Difficult calls to emergency medical dispatch centres - a mixed method study.

BACKGROUND: At emergency medical dispatch centres (EMDCs) telephone triage takes place in three steps: identifying the event, assessing the caller’s need for support, and prioritizing the response. Some calls are considered to be more difficult to handle than others, and decision support systems may in these situations be of limited help. The aim of this study was to describe and characterize difficult calls to EMDCs. METHODS: Retrospective call data from 2022 to 2023 was extracted for Registered Nurse (RN) dispatchers at three EMDCs in Sweden agreeing to participate in this mixed-method study

Discovering Key Topics in Emergency Medical Dispatch from Free Text Dispatcher Observations.

The objective of this work was to discover key topics latent in free text dispatcher observations registered during emergency medical calls. We used a total of 1374931 independent retrospective cases from the Valencian emergency medical dispatch service in Spain, from 2014 to 2019. Text fields were preprocessed to reduce vocabulary size and filter noise, removing accent and punctuation marks, along with uninformative and infrequent words. Key topics were inferred from the multinomial probabilities over words conditioned on each topic from a Latent Dirichlet Allocation model, trained following

Adverse drug reactions in an emergency medical dispatching centre.

PURPOSE: The purpose of this study is to assess the incidence of adverse drug reactions (ADR) leading to call an emergency medical dispatching centre. METHODS: A prospective, observational, monocentric clinical study performed over a 2-year period (2011-2012) in a French prehospital emergency dispatching centre, the Service d'Aide Médicale Urgente (SAMU) covering 1,156,000 inhabitants. All adult patients (age≥18) who called for any cause were included. We created an electronic trigger 'iatrogenic event' implemented by the dispatching physician for each suspected case of ADR, then we completed

Emergency medical dispatchers' experiences of using the Medical Priority Dispatch System telephone triage to identify maternity emergencies: a qualitative focus group study.

OBJECTIVES: The ambulance service plays a pivotal role in the provision of care in out-of-hospital maternity emergencies. Telephone triage of this patient group is complex and must be sensitive to an emergency situation to prevent unnecessary delays in treatment. This study aimed to explore emergency medical dispatchers' (EMDs) perceptions of the structured protocol they use. DESIGN: Voluntary participation in semistructured phenomenological focus groups. The participants were asked to discuss their experiences of using Medical Priority Dispatch System Protocol 24 (pregnancy, childbirth and m

Does emergency medical dispatch priority predict delphi process-derived levels of prehospital intervention?

OBJECTIVE: The Medical Priority Dispatch System (MPDS) is an emergency medical dispatch system widely used to prioritize 9-1-1 calls and optimize resource allocation. This study evaluates whether the assigned priority predicts a Delphi process-derived level of prehospital intervention in each emergency medical dispatch category. METHODS: All patients given a MPDS priority in a suburban California county from 2004-2006 were included. A Delphi process of emergency medical services (EMS) professionals in another system developed the following categories of prehospital treatment representing incr

Which emergency medical dispatch codes predict high prehospital nontransport rates in an urban community?

BACKGROUND: The Medical Priority Dispatch System (MPDS) is a commonly used computer-based emergency medical dispatch (EMD) system that is widely used to prioritize 9-1-1 calls and optimize resource allocation. There are five major priority classes used to dispatch 9-1-1 calls in the San Francisco System; Alpha codes are the lowest priority (lowest expected acuity) and Echo are the highest priority. OBJECTIVE: We sought to determine which MPDS dispatch codes are associated with high prehospital nontransport rates (NTRs). METHODS: All unique MPDS call categories from 2009 in a highly urbanized

Predictive Ability of an Emergency Medical Dispatch Stroke Diagnostic Tool in Identifying Hospital-Confirmed Strokes.

BACKGROUND: Early hospital notification of a possible stroke arriving via emergency medical services (EMS) can prepare stroke center personnel for timely treatment, especially timely administration of tissue plasminogen activator. Stroke center notification from the emergency dispatch center-before responders reach the scene-may promote even earlier and faster system activation, meaning that stroke center teams may be ready to receive patients as soon as the ambulance arrives. This study evaluates the use of a Medical Priority Dispatch System (MPDS; Priority Dispatch Corp., Salt Lake City, UT)

From Emergency Call to Emergency Department Handover: Emergency Medical Dispatch Support for Burn Injury Telephone Triage, Pre-Arrival Instructions and Referral Decisions.

BACKGROUND: Initial burn management often starts at the emergency call. Emergency medical dispatch (EMD) must identify high-risk features, initiate evidence-based first aid, and support safe referral decisions despite uncertainty in burn size and depth. STUDY OBJECTIVE: To synthesize evidence and standards relevant to dispatcher-assisted burn telephone triage, pre-arrival instructions, and destination/referral decisions, and to propose an implementable dispatch-to-emergency department (ED) framework. METHODS: We performed a structured clinical (narrative) review informed by SANRA and complem

Medical dispatchers recognise substantial amount of acute stroke during emergency calls.

BACKGROUND: Immediate recognition of stroke symptoms is crucial to ensure timely access to revascularisation therapy. Medical dispatchers ensure fast admission to stroke facilities by prioritising the appropriate medical response. Data on medical dispatchers' ability to recognise symptoms of acute stroke are therefore critical in organising emergency stroke care. We aimed to describe the sensitivity and positive predictive value of medical dispatchers' ability to recognise acute stroke during emergency calls, and to identify factors associated with recognition. METHODS: This was an observatio

Prehospital Aspirin Delivery: Emergency Medical Dispatcher-Directed versus Emergency Medical Services Field Provider-Directed Aspirin Administration.

INTRODUCTION: For suspected acute myocardial infarction (AMI) and unstable angina patients, prehospital aspirin (ASA) administration has been the standard of care by Emergency Medical Services (EMS) field providers. Recently, Emergency Medical Dispatchers (EMDs), using Medical Priority Dispatch System (MPDS), provide telephone instructions to qualifying suspected AMI patients to take ASA, prior to EMS field provider arrival. No formal studies exist that measure time saved from earlier Dispatcher-Directed Aspirin Administration (DDAA). OBJECTIVES: The primary objectives of the study were: (1)

Clinical evaluation of the Emergency Medical Services (EMS) ambulance dispatch-based syndromic surveillance system, New York City.

Since 1998, the New York City Department of Health has used New York City Emergency Medical Services (EMS) ambulance dispatch data to monitor for a communitywide rise in influenzalike illness (ILI) as an early detection system for bioterrorism. A clinical validation study was conducted during peak influenza season at six New York City emergency departments (EDs) to compare patients with ILI brought in by ambulance with other patients to examine potential biases associated with ambulance dispatch-based surveillance. We also examined the utility of 4 EMS call types (selected from 52) for case de

Factors affecting emergency medical dispatchers decision making in stroke calls - a qualitative study.

OBJECTIVES: Emergency Medical Communication Centers (EMCC) have a key role in the prehospital chain-of-stroke-survival by recognizing stroke patients and reducing prehospital delay. However, studies on EMCC stroke recognition report both substantial undertriage and overtriage. Since mis-triage at the EMCC challenges the whole chain-of-stroke-survival, by occupying limited resources for non-stroke patients or failing to recognize the true stroke patients, there is a need to achieve a more comprehensive understanding of the dispatchers' routines and experiences. The aim of this study was to expl

Characteristics of helicopter emergency medical services (HEMS) dispatch cancellations during a six-year period in a Dutch HEMS region.

BACKGROUND: For decades, Helicopter Emergency Medical Services (HEMS) contribute greatly to prehospital patient care by performing advanced medical interventions on-scene. Unnecessary dispatches, resulting in cancellations, cause these vital resources to be temporarily unavailable and generate additional costs. A previous study showed a cancellation rate of 43.5% in our trauma region. However, little recent data about cancellation rates and reasons exist, despite revision of dispatch protocols. This study examines the current cancellation rate in our trauma region over a six-year period. Addit

Second victims among emergency medical dispatchers in Germany: a cross-sectional study (SeViD-VII).

BACKGROUND: Emergency Medical Dispatchers (EMDs) play a critical role in coordinating emergency responses while being remotely exposed to distressing incidents. The Second Victim Phenomenon (SVP) refers to emotional distress experienced by healthcare professionals following adverse events; however, its prevalence and impact among EMDs remain poorly understood. This study examines the prevalence, symptom burden, and support preferences related to SVP among EMDs in Germany. METHODS: A cross-sectional survey was conducted using the validated SeViD-questionnaire, supplemented with demographic item
