A community-based ambulance model: lessons for emergency medical services and everyday health systems resilience from South Africa.

The role of the emergency medical service (EMS) is changing globally as ambulance crews respond to a shifting burden of disease, as well as societal stressors such as violence and inequality. New ways of thinking about how to provide emergency care are required to shift EMS from a role primarily focused on clinical care and transporting patients to hospital. In this paper, we present the experience of the Philippi Project (PP), an innovative community-based model of care developed by front line ambulance crews in a low-income neighbourhood in Cape Town, South Africa. Our insights were develope

Where Trouble Starts: Communication Breakdown in a Complex Emergency Call.

Emergency calls require efficient communication between caller and call taker to establish a need for assistance and dispatch help quickly. Analyzing communication processes at this first link in the emergency medical care chain has important implications for improving the quality of emergency care across the health system. This paper examines an interaction between a call taker and a caller requesting assistance at the scene of a family murder, using a hybrid interactional sociolinguistic approach to analysis. We also draw from court testimony. We demonstrate how several factors contribute to

Discovering Key Topics in Emergency Medical Dispatch from Free Text Dispatcher Observations.

The objective of this work was to discover key topics latent in free text dispatcher observations registered during emergency medical calls. We used a total of 1374931 independent retrospective cases from the Valencian emergency medical dispatch service in Spain, from 2014 to 2019. Text fields were preprocessed to reduce vocabulary size and filter noise, removing accent and punctuation marks, along with uninformative and infrequent words. Key topics were inferred from the multinomial probabilities over words conditioned on each topic from a Latent Dirichlet Allocation model, trained following

Cortical symptoms described in emergency calls for patients with suspected large vessel occlusion: a descriptive analysis of 157 emergency calls.

BACKGROUND: Emergency medical dispatchers typically use the dispatch code for suspected stroke when the caller brings up one or more symptoms from the face-arm-speech triad. Paramedics and emergency department physicians are trained to suspect large vessel occlusion stroke when the stroke patient presents with hemiparesis and cortical symptoms: neglect, aphasia, and conjugate eye deviation (CED). We hypothesized that these symptoms could be evident in the emergency call. In this study, we aimed to describe common symptoms mentioned in the emergency calls for paramedic-suspected thrombectomy ca

["Round the clock"--an information service for malignant hyperthermia emergencies].

A 24-hour 7-day telephone service has been created for emergency consultation: MH-hotline 030/3035504 (daytime) or 030/30351 (after office hours). In emergencies callers should ask for a consultant, indicating the code word "malignant hyperthermia" and give their name, the name of the institution and the telephone number.

Medical coverage of recreational cycling events.

Our goal is to optimize the level of medical care delivered to riders on the tour in a comprehensive way. Participation of local EMS squads and advance planning is essential for providing acute, prompt medical care at the scene and to provide transport to local or distant hospitals to evaluate or treat more serious conditions. Fortunately the majority of injuries is minor, but when EMS personnel and physicians work closely together, riders and tour staff feel more comfortable with concerns about their health during the event. Based on our experience, we have made suggestions for preventing inj

[Organization of emergency health services in disasters].

Since 1982 the St. Radboud hospital, Nijmegen, has a contiguity plan for peace-time disasters which describes the way to deal with great numbers of patients of traffic-, environmental- or industrial-calamities. The function of the commander, the separate hospital services, the medical and nursing triage team and administrative services are clearly delineated. Also is indicated how to organise communication between hospital, municipality, police and fire-brigade. The plan also provides possibilities for information towards press and relatives of the victims.

Prehospital Workplace Violence (WPV) Dispatch Alerts: Anticipating Behavior and Preventing Violence.

OBJECTIVES: Within our emergency medical services (EMS) agency, workplace violence (WPV) is captured through a documentation feature in the electronic medical record. Leveraging this data, we implemented WPV dispatch alerts for addresses where physical violence occurred. Our primary objective was to assess the association of these alerts on the rate of WPV against EMS clinicians. METHODS: This observational cohort study took place 11/20/2022-11/20/2024 at a hospital-affiliated EMS agency with 23,300 average annual ground calls for service. Alerts were implemented on 12/26/2023 and consisted o

Emergency medical dispatchers' experiences of using the Medical Priority Dispatch System telephone triage to identify maternity emergencies: a qualitative focus group study.

OBJECTIVES: The ambulance service plays a pivotal role in the provision of care in out-of-hospital maternity emergencies. Telephone triage of this patient group is complex and must be sensitive to an emergency situation to prevent unnecessary delays in treatment. This study aimed to explore emergency medical dispatchers' (EMDs) perceptions of the structured protocol they use. DESIGN: Voluntary participation in semistructured phenomenological focus groups. The participants were asked to discuss their experiences of using Medical Priority Dispatch System Protocol 24 (pregnancy, childbirth and m

From Emergency Call to Emergency Department Handover: Emergency Medical Dispatch Support for Burn Injury Telephone Triage, Pre-Arrival Instructions and Referral Decisions.

BACKGROUND: Initial burn management often starts at the emergency call. Emergency medical dispatch (EMD) must identify high-risk features, initiate evidence-based first aid, and support safe referral decisions despite uncertainty in burn size and depth. STUDY OBJECTIVE: To synthesize evidence and standards relevant to dispatcher-assisted burn telephone triage, pre-arrival instructions, and destination/referral decisions, and to propose an implementable dispatch-to-emergency department (ED) framework. METHODS: We performed a structured clinical (narrative) review informed by SANRA and complem

Breaking down barriers: Call-taker strategies to address caller perception of inappropriateness of cardiopulmonary resuscitation during the emergency ambulance call.

BACKGROUND: Ambulance call-takers perform the critical role of prompting callers to initiate and continue cardiopulmonary resuscitation (CPR) for patients with suspected out-of-hospital cardiac arrest (OHCA). This study aimed to identify call-taker strategies to address callers' perceptions of CPR 'inappropriateness' (perceiving the patient as dead and beyond help, or as showing signs of life). METHODS: Using a linguistic approach, we analysed 31 calls previously identified as having an inappropriateness barrier to CPR initiation or continuation. In Phase 1, we listened to call recordings and

System implications of the ambulance arrival-to-patient contact interval on response interval compliance.

BACKGROUND: In some emergency medical services (EMS) system designs, response time intervals are mandated with monetary penalties for noncompliance. These times are set with the goal of providing rapid, definitive patient care. The time interval of vehicle at scene-to-patient access (VSPA) has been measured, but its effect on response time interval compliance has not been determined. PURPOSE: To determine the effect of the VSPA interval on the mandated code 1 (< 9 min) and code 2 (< 13 min) response time interval compliance in an urban, public-utility model system. METHODS: A prospective, ob

Difficult calls to emergency medical dispatch centres - a mixed method study.

BACKGROUND: At emergency medical dispatch centres (EMDCs) telephone triage takes place in three steps: identifying the event, assessing the caller’s need for support, and prioritizing the response. Some calls are considered to be more difficult to handle than others, and decision support systems may in these situations be of limited help. The aim of this study was to describe and characterize difficult calls to EMDCs. METHODS: Retrospective call data from 2022 to 2023 was extracted for Registered Nurse (RN) dispatchers at three EMDCs in Sweden agreeing to participate in this mixed-method study
