BACKGROUND: The current opioid overdose crisis in North America is heightening awareness of the need for and the challenges of implementing harm reduction, notably within complex and diverse settings such as homeless shelters. In this paper, we explore the implementation of harm reduction in homeless shelters during an emerging overdose emergency.
METHODS: The objective of this qualitative study was to identify and understand micro-environment level factors within emergency shelters responding to homelessness and substance use, and the macro-level influences that produce and sustain structura

This study analyzes the COVID-19 homelessness response in King County, Washington, in which people were moved out of high-density emergency shelters into hotel rooms. This intervention was part of a regional effort to de-intensify the shelter system and limit the transmission of the virus to protect vulnerable individuals experiencing homelessness. This study used quantitative and qualitative methods to describe the experiences of and outcomes on individuals who were moved from shelters to noncongregate hotel settings. The study highlights a new approach to shelter delivery that not only respo

INTRODUCTION: Older adults represent the fastest growing segment of the homeless community. Little is known about the prevalence of dementia and mild cognitive impairment (MCI) in this population.
METHODS: Dementia and MCI screening using the Montreal Cognitive Assessment (MoCA) was incorporated into the standard senior evaluation for adult clients aged ≥ 55 in a large emergency homeless shelter.
RESULTS: In a 6-week period, 104 of 112 (92.9%) assessments were positive for dementia or MCI using a standard cutoff of 26, and 81 (72.3%) were positive using a conservative cutoff of 23. There was

Background In the United States, homelessness is an issue that may affect a significant portion of the adolescent population. There is no consensus on the extent to which this population has been impacted by poor mental health and lack of resources. This study aimed to characterize trends among those who struggle with housing insecurity and mental illness to provide a clearer picture of mental health needs among this population. Methods Data from 641 adolescents who presented to a local adolescent homeless shelter between 2015 and 2021 were utilized to determine if there were significant assoc

BACKGROUND AND OBJECTIVES: In 2012, Massachusetts changed its emergency shelter eligibility policy for homeless families. One new criterion to document homelessness was staying in a location "not meant for human habitation," and the emergency department (ED) fulfilled this requirement. Our aim for this study is to analyze the frequency and costs of pediatric ED visits for homelessness before and after this policy.
METHODS: This is a retrospective study of ED visits for homelessness at a children's hospital from March 2010 to February 2016. A natural language processing tool was used to identi

Domestic violence (DV) shelters play a critical role for survivors and lead to a range of positive outcomes for families. However, transitioning in and out of temporary homelessness involves a range of stressors and research indicates that survivors transitioning out of DV shelters continue to have a multitude of needs. The focus of this study is to utilize an empowerment-based feminist lens to understand the process of transitioning out of a DV emergency shelter among 27 shelter residents in urban emergency shelters. Participants described the successes, challenges, and lessons learned while

As urban development accelerates and natural disasters occur more frequently, the urgency of developing effective emergency shelter planning strategies intensifies. The shelter location selection method under the traditional multi-criteria decision-making framework suffers from issues such as strong subjectivity and insufficient data support. Artificial intelligence offers a robust data-driven approach for site selection; however, many methods neglect the spatial relationships of site selection targets within geographical space. This paper introduces an emergency shelter site selection model t

The destructive aftermath of storms and typhoons in the Philippines more prominently affects isolated remote locations that lack methodical and strategic procedures to address the situation. One form of assistance provided during disasters is the construction of shelters, which can be categorized as emergency, temporary, or transitional, serving as protection during recovery periods. Cold-formed steel (CFS) is widely used, and its versatility in fabrication and configurability make it an ideal candidate for building emergency shelters that are lightweight, easy to assemble, upgradeable, and co

INTRODUCTION: Whole Health programs aim to integrate mental, physical, and social health to improve veteran well-being. Homeless-experienced veterans may face barriers to engagement owing to housing instability and comorbidities. This study evaluated Whole Health participation among homeless-experienced veterans at the VA Greater Los Angeles Care, Treatment, and Rehabilitation Service site compared with that among the broader VA Greater Los Angeles Healthcare System veteran population.
METHODS: A retrospective quality improvement study analyzed Whole Health participation among 619 homeless-ex

The COVID-19 pandemic dramatically increased domestic violence (DV) rates and structurally changed how social services like emergency DV shelters could be provided. We surveyed employees at 103 U.S. DV shelter providers about how their organization experienced the COVID-19 pandemic, service provision impacts, and programmatic changes and adaptations. Six main categories emerged from the inductive content analysis: (1) increased need for services, (2) difficulties accessing shelter, (3) modification of the shelter environment, (4) technological adaptation, (5) provider welfare concerns, and (6)

While noncongregate shelters are an increasingly common intervention for homelessness, their effect on residents' psychological well-being is not well understood. This study evaluated the effect of a low-barrier, noncongregate shelter on the well-being of individuals experiencing homelessness in a small Northern California city in 2023. Focusing on 5 key areas of well-being-sleep quality, sense of community, sense of control, generalized anxiety, and psychological safety-this study surveyed 106 (of 170) shelter residents 9 months after the opening of the shelter. A retrospective pretest-postte

Despite steady increases in homelessness in the U.S., only recently has research on transportation needs and use for persons experiencing homelessness (PEH) been the focus of research endeavours. Moreover, limited research has identified how the geographic relocation of homeless community services and resources impacts the transportation needs of PEH and how this process affects access to health and social services. To fill this gap in the literature, using a community-engaged research approach, we conducted a qualitative research study in which 24 professionals working in planning, transporta

STUDY OBJECTIVE: Housing instability is prevalent among emergency department (ED) patients and is known to adversely affect health. We aim to determine the incidence and timing of homeless shelter entry after an ED visit among patients who are not currently homeless.
METHODS: We conducted a random-sample survey of ED patients at an urban public hospital from November 2016 to September 2017. Patients provided identifying information and gave informed consent for us to link their survey data with the New York City Department of Homeless Services shelter database. Shelter use was followed prospe

PURPOSE: To compare the outcomes of ophthalmic surgical emergencies during shelter-in-place (SIP) order with the corresponding period in 2019.
METHODS: This retrospective cohort study compared patients presenting to the Bascom Palmer Eye Institute (BPEI) emergency department (ED) who underwent urgent surgery during the SIP period (March 23-May 17, 2020), compared to the same weeks in 2019 (non-SIP). Main outcome measures included symptom-to-ED time, ED-to-surgical decision time, surgical decision-to-operating room (OR) time, ED-to-OR time, and postoperative follow-up time. Secondary outcome m

BACKGROUND: Domestic and sexual violence have been linked to adverse gynecological and obstetric outcomes. Survivors often find it difficult to verbalize such violence due to feelings of shame and guilt. Vulnerable or socially excluded women are frequently excluded from research, particularly qualitative studies on violence. This study aimed to characterize the perceived impact of domestic or sexual violence on the gynecological health and follow-up among women with complex social situations.
METHODS: We analyzed the data following inductive thematic analysis methods.
RESULTS: Between April

BACKGROUND: Residents and staff of emergency shelters for people experiencing homelessness (PEH) are at high risk of infection with severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2). The importance of shelter-related transmission of SARS-CoV-2 in this population remains unclear. It is also unknown whether there is significant spread of shelter-related viruses into surrounding communities.
METHODS: We analyzed genome sequence data for 28 SARS-CoV-2-positive specimens collected from 8 shelters in King County, Washington between March and October, 2020.
RESULTS: We identified at leas

This paper explores the nature of voluntary sector 'spaces of care.' In particular, the paper is concerned with spaces of care that have been established in response to urban homelessness. These include service environments such as emergency shelters, drop-in centres, and soup kitchens. Renowned for being health affirming, these environments also function as important political spaces in the city. One site in particular, a low-barrier emergency shelter, is examined in detail. This site's political significance is traced to the way in which it partakes in boundary work by defining who is worthy
