[Arthroscopy of the elbow joint].

Clear indications have been recognised for arthroscopic surgery of the elbow since the 1980s. Good indications are loose bodies, mild or moderate restriction of the range of movement and early stages of rheumatoid arthritis. Cartilage diseases such as Panner disease or focal radial chondropathy can be treated by arthroscopic debridement and microfracturing. Impingement syndromes affecting the lateral compartment of the joint respond well to resection of synovial plicae. The technical demands of endoscopic arthrolysis procedures for joints with mild or moderate restriction of range of movement

[Dislocations of the elbow joint].

In a collective follow-up study the results of conservative and operative treatment of luxations of the elbow joint were assessed. In all, 433 patients were followed-up; 315 of them had a ligamentous injury, 225 flake fractures. Because of the amount of soft and bone tissue damage, 241 patients were treated operatively. The functional results of these operations were good in 90% and comparable to those in patients receiving conservative treatment. This confirms that operative treatment of elbow luxations has to be recommended in cases with considerable ligamentous and bone damage.

[Fractures of the elbow joint].

Fractures around the elbow joint comprise fractures of the distal humerus, the radial head, the olecranon and the coronoid process. Combined lesions are particularly demanding for the surgeon. Accurate knowledge of the anatomy and of the biomechanics is an essential requirement for a specific diagnosis and therapy. A stable and painless movable elbow joint is essential for most of the activities of daily living. Risk factors for the development of posttraumatic elbow joint arthrosis are non-anatomically reconstructed joint surfaces, axial malalignment of the joint axis and untreated concomitan

[Arthroscopy of the elbow joint].

PURPOSE OF THE STUDY: The authors evaluate a group of patients who underwent elbow arthroscopy in the period between August 2001 and August 2003. They pay attention to indications, the technique used, surgical diagnoses, postoperative outcomes and complications. MATERIAL: We performed 28 elbow arthroscopies in 26 patients; two patients were operated on both elbow joints. The group comprised 20 men and 6 women between 14 to 73 years, with an average of 44 years. The average follow-up was 18 months (range, 4-28 months). METHODS: We usually operated on patients in a prone position under general

[Arthroscopy of the elbow joint].

A series of 31 patients who had elbow arthroscopy has been reviewed. Good results have been achieved after removal of loose bodies or post-traumatic hematoma. A danger of injury to periarticular structures due to the use of arthroscopic instruments, especially with pathologically decreased joint volume has been pointed out.

[The infected elbow joint fracture].

Post-traumatic infection of the elbow joint is a serious complication, because the bone structure and the thin soft tissue cover make all actions directed at reconstruction and at calming the infection difficult. A good overall prognosis for later function depends decisively on early diagnosis, stability of the fragments and the earliest possible initiation of functional therapy. If this cannot be achieved, the infection must first be treated by transfixation techniques and the joint function restored subsequently. In late cases with chronic inflammation and destruction, nothing but resection

Elbow joint biomechanics for preclinical evaluation of total elbow prostheses.

Total elbow arthroplasty is a clinically successful procedure, yet long-term implant survival rates have historically lagged behind those reported for total hips and knees. Clinical complications associated with implant wear, osteolysis, stem loosening and device fracture have been implicated as reasons for limited long-term survivorship. Unfortunately, there is little published information on the biomechanics and method(s) for preclinical evaluation of total elbow prostheses that could provide insight into the mechanisms of failure. Additionally, there are no consensus testing standards or su

[Arthroscopic arthrolysis of the elbow joint].

OBJECTIVE: Arthroscopic assisted improvement of range of motion in elbow stiffness. Detailed diagnostic evaluation including medical history and preoperative radiographs, CT and MRI scans are necessary for planning the operative treatment. INDICATIONS: Restricted range of motion < 30° in extension and/or more than 100° in flexion related to intraarticular causes (loose bodies, osteophytes or contracture of the capsule). CONTRAINDICATIONS: Extension deficit > 30°, extraarticular causes (e.g., heterotopic ossifications), nerve irritation, incongruity of joint surfaces, acute joint infection.

Tuberculous synovitis of the elbow joint.

Tuberculous synovitis in the elbow joint is extremely rare in developed countries. We describe a 68-year-old man who had had a gradually enlarging mass over the volar side of the left proximal forearm near the elbow joint for 4 months. Plain roentgenograms of the diseased elbow showed early osteoarthritic change. Magnetic resonance imaging revealed diffuse synovitis with a large 8 x 8 cm extra-articular synovial cyst. Synovectomy was performed and histopathologic examination of the surgical specimen revealed granulomatous inflammation with caseation, prominent Langhan's giant cells, and sparse

Tuberculous tenosynovitis in the elbow joint.

A 74-year-old woman was noted to have a mass lesion near the right elbow joint during medication for pulmonary tuberculosis. After discontinuation of medication, the mass gradually became enlarged with swelling and tenderness of the joint. Radiological evaluation disclosed tenosynovitis with an encapsulated abscess. Microscopic examination and culture of an aspiration biopsy specimen from the abscess showed no microorganisms. However, DNA extracted from the specimen contained mycobacterium tuberculosis DNA, permitting a diagnosis of tuberculous tenosynovitis. Mycobacterium is not always detect

[Complex injury of the elbow joint].

High-velocity trauma now often results in complex injuries to the upper extremity, and especially the elbow joint. These can lead to both an enormous reduction in the range of motion of the shoulder, elbow and wrist joints, in severe cases with complete loss of upper extremity function. A complex injury is defined as a fracture and/or dislocation of the elbow in association with (1) a serial injury of the upper extremity, (2) a severe soft tissue trauma, or (3) concomitant injury to vessels or nerves. Serial fractures, in particular can lead to enormous problems with treatment and are often as

[Arthroscopy of the wrist and elbow joint].

Arthroscopy of the wrist and elbow is a helpful diagnostic procedure in patients with chronic disability not demonstrated by clinical and radiological examinations. At the wrist, especially triangular fibrocartilage, chondral or ligamentous defects can often not be demonstrated by other tests. Arthroscopy of the wrist allows examination of more than a dozen separate articular surfaces and the function of eight discrete joints. The geometry of the elbow joint limits intra-articular inspection and the use of instruments. The risk of neurovascular injuries is great and depends on exact puncture s

[CT diagnosis of the elbow joint].

The normal anatomy of the elbow joint is described. Diverse pathologic joint alterations, e.g. fractures and dislocations, osteochondritis, inflammatory changes, and ulnar nerve lesion are demonstrated. Particular attention is paid to three-dimensional (3D-) reconstruction of demonstrable pathologic findings.

[Open fractures of the elbow joint].

Both in open and in closed elbow fractures, anatomical joint reconstruction and rigid internal fixation, which permit early initiation of motion, are essential for good joint function. However, in open distal intraarticular humerus fractures this is more difficult to achieve, owing to the high rate of comminuted types C 2 and C 3 fractures. In addition, the extensive soft tissue lesions often mean that physiotherapy cannot be started until later than would otherwise be desirable. This retrospective study shows that the functional results in 60 patients operated on for correction of open distal

[Computerized tomography of the elbow joint].

An essential pre-condition for the interpretation of computer tomographs of the elbow joint is a knowledge of the normal anatomy. The complexity of the joint requires a definite protocol for the examination, since only in this way is it possible to distinguish all anatomical structures and pathological processes. A clinical example is used to show that CT of the elbow joint is superior to all other radiographic methods for demonstrating osteochondritis dissecans. It is therefore an important additional method for demonstrating the elbow joint.
