Privatization and emergency medical services.

Osborne and Gaebler's Reinventing Government has sparked discussion amongst elected officials, civil servants, the media, and the general public regarding advantages of privatizing government services. Its support stems from an effort to provide services to municipalities while reducing taxpayer expenditure. Many echo the sentiment of former New York Governor Mario Cuomo, who said, "It is not government's obligation to provide services, but to see that they're provided." Even in the area of public safety, privatization has found a "market." In many localities, privatizing Emergency Medical Ser

Inequities in urban emergency medical services.

In the United States, systems such as Emergency Medical Services (EMS) provide rapid aid and medical care during emergencies and crisis events. EMS operations purportedly treat all callers equally, noting that certain information, such as select demographics, remains veiled until on the scene. However, disparities still persist, highlighting the need for further research investigating whether and how pre-existing inequities affect EMS response efforts. Here, we investigated differences in response times in New York City through regression analyses to (1) examine statistical disparities and (2)

Emergency Medical Services Utilization by Homeless Patients.

Homelessness is a rapidly growing issue throughout the United States and has important public health implications. Los Angeles, like other large urban cities, has seen a recent increase in homelessness. However, little is known about emergency medical service (EMS) utilization by those experiencing homelessness. Objective: Describe the utilization of emergency medical services by homeless patients. Methods: This is a 12-month retrospective review of electronic health records of all 911-incidents attended by the Los Angeles Fire Department (LAFD) from January to December 2018. The City of Los A

Physician medical oversight in emergency medical services: where are we?

OBJECTIVE: The objective of this study was to quantify the amount of direct contact with medical direction that nationally registered emergency medical services (EMS) professionals receive. The secondary objective was to determine whether differences in medical director contact were associated with work-related characteristics. METHODS: As part of biennial reregistration paperwork, nationally registered EMS professionals reregistering in 2004 were asked to complete a survey regarding medical direction. There were three survey questions asking participants to indicate, on a five-point scale, h

[New possibilities in emergency medical transportation and emergency services of Polish Medical Air Rescue].

In Poland, two types of medical services are accomplished by the Medical Air Rescue (MAR) operating all over the country: emergency transport from the incident scene to hospital and inter-hospital transport. Helicopters or planes are used for this purpose. In 2009, helicopters performed 4359 flights to incidents and 1537 inter-hospital transports whereas planes performed 589 inter-hospital ambulance and 196 rescue flights. MAR operates from 17 bases of the Helicopter Emergency Medical Service (HEMS) and one airbase. Helicopters are mainly used when medical transport is emergent, within the ope

Prehospital Factors Associated With Low-Acuity Status Among Pediatric Emergency Medical Services Transports in a Multicenter Emergency Medical Services Sample.

OBJECTIVE: To characterize rates of low-acuity emergency medical services (EMS) transports in children and identify factors associated with this status within a regional EMS system. METHODS: We performed a retrospective study of children (<18 years) transported by 242 EMS agencies to one of 17 hospitals between 2014 and 2023. Our outcome was a low-acuity status, defined as an ED discharge without advanced prehospital or ED interventions, medications beyond non-opioid analgesics or antipyretics, imaging or blood testing. We characterized rates of low-acuity encounters and used elastic-net pena

Emergency Medical Services Provider Self-Reported Occupational Safety.

BACKGROUND: Emergency medical services (EMS) providers are at high risk for occupational violence, and some tactical EMS providers carry weapons. METHODS: Anonymous surveys were administered to tactical and nontactical prehospital providers at 180 prehospital agencies in northeast Ohio between September 2018 and March 2019. Demographics were collected, and survey questions asked about workplace violence and comfort level with tactical EMS carrying weapons. RESULTS: Of 432 respondents, 404 EMS providers (94%) reported a history of verbal or physical assault on scene, and 395 (91%) reported wo

Helicopter emergency medical services response to equestrian accidents.

BACKGROUND: Horse riding is a common leisure activity associated with a significant rate of injury. Helicopter emergency medical services (HEMS) may be called to equestrian accidents. Accurate HEMS tasking is important to ensure appropriate use of this valuable medical resource. We sought to review HEMS response to equestrian accidents and identify factors associated with the need for HEMS intervention or transport of the patient to a major trauma centre. METHODS: Retrospective case review of all missions flown by Kent, Surrey & Sussex Air Ambulance Trust over a 1-year period (1 July 2011 to

Pediatric Rural Interfacility Transfers by Emergency Medical Services.

OBJECTIVES: Interfacility transports (IFTs) are critical for the care of pediatric patients in the United States (US), particularly in rural areas that may have limited pediatric specialty resources. Given the paucity of data describing rural pediatric IFTs, we aimed to characterize pediatric rural IFTs in the US. METHODS: We conducted a retrospective, observational study using the 2023 National Emergency Medical Services Information System dataset. Rural IFTs involving patients aged >1 day to <18 years were included and stratified into the following age groups: infant (1 day to <1 year), tod

Considerations for Multiteam Systems in Emergency Medical Services.

OBJECTIVE: Despite good intentions, mishaps in teamwork continue to affect patient's lives and plague the medical community at large and Emergency Medical Services (EMS) in particular. Effective and efficient management of patient care necessitates that sets of multiple teams (i.e., multiteam systems [MTSs] - EMS ground crews, EMS air crews, dispatch, and receiving hospital teams) seamlessly work together. Although advances have been made to improve teams, little research has been dedicated to enhancing MTSs especially in the critical yet often under studied domain of EMS. The purpose of this

Medical emergency motorcycle--is it useful in a Scandinavian Emergency Medical Service?

BACKGROUND: Medical emergency motorcycles (MEM) can be used in time-critical conditions like cardiac arrest and multi-traumatized patients in an attempt to reduce the response time. Other potential benefits with MEM are more efficient patient evaluation, reduction of unnecessary EMS car ambulance missions and reduced cost. The potential benefits have been evaluated in this study. The incidence of accidents when operating the vehicle was also of interest. METHODS: A prospective study was performed when MEM was introduced as a trial in an urban ambulance service in Norway. RESULTS: A total of

Prehospital Aspirin Delivery: Emergency Medical Dispatcher-Directed versus Emergency Medical Services Field Provider-Directed Aspirin Administration.

INTRODUCTION: For suspected acute myocardial infarction (AMI) and unstable angina patients, prehospital aspirin (ASA) administration has been the standard of care by Emergency Medical Services (EMS) field providers. Recently, Emergency Medical Dispatchers (EMDs), using Medical Priority Dispatch System (MPDS), provide telephone instructions to qualifying suspected AMI patients to take ASA, prior to EMS field provider arrival. No formal studies exist that measure time saved from earlier Dispatcher-Directed Aspirin Administration (DDAA). OBJECTIVES: The primary objectives of the study were: (1)

Aggressive Behavior Risk Assessment Tool for Emergency Medical Services.

OBJECTIVES: Emergency medical services (EMS) clinicians are at risk of on-the-job violence that can result in serious injuries. A majority of such violence is associated with underlying behavioral health emergencies. However, no simple screening tools have been validated for use in prehospital settings. This study aimed to examine the utility of the 7-item Aggressive Behavior Risk Assessment Tool for Emergency Departments (ABRAT-ED) modified for prehospital settings in identifying patients at risk for violence. METHODS: This prospective study included adult and pediatric patients aged ≥10 yea

[For an institutional outreach approach in emergency medical services].

FOR AN INSTITUTIONAL OUTREACH APPROACH IN EMERGENCY MEDICAL SERVICES. The "outreach" approach has become the new paradigm of social action. With the creation of regional health agencies, the decompartmentalization between curative medicine, preventive medicine and medico-social care is accelerating. The political will to transpose social tools to the medical field requires removing the confusion between the logic of "outreach" (of Samusocial) and "outside the walls" (of Samu). Especially since Samu, as interface organizations, are privileged hospital actors for this decompartmentalization. The

Statewide Emergency Medical Services Protocols for Status Epilepticus Management.

Although seizures are common in prehospital settings, standardized emergency medical services (EMS) treatment algorithms do not exist nationally. We examined nationwide variability in status epilepticus treatment by analyzing 33 publicly available statewide EMS protocols. All adult protocols recommend intravenous benzodiazepines (midazolam, n = 33; lorazepam, n = 23; diazepam, n = 24), 30 recommend intramuscular benzodiazepines (midazolam, n = 30; lorazepam, n = 8; diazepam, n = 3), and 27 recommend intranasal benzodiazepines (midazolam, n = 27; lorazepam, n = 3); pediatric protocols also freq
