anorectic women (50 restrictive and 45 binge-purging) and to 92 bulimic outpatient women (78 with and 14 without purging behaviours). Symptoms of Psychopathology Within Groups of Eating-Disordered, Restrained Eating, and Unrestrained Eating Individuals.

OBJECTIVE: While eating-disordered individuals have shown high levels of comorbid psychopathology, there has not been an assessment of these symptoms across groups exhibiting different forms of problematic eating behavior. METHOD: Using 1,122 participants recruited via Amazon Mechanical Turk, this study examined self-reported differences between controls, restrained eaters, and individuals meeting criteria for binge eating disorder and bulimia nervosa on several measures of psychopathology unrelated to eating. RESULTS: On nearly all outcome measures, eating-disordered participants had greate
