Who's boarding in the psychiatric emergency service?

INTRODUCTION: When a psychiatric patient in the emergency department requires inpatient admission, but no bed is available, they may become a "boarder." The psychiatric emergency service (PES) has been suggested as one means to reduce psychiatric boarding, but the frequency and characteristics of adult PES boarders have not been described. METHODS: We electronically extracted electronic medical records for adult patients presenting to the PES in an urban county safety-net hospital over 12 months. Correlative analyses included Student's t-tests and multivariate regression. RESULTS: 521 of 536

Pathological gambling and the psychiatric emergency service.

OBJECTIVE: Pathological gambling (PG) has been associated with several negative mental health outcomes. We attempted to assess the impact of PG at the level of the psychiatric emergency service (PES). METHODS: In a first trial, clinical and demographic data were acquired from patients visiting the PES of a major university teaching hospital in downtown Montreal from July 1, 1996, to December 31, 2000. In a second trial, data were simultaneously acquired for a 2-year period in the above PES and in 3 others, beginning in September 2002. RESULTS: In the first trial, from 2000 onward, the number

Alcohol dependence among patients admitted to psychiatric emergency services.

We assessed the prevalence of alcohol dependence among patients examined in the psychiatric emergency service of a general hospital. We compared socio-demographic data and psychiatric status of patients with and without alcohol dependence. One-hundred and four consecutive patients received by the psychiatric emergency service of Bichat-Claude Bernard Hospital (Paris, France) were assessed. Diagnosis of alcohol dependence, acute alcohol intoxication, and antisocial personality was determined according to DSM-IV criteria. Other psychiatric disorders were identified using a structured psychiatric

[Work satisfaction and exhaustion among psychiatrists in psychiatric emergency services].

BACKGROUND AND AIM: Psychiatric emergency services (PES) have recently occupied a new and growing place in the landscape of mental health services. However, few data are available on the way psychiatrists practice in the PES. Our aim was to survey psychiatrists working in PES, focusing on their job satisfaction and the theoretical models they use in their everyday practice. METHOD: We sent a survey to a sample of 508 psychiatrists working in PES in France. RESULTS: Two hundred and thirty-seven psychiatrists returned the survey, yielding a response rate of 47%. On a 0 to 10 scale, the mean le

Utilization of psychiatric emergency services by homeless persons in Spain.

OBJECTIVE: Studies examining the relationship between homeless persons and the use of psychiatric emergency services (PES) in a country with universal access to health care are lacking. This study aimed to determine the prevalence of homelessness in adults visiting a PES in Spain, identify the differences between homeless and non-homeless patients in the use of PES and analyze the factors associated with homelessness and the decision to hospitalize. METHOD: The study included a total of 11 578 consecutive admissions to a PES in a tertiary hospital in Barcelona, Spain, over a 4-year period. Da

"Oldest Old" Attending Psychiatric Emergency Services: A Monocentric Retrospective Study.

"Oldest old", although increasingly numerous, remain insufficiently described in mental health services. By studying those who visit the busiest Psychiatric Emergency Services (PES) in France, our primary objective was to describe the "oldest old" seeking psychiatric care and, secondly, to identify predictive factors of hospitalization. We chose a cut-off age of 80 years and recruited all patients who visited our monocentric PES over a five-year period between 2018 and 2022. This retrospective observational study relied on clinical assessments and medical records. A total of 306 visits from 27

Elevated allostatic load in individuals presenting at psychiatric emergency services.

OBJECTIVE: Individuals requiring psychiatric emergency services are often highly distressed and intoxicated. To provide an objective and comprehensive measure of their stress-related physiological dysregulations, we indexed allostatic load with 14 biomarkers collected within 24 h of patients' admission to the largest psychiatric hospital in the Canadian province of Quebec. METHODS: This study (N = 278) combines data for emergency patients (n = 76; 65.8% women; M age = 44.97, SE = 1.6) and hospital workers who served as sex- and age-matched controls (n = 202; 70.8% women; M age = 40.10, SE = 0

Use of restraints and pharmacotherapy in academic psychiatric emergency services.

Psychiatric emergency services (PES) are an increasingly important component of mental health services. To assess the type and scope of services delivered in the PES setting, the American Association for Emergency Psychiatry sponsored an Expert Consensus Panel Survey of these services in 1999. The questionnaire was mailed to medical directors of PES facilities with 91% (n = 51) responding. More than 90% of the respondents were teaching sites. Restraints were reportedly used in a mean of 8.5% of presentations for a mean of 3.3 h per episode. Restraint utilization correlated with the percentage

The disruptive behavior disorders in the psychiatric emergency service.

Many children and adolescents presenting to the Psychiatric Emergency Service (PES) are diagnosed with the Disruptive Behavior Disorders (DBD), Conduct Disorder (CD), and Oppositional Defiant Disorder (ODD). Sometimes it may be difficult to reliably diagnose these disorders in the PES setting. Given these limitations, a large database of 6 years of PES visits showed 314 patients with DBD compared with 1625 without DBD. More DBD patient visits required the intervention of police and/or the mobile crisis team. These patients are more likely to have additional diagnoses of depression and attentio

Police referrals at the psychiatric emergency service in Taiwan.

INTRODUCTION: The police are the frontline workers in crisis situations involving patients with severe mental illness and act as a primary referral source for psychiatric emergency services (PES) in the community. The aims of this study were to investigate the distribution and characteristics of police referral among psychiatric patients in Taiwan. METHODS: The study cohort consisted of patients who visited the PES of Taipei City Psychiatric Center from January 2009 to December 2010. The associations between the factors of demographics, clinical characteristics, and psychiatric service utiliz

Factors affecting psychiatric inpatient hospitalization from a psychiatric emergency service.

OBJECTIVE: As a gateway to the mental health system, psychiatric emergency services (PES) are charged with assessing a heterogeneous array of short-term and long-term psychiatric crises. However, few studies have examined factors associated with inpatient psychiatric hospitalization following PES in a racially diverse sample. We examine the demographic, service use and clinical factors associated with inpatient hospitalization and differences in predisposing factors by race and ethnicity. METHOD: Three months of consecutive admissions to San Francisco's only 24-h PES (N = 1,305) were reviewed

Managing bipolar youths in a psychiatric inpatient emergency service.

Among the youths referred to our Psychiatric Inpatient Emergency Service, we focused on bipolar disorder (BD), to explore predictive elements for the outcome. Fifty-one patients (30 males, 21 females, age range 8-18 years, mean age 14.2 ± 3.1 years) received a diagnosis of BD, according to historical information, prolonged observations, and a structured clinical interview (K-SADS-PL). Twenty-seven patients (52.9%) were responders at the end of hospitalization according to CGI-I 1 or 2 and 50% decrease of both Brief Psychiatric Rating Scale (BPRS) and Young Mania Rating Scale (YMRS). Responders

[Marginal case of a practice fee for psychiatric emergency service].

Since 1 January 2004, the German Act for the Modernisation of Health Insurance (GMG) has regulated collection of a so-called practice fee-a charge to patients for their first outpatient health care in any given quarter. We report the case of a slightly inebriated patient who, against his will, was taken to a psychiatric outpatient department by local authorities, where he had to pay the quarterly charge of 10 EUR. Considering that he was forced and therefore had to pay the fee against his will, the present regulation is being discussed with respect to legal and moral implications in psychiatri

Prevalence of psychiatric diagnoses among frequent users of rural emergency medical services.

OBJECTIVE: This study aimed to determine whether there was an increased prevalence of psychiatric disorders among frequent users of rural emergency medical services. METHODS: In a matched comparison design, I compared frequent users of the emergency departments of 2 rural hospitals, both affiliated with an academic centre, with randomly selected users and with randomly selected users who had the same medical diagnoses. The main outcome measures were psychiatric diagnoses on a structured clinical interview, along with medical diagnoses and number of emergency department visits in the past year

Race, quality of care, and antipsychotic prescribing practices in psychiatric emergency services.

OBJECTIVE: The study examined whether the prescription practices of clinicians in psychiatric emergency services differed for African-American patients. Prescription of antipsychotic medications and its relation to quality of care was a particular focus. METHODS: Data from 442 independently observed evaluations of patients in psychiatric emergency services were examined using multivariate analyses. The observations were made during a five-year period at four urban general hospitals in California. RESULTS: Clinicians in the four emergency services, most of whom were Caucasian, prescribed more
