Accuracy of an electronic prescribing system for standard treatments.

BACKGROUND: The prescription system known as RELE is the electronic prescribing and dispensing system that has been used by the Balearic Islands Health Service (IB-SALUT, Spain) since April 2006. This repository contains patients' computerised medication profiles (CMPs). Data from studies conducted in other settings have shown the existence of discrepancies between the medications recorded in patients' CMPs and the medications actually taken by the patients. METHODS: Chronic medications recorded in RELE CMPs were compared with those obtained through a structured clinical interview with patien

Electronic Prescribing in the Neonatal Intensive Care Unit: Analysis of Prescribing Errors and Risk Factors.

Patients admitted to neonatal intensive care units are up to eight times more likely to experience medication errors than patients admitted to adult intensive care units. Prescribing errors account for up to 74% of medication errors. Electronic prescribing has been postulated as a tool to reduce errors. The objective was to analyse prescribing errors with the e-prescribing system and risk factors. All patients who were admitted for at least 24 h and who received active pharmacological treatment during the study period were included. Prescriptions were made using electronic assisted prescriptio

Electronic prescribing: Implementation of a new approach to undergraduate teaching and assessment.

INTRODUCTION: Electronic prescribing and medicines administration (ePMA) systems are increasingly used across UK healthcare to reduce medication errors and improve patient safety. Undergraduate medical curricula have traditionally relied on handwritten prescribing, creating misalignment between education and clinical practice. To address this, the University of Manchester introduced electronic prescribing using the British Pharmacological Society (BPS) platform across its undergraduate medical programme. METHODS: This descriptive implementation (quality improvement) report outlines a curricul

Electronic health record interventions to reduce postoperative pregabalin prescribing: a quality improvement initiative.

PURPOSE: Perioperative gabapentinoids may not provide meaningful analgesia and can have significant adverse events. Our objective was to estimate the association of two electronic health record (EHR) interventions with pregabalin prescribing by the acute pain service (APS) at a multi-site academic health sciences network. METHODS: We conducted a quality improvement initiative using a retrospective observational cohort and a quasi-experimental interrupted time series design. Following a baseline period (19 January 2021-19 January 2022), we introduced a Best Practice Advisory that warned of pre

PRescribing Interventions for Chronic pain using the Electronic health record (PRINCE): Study protocol.

BACKGROUND: Primary care is a frequent source of pain treatment and opioid prescribing. The objective of the Prescribing Interventions for Chronic Pain using the Electronic health record (PRINCE) study is to assess the effects of two behavioral economics-informed interventions embedded within the electronic health record (EHR) on guideline-concordant pain treatment and opioid prescribing decisions in primary care settings. METHODS: Setting: The setting for this study is 43 primary care clinics in Minnesota. DESIGN: The PRINCE study uses a cluster-randomized 2 × 2 factorial design to test the

The Value of Electronically Extracted Data for Auditing Outpatient Antimicrobial Prescribing.

OBJECTIVE The optimal approach to auditing outpatient antimicrobial prescribing has not been established. We assessed how different types of electronic data-including prescriptions, patient-visits, and International Classification of Disease, Tenth Revision (ICD-10) codes-could inform automated antimicrobial audits. DESIGN Outpatient visits during 2016 were retrospectively reviewed, including chart abstraction, if an antimicrobial was prescribed (cohort 1) or if the visit was associated with an infection-related ICD-10 code (cohort 2). Findings from cohorts 1 and 2 were compared. SETTING Prima

Identification of antibiotic allergies in children using prescribing patterns in the electronic medical record.

BACKGROUND: By age three, 5%-75% of children have a documented antibiotic allergy (AL). Despite this prevalence, recognition remains low because of limited clinician knowledge and time. Our study evaluates a novel approach to identifying AL using electronic medical record (EMR) prescription patterns. METHODS: An EMR report was developed to identify paediatric encounters where adrenaline and/or an antihistamine was administered within 48 h of the last antibiotic dose. The first 100 cases were reviewed to refine exclusion criteria. Exclusions included diagnoses of eczema, or rash; antihistamine

Impact of electronic stewardship interventions on antibiotic prescribing for asymptomatic bacteriuria: A multicenter observational study.

BACKGROUND: Inappropriate antibiotic treatment of asymptomatic bacteriuria (ASB) remains common in hospitalized patients. This study evaluated the impact of 2 electronic stewardship interventions across 5 hospitals. METHODS: This multicenter, retrospective study included adult patients (≥ 18 years old) admitted between January 2020 and January 2026 with confirmed ASB. Patients were excluded for death within 48 hours, neutropenia, receipt of antibiotics for an active infection, or urologic procedure with mucosal involvement. Association of Lowering Default Pill Counts in Electronic Medical Record Systems With Postoperative Opioid Prescribing.

IMPORTANCE: Reliance on prescription opioids for postprocedural analgesia has contributed to the opioid epidemic. With the implementation of electronic medical record (EMR) systems, there has been increasing use of computerized order entry systems for medication prescriptions, which is now more common than handwritten prescriptions. The EMR can autopopulate a default number of pills prescribed, and 1 potential method to alter prescriber behavior is to change the default number presented via the EMR system. OBJECTIVE: To investigate the association of lowering the default number of pills prese

Usability testing of an electronic patient-reported outcome system linked to an electronic chemotherapy prescribing and patient management system for patients with cancer.

BackgroundPeople affected by cancer experience a wide range of symptoms which have a major impact on their functioning and health-related quality of life (HRQoL). One way to measure the impact of cancer symptoms is through the use of patient-reported outcomes.MethodsAn electronic patient-reported outcome (ePRO) application (ChemoPRO®) was designed to be used by cancer patients to report their symptoms and communicate with their clinical team. Usability testing sessions were conducted with people with lived experience of cancer to understand how real users interact with the ChemoPRO® system. On

Improving antimicrobial prescribing in Irish primary care through electronic data collection and surveillance: a feasibility study.

BACKGROUND: The increase in the spread of antimicrobial resistance (AMR) in bacterial pathogens and limited availability of new antimicrobials places immense pressure on general practitioners (GPs) to prescribe appropriately. Currently, electronic antimicrobial prescribing data is not routinely collected from GPs in Ireland for surveillance purposes to assess regional specific fluctuations or trends in antimicrobial prescribing. The current study aimed to address this issue by assessing the feasibility of remotely extracting antimicrobial prescribing data from primary care practices in Ireland

Towards a prescribing monitoring system for medication safety evaluation within electronic health records: a scoping review.

BACKGROUND: Medical care can fail for various reasons: diseases can remain undetected and their severity misjudged, therapies can be incorrectly dosed or ineffective, and therapies can trigger new conditions or adverse drug reactions (ADR). To manage the complexity of changing patient circumstances, data-driven techniques play an increasingly important role in monitoring patient safety and treatment success. Therefore, clinical prediction models need to consider longitudinal factors ("Prescribing Monitoring") to ensure clinically meaningful results and avoid misclassification in the dynamic he

Trends in consultations and prescribing for rheumatic and musculoskeletal diseases: an electronic primary care records study.

BACKGROUND: Rheumatic and musculoskeletal diseases (RMDs) are common and generally managed in primary care through supported self-care, physiotherapy, analgesia, and specialist referral where indicated. The COVID-19 pandemic led to abrupt changes in primary care delivery, including moves to remote consulting, pauses on group-based self-care, and restricted referrals. AIM: To describe how patterns of UK primary healthcare consultations and analgesic prescribing relating to RMDs changed during the COVID-19 pandemic. DESIGN AND SETTING: Observational study using routinely collected national pri

Implementing prescriber-pharmacist collaboration to improve evidence-based medication prescribing using asynchronous, non-interruptive electronic health record notifications.

BACKGROUND: Inappropriate prescribing of Direct Oral Anticoagulants (DOACs) is a leading cause of adverse outcomes. Electronic health record (EHR)-based notification strategies may support evidence-based prescribing and reduce adverse events. Engaging clinical pharmacists (vs. prescribers) through EHR-based notifications that review inappropriate DOAC prescribing may be an effective strategy for ensuring evidence-based medication prescribing. METHODS: We conducted a pragmatic, single-center, parallel-group, randomized implementation trial using notifications (asynchronous EHR-based notificati

Analgesic prescribing in patients with inflammatory arthritis in England: an observational study using electronic healthcare record data.

OBJECTIVES: International data suggest inflammatory arthritis (IA) pain management frequently involves opioid prescribing, despite little evidence of efficacy, and potential harms. We evaluated analgesic prescribing in English National Health Service-managed patients with IA. METHODS: Repeated cross-sectional analyses in the Consultations in Primary Care Archive (primary care consultation and prescription data in nine general practices from 2000 to 2015) evaluated the annual prevalence of analgesic prescriptions in: (i) IA cases (RA, PsA or axial spondyloarthritis [SpA]), and (ii) up to five

Prevalence of healthcare device-associated infection using point prevalence surveys of antimicrobial prescribing and existing electronic data.

This study extended a previously described method for the prevalence of healthcare-associated infection, based on point prevalence surveys of antimicrobial prescribing and electronic data, to estimate the prevalence of device-associated infections. In June 2009, the six-month point prevalence survey of antimicrobial prescribing was carried out in accordance with the European Surveillance of Antimicrobial Consumption Protocol. For patients receiving antimicrobials the presence of devices was recorded. A census on device use was carried out concurrently in the relevant hospitals. We selected pat

A comprehensive study of prescribing, administering and drug handling medication errors in ten wards of a university hospital after implementation of electronic prescribing, clinical pharmacists or medication reconciliation.

Background and aim: Medication errors lead to preventable risks. Preventing strategies such as e-prescribing, clinical pharmacists and medication reconciliation have been implemented in recent years. However, information on long-term medication error rates in routine procedures is missing. Investigations: We aimed to identify predefined medication errors in ten wards of a university hospital where e-prescribing, clinical pharmacists and medication reconciliation have been partially implemented. Patient files were reviewed and routine processes were monitored for drug prescription errors (missi
