BACKGROUND AND PURPOSE: Several authorities have recently advocated carotid stenting for recurrent carotid stenosis because of the perception that redo surgery has a higher complication rate than primary carotid endarterectomy (CEA). This study compares the early and late results of reoperations versus primary CEA.
METHODS: All reoperations for recurrent carotid stenosis performed during a recent 7-year period by a single vascular surgeon were compared with primary CEA. Because all redo CEAs were done with polytetrafluoroethylene (PTFE) or vein patch closure, we only analyzed those primary CE

Carotid endarterectomy is a surgical procedure to remove atherosclerotic occlusions from the carotid artery. The surgery is usually performed in patients with transient ischemic attacks (TIAs), asymptomatic stenosis, or stroke in order to reduce stroke risk and increase cerebral blood flow. Complication rates vary widely among surgeons and hospitals, and numerous studies testing the efficacy of carotid endarterectomy and documenting its associated morbidity and mortality have produced conflicting or inconclusive results. No properly designed prospective clinical trial has convincingly demonstr

Thousands of carotid endarterectomies are now performed each year. In properly selected patients in the hands of competent surgeons, the mortality and morbidity rate should not exceed 2 per cent. Although the long-term results are good, this procedure is prophylactic, and a patient who has already sustained a fixed neurologic deficit will rarely be improved by increasing the blood flow in the carotid arteries. This procedure, probably more than any other in the armamentarium of the vascular surgeon, depends on meticulous attention to detail and careful and gentle handling of tissue.

PURPOSE: This study is an analysis of the outcome of a common method of management of the external carotid artery (ECA) during routine carotid endarterectomy (CEA).
METHODS: Between 1986 and 1997, 1069 primary CEAs were performed with a combination of proximal eversion technique and blind distal endarterectomy on the ECA. Of these, 973 CEAs (91%) had 1 or more postoperative duplex scans that included the ECA. Both preoperative and early postoperative studies were performed on 313 of these CEAs. Intraoperative post-CEA continuous-wave Doppler scans identified low flow or occlusion of the ECA i

This review focuses on the natural history of symptomatic and asymptomatic carotid stenosis and on the risk/benefit ratio of carotid endarterectomy. Five randomized trials for symptomatic stenosis and four trials for asymptomatic carotid occlusive disease are reviewed. Carotid endarterectomy has been proved effective in reducing stroke risk in symptomatic 70-99% stenosis. Conclusive results are still pending for moderate symptomatic stenosis and the asymptomatic lesion. Surgical controversies and techniques are discussed. Algorithms for management of symptomatic and asymptomatic stenosis are o

A retrospective review of carotid endarterectomies performed by the Neurosurgical Service at Massachusetts General Hospital from July, 1976, through December, 1985, disclosed 64 procedures that were performed on an emergency basis. The patients included 40 men and 24 women, with a mean age of 64 years (range 32 to 87 years). Correlation of angiographic findings with outcome revealed that of the 27 patients with severe stenosis, usually with delay in blood flow, 25 (93%) were the same or improved postoperatively; of the 11 patients with stenosis and an intraluminal filling defect (six of whom h

Forty-nine external carotid revascularization procedures were performed in 36 patients. Indications included transient ischemic attacks, amaurosis fugax, and the procedure as a prelude to extracranial-to-intracranial bypass. Twenty patients had a preoperative neurologic defect. Twelve patients were classified as having bilateral and 24 as having unilateral carotid disease. Unilateral external carotid endarterectomy was performed in 29 patients, with the remaining patients undergoing procedures for additional occlusive lesions. Five patients had operation-related strokes (one death). Three of e

External carotid endarterectomy may be useful in the management of selected patients who have internal carotid occlusion, persistent neurologic symptoms and additional atheromatous ulceration or stenosis within the carotid bifurcation. The procedure should be associated with a low risk for intraoperative stroke and a reasonable likelihood for symptomatic improvement. If symptoms are not completely corrected by external carotid endarterectomy, superficial temporal artery-to-middle cerebral artery bypass may be performed with greater confidence in a successful hemodynamic result.

Atherosclerotic carotid artery disease remains an important cause of cerebrovascular ischemic disease. We present a patient with residual stenosis of the distal internal carotid artery following carotid endarterectomy that was treated with stenting. The case highlights the potential complimentary benefits of carotid endarterectomy and carotid stenting.

BACKGROUND: Severe narrowing (stenosis) of the carotid artery is an important cause of stroke. Surgical treatment (carotid endarterectomy) may reduce the risk of stroke, but carries a risk of operative complications.
OBJECTIVES: To determine the balance of benefit versus risk of endarterectomy plus best medical management compared with best medical management alone in patients with a recent symptomatic carotid stenosis (i.e. transient ischaemic attack (TIA) or non-disabling stroke).
SEARCH STRATEGY: We searched the Cochrane Stroke Group Trials Register (July 2010), MEDLINE (1966 to March 201

The release of several randomized trials comparing carotid endarterectomy (CEA) to other methods of stroke prevention in the early 1990s established CEA as the "gold standard" in the prevention of stroke from carotid occlusive disease. This study examines 510 of the CEAs performed by the first author at Charleston Area Medical Center in Charleston, W. Va., from 1991-99, which were part of three prospective randomized CEA trials at CAMC. All patients were observed clinically and underwent postoperative color duplex ultrasound scans at 30 days, six months, 12 months, and every year thereafter to

BACKGROUND: Restenosis after carotid endarterectomy (CEA) is associated with an increased risk of stroke, and the management of critical or symptomatic restenotic lesions poses a treatment challenge. The superiority of CEA vs carotid angioplasty and stenting (CAS) for restenosis remains debatable because existing studies are few and limited by small sample size or the inability to align interventions with ipsilateral events beyond the periprocedural period. We performed a population-based evaluation of CEA vs CAS in a large contemporary cohort of patients with carotid artery restenosis.

Stroke, either ischemic or hemorrhagic, remains the second commonest cause of death worldwide in the last decade. Etiologies for ischemic stroke (IS) vary widely. Atherothrombotic occlusion is an essential cause to which carotid artery stenosis (CAS) is a major contributor. Administration of anti-platelet agent to patients with CAS has been shown to reduce incidence of long-term IS. In additional, in patients with symptomatic CAS, clinical trials have demonstrated that carotid endarterectomy (CEA) is superior to medical therapy for prevention of future CAS-related IS. However, CEA is not suita

The syndrome of inappropriate secretion of antidiuretic hormone after carotid endarterectomy is very rare; only two cases have been reported in medical literature. We describe the case of an 82-year-old woman presenting with lethargy and drowsiness due to severe hyponatremia with urine hyperosmolarity and plasma hypo-osmolarity after carotid endarterectomy.

HYPOTHESIS: The North American Symptomatic Carotid Endarterectomy Trial and the European Carotid Surgery Trial demonstrated that a greater benefit from carotid endarterectomy (CEA) was seen in elderly compared with younger patients. However, no patients older than 89 years were included in either study. We hypothesized that CEA is safe and effective in patients 89 years and older.
DESIGN AND SETTING: This is a retrospective review of 3 neurosurgeons' CEA experience with nonagenarian patients.
PARTICIPANTS AND INTERVENTIONS: Of our 1800 patients who underwent CEA, 26 were 89 years or older. T

Surgery of the extracranial vascular bed involves in the first line surgery of the carotid arteries. It is difficult to separate the problems of surgery of the other arteries from the carotid surgery. This study focuses on carotid surgery as well. Cerebral stroke, however, is much more frequently based on embolization from the area of stenosis than on the hemodynamic influence of the stenosis alone (Ref. 17).

BACKGROUND: Since the validation of carotid endarterectomy (CEA) as an effective means of stroke prevention, there has been renewed interest in its best indications and methods, as well as in how it compares to carotid angioplasty and stenting (CAS). This review examines these topics, as well as the investigation of carotid stenosis and the role of auditing and reporting CEA results.
INVESTIGATION: Brain imaging with CT or MRI should be obtained in patients considered for CEA, in order to document infarction and rule out mass lesions. Carotid investigation begins with ultrasound and, if resul
